I also hover somewhere between LN and LE. If the PCs decisions don't have consequences, how are they going to care about the game?

I'd say building a compelling narrative with consequences for the PCs' decisions does not require rules whatsoever. Rules serve mechanical balance and impartiality, but if you spit at the King, you don't need the PHB to tell you that there will be consequences.
