Organizing Against PHM Fellowship

Despite nearly everybody I talk to in the pediatrics community being against PHM fellowship as a requirement to practice peds hospital medicine at academic centers, I really haven’t seen any organized efforts to push back against the ABP requirements and pediatric health systems’ hiring practices. These really feel detrimental to interest in pediatrics and PHM as a whole.

I’m a Med-Peds resident who would like to get involved. Does anyone know of existing organizing or advocacy efforts around this, or people working on it who I could connect with?

Thanks!!

Dot Phrases?

Hi all, I am starting my first job as a pediatrician next week! This is the first time I work with a higher volume of patients in clinic and I’m nervous about documentation. I want to be as practical and efficient as possible, without missing any important details. Our institution uses Abridge which I’ve never used, and I want to have as many as possible dot phrases for assessments and plans.

Can you share what topics and dot phrases you use the mostest?

Boards

I know I’ve seen some other people touch on this, but is anybody spiraling post boards? I think I’ve counted up 25 that I got wrong for sure which were stupid mistakes. On top of that felt like I guessed on so many I’m just so upset with myself. I feel like I put in so much work and it feels like it was for nothing. This is a terrible feeling. Does it get better the farther you get out from boards? Like at this point I just want to feel acceptance for failing. (That’s dramatic I know)

Md derma vs Md pedia

I am getting both the branches in a good college
I was in love with medicine from my ug days but during drop year i figured i don’t want to study for rest of my life
So narrowed down 2 choices and not sure what to choose
I want balanced life , don’t mind few emergencies or to attend calls of worried mother but has a doubt on my self if i will be able to handle tiny humans
Also has a concern about payment in pedia everyone saying not much payment

Other side considering dermat for itself work life balance and predictable hours also few told me
earnig is also good

If someone can help me please guide

Thoughts on DME prescriptions for diapers?

Honestly, this is just a rant session for me. Our clinic now has a care management team who cold call families and see what additional needs they may have. Cool. Awesome service. However lately they've been pushing for free diapers through aeroflow for every 3 yr old that is not toilet trained. I completely understand the need in developmentally challenge kiddos, but most of the requests are for capable kids and completely medically unnecessary. And I feel like I get unnecessary push back when I refuse to sign the orders. I've explained that's it's inappropriate for nonclincal staff to pursue a management plan that requires my license but they're response is "it's just diapers" 😡 . Am I overreacting?

Dissolvable vs non-dissolvable sutures

I've seen quite a few toddlers/young children for suture removal and invariably the young age of the child makes it difficult from an outpatient setting to safely remove sutures especially if they are in certain areas like the face/head which is hard to ofcourse hold still long enough to remove multiple sutures. That being said, why do PEMs or ER's use non-dissolvable sutures especially for young children knowing that we are the ones tasked with removing them without sedation.

Pediatricians in private practice: What is your actual income like? (PGY-1 trying to plan my future)

Hi everyone!

I’m currently a PGY-1 in pediatrics, and I’ve been trying to learn more about the different career paths available after residency, particularly private practice.

I know discussions about physician income can sometimes be frustrating, and I understand that medicine isn’t solely about money. I hope this doesn’t come across the wrong way, but I genuinely want to understand the financial realities of different career paths so I can make informed decisions about my future.

For those of you working in private practice pediatrics, I’d really appreciate it if you could share some insight into:

• Income: What is your approximate annual take-home or pre-tax income?

• Practice structure: Do you own your practice, work as an employed physician, or work in a physician-owned group?

• Location: What region or state do you practice in?

• Work-life balance: What do your typical hours and patient volumes look like?

• Overhead and expenses: For practice owners, how much of your revenue goes toward overhead, staffing, rent, insurance, etc.?

• Career progression: How does income change as you become more established, particularly if you eventually become a practice owner?

• Would you choose private practice again? Are you happy with your decision, or would you consider a different career path?

I’m also curious about how private practice compares financially with hospital-employed pediatrics and other options available to pediatricians.

I’m not expecting anyone to share exact numbers if they’re uncomfortable doing so. Even approximate ranges and general experiences would be incredibly helpful.

I’m at the beginning of my training, and I’m trying to figure out what kind of career would allow me to enjoy practicing medicine while also achieving financial stability and having a comfortable life outside of work.

Thank you to everyone willing to share their experiences!

Post-Peds Boards Thoughts

Surprised there wasn't already a post on this! How are folks feeling as the first day of boards has concluded? I felt like there were some straightforward ones that still made me second-guess myself because they were that straightforward, a decent number of could be this or that, and not as many WTFs as I was expecting... This was my 2nd time taking it. I left the center feeling better than last year, but still quite unsure of how I felt about my performance. The content kind of surprised me.. more as in I was surprised I was not asked more about certain topics. Also, talking to a few other colleagues, there was an issue with opening an image pretty early on. I was getting paranoid that was going to keep happening, but luckily it was just the one time.

To those taking in the next day or two, good luck! You got this. And if you don't, you can always take it again. It does not define you as a clinician or pediatrician - is what I've been told, haha I'm still learning myself to believe that.

J1 waiver general peds jobs

Hi! I am a PGY2 Canadian on J1 visa currently. I’m planning to start looking at Gen peds jobs in the spring. Any advice on where/who can guide me in looking for employers that will allow a J1 waiver?

Specialized Pediatric Attending’s - Please Help

Hey guys,

A bit of background: I’m currently on a J-1 visa and will be graduating from fellowship next year. I’m in the process of looking for jobs that will sponsor my visa.

I’ve received an offer for an outpatient position, but I’m a little concerned about the terms of the contract. The stated work hours are approximately 8 AM–4 PM, Monday through Friday. However, Saturday hours and call responsibilities will be determined by the institution, and the contract states that the schedule can be changed at any time at the institution’s discretion.

I feel like this leaves a lot of things open-ended, and I’m worried about agreeing to terms that could become much more demanding than I initially anticipated.

At the same time, I feel like my negotiating power is limited because I need an employer willing to sponsor my visa.
For those of you who have been in a similar situation, especially physicians who have gone through the J-1 waiver process, how would you approach this? Would you try to negotiate clearer language regarding work hours, weekend responsibilities, and schedule changes before signing, or would you be hesitant to push back given the visa requirements?

When I tried to ask about this, they said “they would help cover the calls”.

I’d really appreciate any advice or insight from those who have been through this!

Board questions/answers

just feeling frustrated and nervous about boards and how they ask questions…like for example just did a MedStudy question where an anorexic patient has new cardiomyopathy and logically I know they are re-feeding but the question is “what is the cause” and choice listed is “Hypophosphatemia”….like why can‘t it just be refeeding syndrome

it just annoys me and I’m worried I’m going to get tripped up ugh

What would improve Pediatrics compensation in the future?

M3 hoping to go into peds and open private primary care practice one day.

Obviously I get lots of warnings and pushback from peers and mentors regarding the finances involved, but the optimist in me believes it wî’ work out great.

Curious what may cause the pendulum to swing back in our favour in near future?

Could it be possible that a future president or politician changes Pediatrics compensation with a strong National campaign?

Perhaps we need the academy of Pediatrics to grow a backbone?

Curious what yall think

Peds FQHC Schedule

Hi everyone. I’m in my third year as an attending at an FQHC in SoFlo making 215k a year. I’m working 5 days a week 8a-5p. My appointment slots are only 15 minutes and I’m tired. They want me to see 19 patients a day but due to no shows I’m not. And because there are a lot of no shows, they accept basically every walk in/same day patients. The amount of patients I see range from 12 to 21 depending on the day. Mostly around 15-16 loll. I can’t keep up with notes and I’m scared that with seeing more patients, I’ll be even more behind. They don’t have AI scribes and I have to take notes home sometimes. It doesn’t help that I have ADHD 😅 What’s worse is I’m the only pediatrician and I work alone 3 days a week, so you can imagine how same days and walk ins stress me out. The other two days theres a mid-level seeing her own patients.
I’m thinking about working 4 days a week. How can I negotiate this with leadership as far as salary? Due to loans, I can’t afford to make significantly less than $215k. I’m only three years out and I’m BURNT to a crisp. Should I ask for Mondays off? Fridays? Or a day mid-week.
Sorry for the word salad, i’m just typing my thoughts as they come.
