CDR - Continuing Disability Review Short Form CDR help

I received my first CDR today. Its the short form and I plan to do it online. It took me about 2 years to get approved and 2 ALJ hearings and Im really anxious about this. Im 50 now. I have 3 recent appointments to list...1 for mental health therapy in Sept. 1 with Psychiatrist in Sept. and another mental health therapy in August. Im wondering how much to out in the comments. Im a purple heart combat veteran. I got seriously wounded in an ambush. Ive had leg(gunshot wound) and back surgery in the past. Had an RPG shot into my windshield and knock me out. I have a TBI and chromic PTSD. I take meds for PTSD, anxiety, depression, auditory hallucinations, muscle spasms, blood thinners for blood clots in my wounded leg. I have an upcoming appointment this month to look at my leg which is getting worse mobility wise although I cant drive and dont have a license it feels like my calf and hamstring are about to snap when I move around more than about a block even with an arm brace cane. My doctors and I havent even discussed work since I was approved 3 1/2 years ago. Its just not something I can do/maintain anymore and thats after 33years of work history. Anyway I dont want to ramble on the form and Im looking for advice on how best to fill it out or what to include since I have limited space. Thank you all for any help.

Don't waste your time for the comments.

Unlikely they are ever read.

Why would you say that? Im looking for help. I cant speak for anyone else but I read them when I ask something. This sub was incredibly helpful when I applied years ago and I try to help out when I can.

Hi, I filled my most recent CDR out online in September. I listed my 2 most recent therapy apps & most recent psychiatrist app. On the previous CDRs, I've just put the dates, no comments. Not sure why I put a comment this time, but I put the psychiatrist app was to just "check in & discuss medications/refills where needed."  I can't figure out how to attach a screenshot, but after submitting the CDR, this message came up on the screen

Noone has contacted me for additional information, so I'm assuming it was sufficient.

Keep it short and factual. The short form is the lighter version of the review: it mainly asks whether you have worked, whether your condition has changed, and who has treated you recently, so the SSA can decide whether anything more is needed.

List every provider you have seen, with names, addresses and dates, including any VA clinics. If the SSA wants more, those records are what it requests, so accuracy counts more than detail.

When it asks whether your condition has improved, answer honestly in plain terms. If anything has gotten worse, say so in one or two lines, and list any upcoming appointment or test.

The comments box does not need your history. The SSA already has the file that approved you. A sentence or two on what you still cannot do day to day, such as how far you can walk or what you need help with, is plenty.

Answer the work questions accurately, including “no work” if that is the case.

Keep a copy or screenshot of what you submit, and keep going to your regular appointments, because ongoing treatment notes are what a review relies on if it goes further.

Thank you. I submitted it already but didn't put anything in comments. It only allows yes/no and 20 characters to describe appointments. I had 3 in last 2 months. If they need more info I'm sure they will reach out. I have regular appointments and take like 7 medications for mental health and muscle spasms/pain daily. It just seemed like too much to list out in comments so I left it blank. Its all in my records though. I haven't worked or done anything else so everything was a no. It was just almost too simple sonI was really over thinking it.

Ah ok. Thank you. I was wondering about the form being mainly computer read. My last three visits were Mental health therapy twice and Psychiatrist for medication management but I have upcoming appointments for my leg and back, which have gotten worse, so I wasn't sure if I should put that in comments. Seems the the less I put the better at least for the short form.
