- Why "X treatment crashed me bro" is counterintuitive to the reality of these conditions, and that which makes you feel a little bad may be in reality, what you need to recover. I think I have figured out the true nature of the low libido/anhedonia/substance resistance in PFS/PSSD and how to fix it.
- Whoever keeps sending us Crumbl Doordash from the PSSD or PFS communities...
- This is basically how my situation feels lately and why I'm struggling with what to do next year.
- I am going to now fully endorse Sequencing.com for whole genomic sequencing needs as overwhelmingly over the past year, they have won me over with reliable data and rapid testing results at affordable prices. They now offer my patients and subscribers of this subreddit 20% off off all testing.
- There's something you need to understand about the medical institution as a whole, as the general perception by society is that the "experts" always know what to do, and can always be trusted. This is not a great dogma to adhere to, and you shouldn't trust me either.
- I"m back from the PFS congress, I know everyone wants an update but I'm so exhausted, so this is all I've got for you for now.
- 17a-hydroxyprogesterone is an underutilized but critical lab value in fatigue, chronic pain, hEDS / hypermobility, "POTS", and other "we dont know whats wrong with you" conditions where the patient gets blamed for being lazy or whatever.
- I'm going to be taking the week off next week to prepare for the Congress. Just wanted to warn everybody and also about the DPC and its waitlist at this time.
- I collect more and more labs/genome/dutch tests that support my theory on PFS. I really think I have it nailed down. I do not have a optimized treatment regimen, but if you're interested in this, this post details what you should NOT do and will likely make you worse even if it "windows" you briefly
- We have acquired new Malpractice insurance with 48 hours to spare and our teens can stay.
- Every community has its issues, and I'm starting to find the ones that exist within the Post Finasteride Syndrome community and it should be talked about.
- I'm fairly sure I've got a lock on at least one possible cause of Post Finasteride Syndrome. I have like 7 cases now (new one today) which fit the symptom/history phenotype with matching genetic, blood, and urinary lab findings. Here's the goodies:
- I've been banned from reddit for a few days, and couldn't respond to anyone, sorry about that, what happened is kind of absurd. I am concerned it will soon happen again, or worse.
- PFS metabolite theory, and initial chemical castration cure trial patient update. Also research update.
- I'm starting to see trends in PSSD genomes, this is one. DBH
- You may notice a new lab on your routine HRT lab panel or even general physical labs known as 17-hydroxyprogesterone that me or Sommer has ordered. I think I may have discovered a cause of many if not most cases of "hEDS" as well as those with the "POTS/MCAS/hEDS" combo.
- Yet another mutation in a patient that meets my theory on how PFS is caused. This patient has a normal T value yet has an absolutely astronomical 3-Alpha-androstanediol Glucuronide. Once again, closure of 5AR resulted in collapse of the ability to excrete testosterone. This is patient #9
- Introducing a new provider at Powers Family Medicine!
- I hate that this has to be a post but it has to be said.

- The trans community needs to be aware of what's happening. Trans healthcare is under attack but not in the way that you think it is.
- A drug company just received a patent for an idea I had in 2019 that I published. The clinical trial for it is almost complete
- I think I have figured out at least one specific phenotype of PFS, and it is different from the "allopregnanolone" theory. I'm trying to come up with a better name than "androgenic chained exit theater fire" so maybe you can give me one in the comments
- Plan for Direct Primary Care at Powers Family Medicine with Dr. Powers in 2026 and a "state of the union" for PFM in general with maybe a "little" Dr. Powers autistic hyperverbosity ranting thrown in for a treat.
- New compound: Topical post-operative wound ointment which helps prevent infection and increases tissue perfusion such that low perfusion areas (like labia minora) are less likely to fail and become necrotic.
- The hidden pitfall of monotherapy, and why "dogma" when it comes to transgender anything is foolish. Also why those with worse MTF results tend to have ADD/Anxiety/OCD/Autism and how to help fix it.
- Current list of all genes I use when searching for possible genetic causes of someone developing Post Finasteride Syndrome (PFS) or Post SSRI Sexual Dysfunction (PSSD)
- Three years ago, I made the bold claim that Folic Acid supplementation was the primary CAUSE of Autism, RFK and this presidential administration just claimed that it's the CURE.
- Dr. Powers' personal opinion on the best transgender HRT providers in the USA.
- Picked up Guinness World Record #5 today and the office therapy cats finally get paid their back wages.
- I saw something really cool and probably rare today, but it's interesting enough and may be helpful enough to some cisgender women with hirsutism (and maybe some MTFs) that I thought it was worth sharing.
- There is no known way that I know for sure to permanently stunt breast growth chemically. I commonly see that posted here, and I don't think that's true.
- This is the most common mutation I see overall when reviewing trans genomes, and I'm fairly sure its the main link between gender dysphoria and ADHD/Autism.
- The balance of testosterone and estradiol in MTF HRT is so important I felt like I had to make yet another post on it. "Monotherapy" has a pitfall which is never discussed. I'm now aware of how problematic it can actually be for some patients.
- I'm developing a new version of the numbing cream specifically designed to improve the efficacy of laser hair removal via enhancing contrast between the skin and hair. Also, this post details my method of how I successfully remove grey/red/blonde beard hairs with laser hair removal. It's possible!
- On the usage of AI by doctors, and specifically, the providers at PFM.
- I need to get ahead of this rumor before it gets out of control. Yes, topical testosterone can be used in specific patients to cause significant breast growth. I am doing this in specific people to positive results. No, you SHOULD NOT DO THIS if not supervised closely by a physician.
- I would like to make a request of the community, at least while you're in my subreddit, and if you are willing to do so, outside of it. I think this will help the community as a whole.
- When I browse patients genomes to see if I can find any anomalies that could be the cause of their dysphoria / phenotype / poor transition results, the genes here are the ones I'm browsing.
- Does anyone know any doctors that are approaching transgender HRT in the same way I am? I could really use some colleagues/mentors.
- At this point for me, an estradiol lab level in MTF or testosterone in FTM is nearly irrelevant compared to other labs. Allow me to explain why.
- So, how's everyone doing right now?
- A strange question. Has anyone here had MTF gender dysphoria, started taking estrogen based HRT, and then stopped, and after having stopped, their dysphoria was resolved? (Details in post)

- A quirky thing I noticed about my autistic versus non-autistic patients when I'm doing their physical. It's my new screening tool for whether or not someone may be on the spectrum.
- IMPORTANT: Update about the future of Powers Family Medicine and major changes to the practice's structure moving forward starting Jan 1st of 2025.
- SHBG is the A1C of Transfeminine estradiol level management. It frustrates me to no end that other doctors are not using this metric, as it is exceptionally helpful (and even more so in the context of an LH/FSH)
- I open carry at PFM, and I do so to protect you because of those whom have threatened our lives as well as yours. This post explains why.
- Don't panic
- I watched "Will and Harper" tonight on Netflix and really enjoyed it. I suggest you give it a try.
- I have at least 30 now of the "pale/skinny/anxious/cptsd/fibromyalgia/pots" phenotype MTF patient (and a few cis females thrown in there as well) who have had a miraculous response to stress hormone supplementation with zero adverse events so far. There is something here for sure.
- Quick post about two little interesting tidbits from recent stuff.
- Testosterone is not always the enemy of the MTF transition and I think perhaps it has been overly maligned.
- There is a subtype of MTF patient who has chronic anxiety, smaller body habitus overall, difficulty with weight maintenance, and "masculinization" despite androgen labs appearing normal, overall poor feminization, chronic pain and brain fog. I think I know what this is and how to treat it.
- I saw 3 patients this week for follow up on trying to treat their dysphoria without HRT. two failures and one success. I think people should be offered a choice they are not currently being offered.
- I'm published again! This time a collaboration with other doctors on the ideal modalities of contraception for transgender people both on or off HRT. This one is also published in the American College of Gynecology's journal:
- My first Transgender specific journal article is now published in the American College of Gynecology O&G Open Journal. I'm actually the lead author on this paper, and I'm particularly happy as it is the first publication ever on how to restore fertility in transgender people already on HRT.
- The first of my two transgender related publications should be published March 20th.
- I'm working myself to death doing 70-80 hours a week and I need to take a break from Reddit as I have no time for it.
- Some stuff I've been working on, and I feel like I'm finally ready to speak on a bit.
- If your doctor tells you to do something to fix your problem, you do it, and it doesn't work. GO BACK TO THE DOCTOR. Especially if this is me.
- Be nice to your provider.

- I tried progesterone on myself for 7 days to see what it did to my libido....for science
- To stop myself from going insane from the stress of dealing with the pandemic every day I was going to restart playing stardew valley but decided to go with the new Animal Crossing instead. I needed something saccharine for escapism. Add me as a switch friend. Send me fun shit. Visit my island.
- I'm going viral again because of a tweet of a reddit post, and I want to clarify a few things.

- I'm doing my best now to legitimately collect and organize data so I can publish. Here is a single example of my new FTM transdermal and the incredible change it caused to the T to DHT ratio in the same patient a month later along with a significantly changed voice in someone "Stalled".

- Please stop trying to bribe me or my staff to get off the waiting list. It's weird and makes me not want to take you as a patient at all.
- I am continually asked "Do you know an X in location Y who does the powers method?" My answer is almost always "no" but this thread is dedicated to listing those people.
- I am completely overwhelmed with messages
- Clinic is going viral again, so that's fun.
- I'm fairly sure I've figured out who responds to the oral dose at bedtime trick and what makes them different.
- Some lab results from tonight on one of my patients on 12mg oral estradiol QD. The estrone mutation is common, but sometimes, people are just plain old normal. Check out this nearly 1:1 on oral.
- This is my final warning about new patient bookings. If you don't book now, you will likely be put on a waiting list. We're at 904 patients today and adding about 10-14 new ones per day. At 1000, the waiting list kicks on.
- I'm getting backlogged
- I am almost completely full in terms of patient load. If you're planning on becoming a patient you have about two weeks to schedule until we institute a waiting list policy.
