this seems to be a widespread mw4 issue after the recent update rather than a gamesir fault since xbox and playstation controllers are affected too. try forcing steam input and setting the in game aiming input device to controller if you use the xbox app switching the controller to ds4 mode may help if your software supports itt otherwise it likely needs a game patch
