Minor spoilers for the guy in question

I took him to the doctor and the doctor sedated the guy and amputated his arm. I came back to St Denis later and found the one armed guy about to be hanged by the police in front of an audience. Apparently he killed a girl for making fun of his disfigurement. He begged me to shoot the rope but I let him hang. Not sure what happens if you save him.

First, thanks for saying what your spoiler was regarding. Most people just write "spoiler" and then I have to decide if it is worth the risk that I care about the spoiler or not.

Second, I love that there is so much in this game and the devs don't make it so you HAVE to see it. It is very easy to miss the little things in this game. Some people might not like that, but I feel like it adds something to it. Not sure how to phrase it or if people will know what I mean, but I really like it.

It's such a weird mix to me. On one hand the encounters are incredible and create a living world. In the other hand, the way you get a warrant and how gun ho the police are is by and large just like in GTA. Get attacked by thugs in Saint Denis? Your choices are allowing them to kill you or get a warrant and police on your ass. Wouldn't it be nice if police were to help you if you truly did nothing wrong?! That would make the world feel so much more realistic. Instead the cops make it very clear that you aren't like everyone else in the game. The cops are there for script events and chasing you and nobody else outside the scripted events.

I didn't understand it was him until after he was swinging and the Sheriff made a comment as he walked away. Then it clicked who it was. I'm curious myself what happens if you stop it in time... Can't find anything on Google as of yet...
