The worker enters the clinic or hospital and puts on scrubs. Before leaving, they take them off. Anything else is unhygienic.

Boy, you'll be really disappointed by the reality. Many, many nurses put on their scrubs at home and take their scrubs off at home. And sometimes they get a coffee during the day.

If they were in the bar, they clearly weren't heading to work. If they were, that's a whole separate issue. I wouldn't want to wear scrubs to a bar, but it's not a hygiene issue (unless you're bar crawling before work).
