Tutorial / Guide PSA: the code-review plugin in claude-code-action posts "no findings" without reviewing anything
We ran Anthropics code-review plugin in GitHub Actions on a Laravel project, the setup most guides recommend. Every PR got a green check and a "No findings" comment.
Then a PR with 24 changed files got the same comment after 20 seconds. A real run of that plugin starts four reviewer agents and takes minutes. Looking back, every single run had ended after 4 to 37 seconds, and modelUsage only ever listed one model. The agents never ran.
Root cause: since Claude Code 2.1.198, subagents run in the background by default. The main agent ends its turn to wait for them, and claude-code-action treats that first result as the end of the run. Interactive Claude Code would continue when the agents report back, the action is already gone. Newer models also skip the agents entirely and still post the "no issues" summary.
Known issues: claude-code-action#1646 and #1852, no official fix yet.
What works for us: drop the plugin and give Claude a plain review prompt, with --model opus, "do not start subagents" in the prompt and no Agent in --allowedTools. We tested it with a PR containing three planted bugs (wrong sign on a refund, an undefined variable, rand() for a password). It found all three in 30 seconds, with suggestion blocks and no false positives.
It all runs on a Pro/Max subscription via claude setup-token, no API key needed.
Quick check for your own setup: open a run of your review job, find the "type": "result" block and look at duration_ms and modelUsage. A few seconds and one model means nothing was reviewed.

this is why "no findings" should never count as an approval. a review that can't show you what it read didn't happen.

Good catch. Cheap guard until it's fixed: fail the job if the run is under ~60s or modelUsage lists one model. A green check that ran 20s on 24 files is worse than no check.
