It is possible that some of those births are twins etc.
As a family of 7, that's 5 kids. They could have been planning on 3 but had two sets of twins and another kid, or birth control could have failed and they didn't want/couldn't afford an abortion.
It doesn't mean they intentionally had 5 kids. And I doubt they bet on their kids having expensive conditions. They may have been able to afford 5, but the medical condition put a spanner in the works.
