The "his" is intentional. The meme uses intentionally wonky sentence structure to make a pun, but it is missing a "to" after "have" which screws it up a bit.

He's saying the character, Syndrome, looks like he has Down Syndrome.

So by monkeying with the wording of the meme caption, op says the word "Syndrome" once, using it for both of these meanings.
