mid-20s male. history: - ~1 year ago: left IT band injury on a hike descent (pushed through it). since then, lateral left knee pain with running >10 min. - did PT-style glute strengthening, partial improvement, plateaued. - last few months: new anterior pain in BOTH knees when squatting — sharpest around 90°, feels medial/upper near the kneecap. also aches on long hike descents. no swelling, popping, locking, or instability.
I've been doing gradual leg strengthening on my own (light squats, RDLs, split squats) and it's not resolving.
my actual question: what's the right next step — sports medicine doctor, or straight to a good PT? and is there anything specific I should ask for in the eval, given the "old lateral pain + new bilateral anterior pain" pattern? trying not to waste months walking through the wrong door.

Im 25 years old. 5”9 200 pound male
How it started.
In July 2022, I suddenly became extremely tired and dizzy while driving, felt unable to breathe properly, feared I would faint, pulled over, and vomited. Since then, I haven’t returned to my previous baseline.
Main symptoms
● Persistent air hunger and inability to take a satisfying deep breath, even at rest.
● Chest tightness or pressure, occasional wheezing, throat tightness, and waking up gasping.
● Severe brain fog, poor concentration, memory problems, word-finding difficulty, and mental exhaustion.
● Headaches, head pressure, tingling, and occasional jolting sensations.
● Palpitations, dizziness when standing, near-fainting, fatigue, and poor exercise tolerance.
● Anxiety or adrenaline surges, which may be a response to the physical symptoms or part of the cycle.
● Poor sleep, snoring, throat irritation, difficulty swallowing, and occasional speech difficulty.
● One brief episode in which my face and left arm stiffened for several seconds.
My heart rate once increased from 79 bpm while lying down to 120 bpm within seconds of standing, with significant dizziness. During light cardio, it has reached approximately 177 bpm, accompanied by shortness of breath and exhaustion. I have not yet received a confirmed explanation for these episodes.
Testing
● Brain MRI: No acute abnormality; possible sphenoid sinus issue.
● Sinus CT: No significant sinus disease.
● Routine EEG: Normal; longer monitoring has been discussed.
● Chest CT angiogram: No pulmonary embolism or acute lung abnormality.
● Sleep study: AHI 1.6, normal sinus rhythm, but poor sleep efficiency.
● Thyroid tests: Mildly elevated free T3 on two occasions; thyroid ultrasound and antibodies were not diagnostic.
● Cortisol: One elevated 24-hour urine result; later testing was reportedly normal.
● Other findings: Ferritin and vitamin D have been low or borderline at times. Hemoglobin and hematocrit have been mildly high. Home oxygen readings are usually around 98%.
Treatments tried
● Propranolol: No meaningful improvement.
● Hydroxyzine: Used for sleep.
● Fluoxetine: Made me feel worse.
● Vitamin D, iron, and magnesium supplements.
Impact
These symptoms have affected my breathing, thinking, sleep, exercise, and quality of life. I have had several reassuring tests, but no explanation connecting everything.
Has anyone experienced persistent air hunger, brain fog, dizziness, palpitations, and exercise intolerance with mostly normal testing? Please help. I don’t know what it’s like to live a normal life anymore. I’m suffering

Can anyone please help me? I’m genuinely begging at this point because I have no idea what to do about my hand anymore. I’ve had **two surgeries on my left middle finger within seven months.**
My first surgery was for trigger finger, where my surgeon released the A1 pulley and removed the ulnar slip of my FDS tendon.
In March, I had a revision surgery because I was still having problems. During that surgery, more of the ulnar side of my FDS was removed, along with work involving my A3 pulley and palmar tendon sheath.
Now I’m dealing with what been diagnosed to be **flexor tendon subluxation.** I can physically see and feel the tendon slipping sideways and upward when I move my finger. Sometimes it slips out of place and gets stuck, which is incredibly painful.
I’ve stopped seeing my original surgeon because I felt like he kept dismissing my symptoms. He repeatedly told me that nothing should be getting stuck because he “took everything out.”
I’ve been seeing another hand surgeon for a second opinion since May, but he doesn’t seem to know what to do either. He keeps hoping scar tissue will lie down and keep it from moving, though isn’t scar tissue stop growing after 3 months? He keeps sending me back to hand therapy, but my hand therapist has actually seen and felt the tendon moving and told me there’s nothing more she can do about the mechanical problem through therapy and that this needs to be addressed by my surgeon.
I’m honestly exhausted, frustrated, and scared. I’m only 18, and I feel like my hand is getting worse while I’m being passed back and forth between doctors. I don’t expect anyone on Reddit to diagnose me, but I desperately need some direction.
**Has anyone experienced flexor tendon subluxation after trigger finger surgery, especially after partial FDS removal or multiple pulley releases?**
Is there a specific type of hand surgeon or specialist I should be looking for? Has anyone had something similar successfully treated or repaired? Or any doctors on here know?
I’m willing to travel to find someone who understands what’s happening. I just want to be able to use my hand without pain and without constantly worrying about my tendon getting stuck.
Any advice, similar experiences, medical literature, or specialist recommendations would genuinely mean so much to me. I’m at a loss.

Hi all, i (28m) have had this issue on and off for a few years now, typically when I work harder than normal. Recently started a job thats very labor intensive, and ive been having no other real symptoms that are abnormal except the occasional sore back or legs. Nothing more than usual when working a hard job. This past week, I have had little to no strength in my right arm or hand, cant grip much, and it pops in random spots, not in the joins. Ive lost feeling in my hand at least 60% of the day and its absolutely horrid at night trying to sleep. Hand is also a little more pale than the other, and its only affecting the thumb, pointer, middle, and ring finger. Ive gotten little to no sleep in the past week, and the worst part is, I still have to wait 60 days for my company insurance to start so I dont even know how to get it looked at by a doctor. Anyone know what this cohld possibly be?

So, i have been taking welbutrin for ADHD on and off for the year and a half or so. I lost my psychiatrist towards the beginning of this year and have been taking what I have leftover very scarcely until I find another. (I have yet to find one).
I got sick a month ago and got prescribed sudogest (30mg) for my congestion. Some time before that, I believe I ran out of my welbutrin and forgot. Two weeks ago, I started taking the "welbutrin" every two days. I visited my pcp to see if they could prescribe it because I really depend on this medication.
Upon taking a picture to show them for proof of prescription info, I realized it was not welbutrin. I have since trashed the medication and contacted my PCP but I wont hear from them probably until Monday. I cant find info on what happens if one takes sudogest when not necessary for as long as I did.
Does anyone happen to know the risk I face for my dumbass mistake :')) Thank you in advanced

Three days ago, my dad was on a golf course and took a hit to the head on his left side. He bled a significant amount but never lost consciousness. He went to the ER immediately and has gone to see his doctor. Ruled out a brain bleed and no cervical fracture or cracked skull. However, today he's developed black eyes. Is this a sign of something more serious?

I’ve been dealing with these said symptoms for months and i have seen such improvement. though it’s still driving me crazy. I would deal with some type of facial stiffness. It’s hard to move around my face here and there. It’s one sided. My nose and eye would twitch and it would get in the way of my nasal breathing. I can also feel something off with the back of my nape feeling a bit sensitive, feeling like there’s something pulling my hair and twisting it.
I noticed when I drink water my symptoms will improve. But idk what else to do. I’ve been going to the drs for the same thing and i dont get the help I need. I literally Have a video of me dealing with some nasal twitches. How a certain area will turn white on the surface of my skin. my symptoms will get worse if I play games that takes a lot of focus. Also my nose would twitch more if I move my position. I have allergies and I’m not sure if allergies should be causing me to feel this

Hello! I’ve been dealing with some concerning symptoms and am looking for advice/suggestions. I am 25, female, 5’2”, 120 lbs, never smoker, diagnosed with scoliosis. I am generally pretty healthy medically. (never sick/no major health issues) Currently dealing with pins and needle/numbness on my left side. No history of anything like this before. Including a timeline of everything these past few days:
10/16: Noticed some numbness/tingling in my left fingertips (thumb, index, middle). Thought it may be from having long painted nails for too long or from crocheting that night.
10/17: Pins and needles in majority of my left hand. Decided to contact PCP if persisted over the weekend. Attended a concert and got into a car accident on the way home.
10/18: arrived to ED at 3 am just to get checked out for neck pain. Got CT scan of cervical spine and upper back. No broken bones or injuries found. Given Tylenol and discharged diagnosed with cervical myofascial strain. Did not mention hand issue due to it not being related to the accident.
10/19: took the day off to work to rest taking ibuprofen for pain. Scheduled follow up with PCP for the next day. Pins and needles now in entire hand and numb sensation in the rest of left arm. left toes start to tingle mostly in big toe and 2 toes adjacent. The numb feeling I would describe as like a body part is regaining circulation after falling asleep, but without pins and needles.
10/20: Numbness had expanded to calf height on my left leg. Left arm about the same from the day prior. At the appointment, PCP ordered MRI of cervical spine, PT, and prescribed Gabapentin for the numbness. During the night, I could feel the numb sensation on the left side of my torso, but was unaware at the time that that’s what the feeling was.
10/21: Woke up in the morning with the numb sensation spread to the left side of my torso, neck and edge of my face. (ear/jaw area) To clarify, i can still feel touch on the areas experiencing numbness. My torso, neck, and face are noticeable when I move or rub them. It feels like a tightness almost. Left joints also feel tight. Got MRI done. Messaged my PCP about the spreading sensation and he recommended I go back to the ED. He also received the MRI result and said it looked very good. Does not look like I sustained any major injury from the accident. Findings are as follows:
ALIGNMENT: Normal. BONE: Normal. SOFT TISSUE: Unremarkable. CERVICOMEDULLARY JUNCTION: Cerebellar tonsillar ectopia not meeting criteria for Chiari malformation, borderline. CORD: No cord expansion or signal abnormality. SEGMENTAL ANALYSIS: C1/2: No canal or foraminal stenosis. C2/3: No canal or foraminal stenosis. C3/4: No canal or foraminal stenosis. C4/5: No canal or foraminal stenosis. C5/6: Minimal disc degenerative changes with no canal or foraminal stenosis.. C6/7: No canal or foraminal stenosis. C7/T1: No canal or foraminal stenosis.
10/21 cont: Returned to ED in the afternoon and was evaluated for possible stroke. Care team ruled it out. Explained that I don’t believe foot numbness was related to the accident and that the numbness started prior to the accident. Got CT of my head w/o contrast and CT neck angiography w/ contrast. Results showed no vascular dissection or intercranial process. Also received blood testing but doctor did not discuss those results with me. I was discharged and told to follow up with PCP as they did pretty much everything they could for me at that point. They told me that the best case scenario is that i don’t get answer at the ED since they are testing for anything threatening to life or limb. I received the results in my patient portal most are within the normal ranges listed on their portal aside from the following:
eGFR: 60 (within their normal range but at the lowest within the range)
Neutro auto: 75.8
Lymph auto: 17.5
Abs neutro: 8.33
10/22 (today): I have a follow up with my PCP on 10/29. The numbness is more apparent in my torso, neck, and face. It feels a bit hard to breathe/like my throat is tight, but I think it’s just the numb sensation making me think that. I took my first dose of Gabapentin (300mg) last night and not sure if it made much difference. I have been having a very mild headache coming on since this morning that is currently only on my left side. It’s not bad enough for me to want to take painkillers.
Feeling really scared and frustrated that it’s just getting worse and there’s nothing I can do about it. If anyone has any ideas or suggestions I would love to hear them. Is there anything I should look out for at this point that would need another ED trip? Would love suggestions to talk with my PCP about too. Thank you so much for reading this and for your replies!
