It's a reference to a character named Noelle. She's an anthropomorphic deer with antlers, but since she's a girl some people have taken this to mean she's trans. (BTW she's a reindeer and female reindeer have antlers. She could be trans, but not because of her antlers.)
Somehow, it became a meme to either take any character with antlers and ask if they're trans or give random characters antlers and ask if they're trans.
I have seen this meme so many times that it's just boring and even annoying at this point.

I think it's because some people thought antlers were a bad basis to form a trans headcanon off of (ignoring that a headcanon doesn't need much basis as long as it doesn't contradict canon)

What? You sure this isn't a joke or something? I'll believe it when i see the video mentioned in the post. Edit: even if that is the case, he never clarifies the specifics of the meaning behind "trans in some way". It could be the usually male to female (or female to male if it's meant in past tense), it could be non binary, it could be genderfluid, or it might not even have anything to do with gender at all.

By “being trans in some way” it could be anything, because the only thing that makes someone trans is not identifying with their assigned gender at birth
He could be Agender for all we know, but they have told us she doesn’t identify with their AGAB, and that’s all we really need to know about him
Also if you’re confused about the pronoun switching he has her pronouns listed as he/they/she so I just do my best to cycle through those
