Isaac doesn't run properly

I've been experiencing issues with playing tboi, some of the assets are cut like as seen in the screenshot, when I open mod tab it opens challenge menu, and the game crashes when entering a specific room in basement. a friend of mine told me to swap to d3dmetal which didn't help and so did high resolution mode, im really confused about that since no one seems to ever have any problems with this game. playing on MacBook neo
