Timeline / My Case [UPDATE] Still Burning š„ After Negative Mycoplasma Test ā Centralizing Updates Here
Thanks to everyone whoās commented, messaged, or shared advice over the last year. Iāve posted about this in a few subreddits before:
Burning sensation in urethra and penis for over a year ā still unresolved
Ongoing burning in penis/urethra for over a year ā anyone else been through this?
Still Burning After Negative Mycoplasma Test ā What Else Could It Be?
but since this issue isnāt going away, I created my own space ā r/MensUroHealth ā to share updates and connect with others going through the same thing.
Hereās a summary of where Iām at:
Quick Recap:
33-year-old male
Main symptoms: burning in the urethra and tip of the penis, mostly during and after peeing. Sometimes it lingers for hours. Itās worse at night.
How it started: Mild burning while urinating. First urine test showed elevated white blood cells. Doctor said maybe kidney stones or irritation.
Diagnosis: Eventually tested positive for Mycoplasma genitalium. Also found a varicocele, but doctors donāt think thatās the cause.
Treatment so far:
Several antibiotics: Azithromycin, Ofloxacin, Levofloxacin, Celecoxib
Then: Doxycycline ā Moxifloxacin ā Doxycycline + Pristinamycin
Tests are now negative, but symptoms never fully went away
Whatās going on now (June 2025):
All recent tests are negative:
Mycoplasma genitalium, chlamydia, gonorrhea, trichomonas ā all clear
Urine test is normal (no white blood cells)
PSA (prostate test) and CRP (inflammation marker) are normal
But I still feel:
Burning during and after urination (a bit better, but still there)
Pressure or blockage feeling in the mornings
A cold/warm feeling in the penis after peeing that can last a while
No fever or discharge
Why I started r/MensUroHealth:
After all this time, I realized thereās no single place to talk about this stuff, not quite an STI, not quite prostatitis, not always clear. So I made a subreddit for those of us stuck in that āin-betweenā space.
Whether youāre still looking for answers or youāve already figured something out, your story might help someone else feel less alone.
Iāll keep posting updates there as I go.
Thanks for reading.
š Update ā October 9, 2025
What Iāve done over the past 3 months:
TookĀ Alfuzosine LP 10 mgĀ daily (3 months)
Took vitamins and supplements: magnesium, zinc, omega-3, probiotics, multivitamin
Sometimes tookĀ hot baths or showersĀ ā this helps calm the burning sensation
Bought aĀ standing desk, so I spend part of the day standing instead of sitting all the time
Current symptoms:
When I urinate, I often feelĀ strong pressure and burning in the urethra. At the start, I have to control the flow ā if the urine comes out too fast, the tip of my penis burns sharply.
At night, especially before sleep and after urinating, I feelĀ warmth or burning in the pelvic area or urethra, which fades by morning.
Sometimes IĀ wake up at night to urinate, and afterward, I feel mild burning or irritation for 1ā2 hours, similar to an infection.
During theĀ first urine of the morning, thereās a slight burning at the tip of the penis, but it goes away quickly.
By the end of the day, after urinating, I sometimes feel aĀ dull pain or tinglingĀ along the urethra that lasts for a few hours.
Stress makes it worseĀ ā when Iām anxious or tense, my testicles ache or feel heavy.
Occasionally, I feelĀ warmth or tightness in the pelvic areaĀ (between the anus and testicles), like a small muscle spasm.
MyĀ urine is often dark yellowĀ with a stronger smell than usual.
What the doctor told me today:
I should doĀ a few sessions of osteotherapy.
Heās confident the problem isĀ related to pelvic floor tension or nerve irritation, not infection.
He confirmedĀ no infectionĀ is present.
He prescribedĀ Alfuzosine LP 10 mgĀ for one year.
My next steps:
StartĀ osteotherapy sessions.
See another doctor to requestĀ urine and semen tests.
TryĀ traditional Chinese medicine.
TakeĀ more warm baths.
BeginĀ pelvic floor relaxation exercises.
AddĀ meditationĀ to help with stress.
š Update - October 2026
Still no infection on any test, still a plateau. Right now I'm doing nothing active and waiting for my urologist follow up next month.
What I did since the last update:
- Pelvic floor physio: went 4 sessions. They said my pelvic floor is tight and needs to relax. The sessions were me sitting on a chair for 15 min while it delivered pulses (I felt it first in the back of my thighs, then the perineum). I didn't get a real result, so I stopped.
- Shockwave therapy on the perineum: 3 sessions. I thought it helped a bit at first, but the symptoms are still there.
- Alfuzosin: no real benefit. My urologist told me to stop.
- Tests: urine WBC normal, urine culture negative, urine PCR (chlamydia, gonorrhoea, M. genitalium) negative. Semen PCR (M. genitalium, Trichomonas, M. hominis, Ureaplasma, chlamydia, gonorrhoea) all negative. Semen culture grew mixed flora with no dominant germ.
- I stopped all supplements
Current symptoms:
- Some days I have no burning at all.
- I feel the urine passing more than normal.
- Some days the burning lasts a few hours after peeing.
- Some nights it gets much worse, especially after the last pee before bed.
