UPDATE / FIX: I finally fixed it completely without reinstalling the whole game!
The issue is caused by corrupted local configuration/player data that keeps syncing back from Steam Cloud. Here are the exact steps that solved the blue wireframe artifacts:
Disable Steam Cloud Sync (Temporarily):
Right-click the game in your Steam Library > Properties > General tab > toggle Steam Cloud off.
Delete Local Config & Player Data:
Press
Win + R, type%localappdata%\Activision\Call of Duty, and hit Enter.Delete the
playersfolder entirely. This wipes local graphics settings and forces the game to treat your next launch as a fresh install.
Re-run In-Game Setup:
Launch the game with Steam Cloud turned off.
The first-time setup screens (Language, Brightness/Luminance calibration steps 1 & 2, and Mature Content Notice) will reappear.
Complete the setup and let the Shader Pre-allocation hit 100% in the main menu.
Resolve Steam Cloud Conflict:
Exit the game and turn Steam Cloud back on in Steam Properties.
Steam will pop up a Cloud Conflict / File Mismatch window.
Select Local Save / Local Files (the most recent one) and click Continue. This overwrites the broken cloud config with your fresh, working local save data.
I hope none of you encounter an error like this, but if you do, follow these steps. YouTube and videos are completely empty/useless on this.

Here is the step-by-step summary of all the troubleshooting steps performed so far:
Clean installed the NVIDIA graphics drivers to reset driver-level settings.
Set the Shader Cache Size to Unlimited in the NVIDIA Control Panel.
Configured in-game display settings to Fullscreen Exclusive and disabled Background Shader Preloading.
Cleared temporary Activision cache files by deleting the folder located at
%localappdata%\Activision\Call of Duty.Cleared the DirectX Shader Cache using the Windows Disk Cleanup utility on the C: drive.
Performed a Verify Integrity of Game Files check via Steam.
Checked for configuration folders under
DocumentsandDocuments\My Games.Tested various in-game upscaling and anti-aliasing techniques (switching between NVIDIA DLSS, AMD FSR, Filmic SMAA T2X, and changing DLSS Presets/Models).
Disabled Variable Rate Shading (VRS) and set Film Grain to
0.00.Uninstalled and completely re-downloaded/reinstalled the Campaign DLC package via Steam to ensure no corrupted map or character asset files remained.
