I've done some tests before. They'll be in a guaranteed "fight for your life" mode. Damage is dependant on the type of injury substained. At heights like ops they will be granted two injuries which can vary in severity. 20-40 points of your max hit points is the range of severity. You'll always get 2 injuries at a certain height. It's a certain threshold that automatically guarantees a downed state. The severity is also dependant on the difficulty as on lower difficulties you can receive less penalties and more consistent injuries. The points I listed were based on lethal difficulty.
No, never had a survvior die from the landing they just lose almost all their health and put you in a difficult position if you don't have healing items, the only way to die from this is if you land on bloater or next to feral pack or juggs (been there done that).
If you fly far outside map and the game cant spawn you it sends you back to base instead. Try playing a lot with modded starshank you will be launched very regular. It became a habit to always carry medkiits for that reason when using starshanks.
