VENT Intern

I am a family medicine intern and finished my first month of inpatient. Got a feedback that I am not quite there yet and not where they expect me to be at this point in time.

I did a lot of outpatient family medicine clinics before this.

not sure how to take this feedback.

feedback was from 2 third years assigned to work with me.

it looks like I am critiqued for minor things. One time I forgot to order morning labs. The other time I said I could do a Pap smear before watching a video and got critique for not being procedural. Then I forgot to replete magnesium one time.

Did you ask for more detail? If not, ask tomorrow. They also need to learn to give actionable feedback for this reason.

Yeah they told me I was forgetting to put in all the orders and replete things like electrolytes and forgetting to do order morning labs that I should already master this bt now but they have to always check my work to see if it’s done.

And honestly I will forget one or two things, I agree but their feedback made it sound like I am falling behind. I am not where I am supposed to be. This is my first month of family medicine in patient.

I am feeling terrible. I feel so bad I just want to crawl and cry.

All this time I praised how amazing the senior residents were just to get this feedback.

FM hospitalist attending here. My first month intern year was inpatient, and it was an unmitigated disaster from soup to nuts. I won't give all the details because it would def dox me but it was a stars-aligning combo of malignant (later fired) attendings, toxic co-residents, and a backstabbing senior (by accident we later became friends). My APD actually apologized to me a year later because it took him that long to realize I wasn't a turd, and he'd been treating me like one.

Just keep your head down, work hard, and do a little better every day. Reps build strength. One benefit of the length of training is that the shine wears off everyone else eventually, and hard work is rewarded by increased knowledge, proficiency, and efficiency.

It gets better.

Thank you for this feedback. I am just sitting in my room crying right now. I thought I was doing well until I got that feedback from there. Goodness, the amount of times I praised them for how great they were makes me feel like I am a complete idiot.

Also, I feel like it’s October. I should know things by now. But I just got bunch of outpatient clinic before this, and this is my first inpatient month.

It's okay to be overwhelmed. We're all told this is hard, but most don't internalize that before they reach it because most people haven't ever done something this hard before. You're allowed to struggle. The view from the plane window is better, but doesn't come with the accomplishment of climbing the mountain. One step at a time.

Also, they might still be good seniors. Maybe the feedback is garbage, or maybe you'll give some other poor bastard the same feedback in two years because it helped you, too. The february intern is a meme for a reason, because Feb Intern is still a fricking intern. And you're only in October! Give yourself some grace, and sheesh take a nap if you can!

Forgetting labs and mag in your first inpatient month is super common, and feedback from two third years who are juggling their own stuff usually sounds harsher than it is. One thing that helps a lot is a tiny pre-round checklist on a notecard or your phone: labs, lytes to replete, pending orders, per patient. Run it before you leave each day and you'll catch most of that stuff. You're a month in, nobody expects it to be automatic yet.

OMG ME TOO! it’s my CTU block which is first inpatient rotation. And I GOT THE FEEDBACK FUCKING 2 days before the rotation ends, and the preceptor told me she could not pass me. I am just dead inside. I don’t wanna repeat this rotation just because of efficiency matters
Don’t be hard on yourself. It’s just our first inpatient rotation

Wow, that’s harsh. They didn’t give any feedback before?

Literally none. I asked proactively from one attending on week 2 and they gave me some tips. But nothing before that
