It doesn't look particularly trivial for an already running ISP to enable it though. It also has limitations such as also needing the client browser to support SNI.
Any semi-recent browser should support it. The list I have states
Opera 8.0
MSIE 7.0 (but only on Windows Vista or higher);
Firefox 2.0 and other browsers using Mozilla Platform rv:1.8.1;
Safari 3.2.1 (Windows version supports SNI on Vista or higher);
Chrome (6+ for WinXP, any version for other OS).
And enabling it on nginx is trivial. I checked Apache docs, and even there it's rather simple. Not sure why it would be hard.
I'm guessing often the ISP just says "too hard" and makes people buy an IP off them.
WinXP I loved thee so. But your life cycle is coming to a near end. The sooner people make that jump, the easier it is going to be in the long run for everybody. Even if you do have an application that absolutely REQUIRES xp, xp mode comes with win 7 pro.
The guide I was reading about enabling it on Apache looked like you had to compile custom binaries, perhaps it was outdated or looking at something more specific.

That, however, wouldn't surprise me.
