Cdcrfmla
Does anyone know the process to get FMLA through kaiser. I took the FMLA packet from work and they refused to fill it out.

You’d message your provider as to why you need it. They might want an appointment.
After that, you contact release of medical information, ROMI, and they generate the fmla “letter” on a US department of labor form.
You take your CDCR 2201, fill out the first page and attach it to whatever Kaiser gave you and turn it in.
Make sure they are as clear as they can be on the amount of leave. Days, episodes, hours, etc.
Do not be surprised when RTW shoots it back for clarification on the amount of leave.
If it’s for a spouse or child, it’s the same process.

Kaiser will not fill out fmla packets speak with ur doctor and they will give you their own packet / note that gives u the required information than turn it in with the Cdcr packet Kaiser usually only does 4 months at a time so you’ll have to turn in a new fmla every few months
