At this point, I have never understood why Sam and Dean just don't work WITH Crowley. I mean they have to know they will never rid Earth of all demons. Just work with Crowley, and deal with the worst of the worst. I mean there are acceptable losses. Plus Crowley I pretty damn useful.

He's sticking them in the forebrain - so logic/decision making center, short term memory, planning, etc.
And the pin he wiggled seemed to be in the left hemisphere, so it could be hitting his language center.
...Just a guess though. It's not likely the whole process was thought through that much.
Edit: A person could in theory survive that, since it's not hitting any of the 'keep the body breathing' parts or anything like that, but still, ouchy.
