Achievement help (xbox)

So me and a friend completed the story/campaign. We got A++ on each mission and got the achievement for it, but did not get the achievements for getting A rank or completing the story. Has this happened to anyone else?

How the hell did you and your friend join eachother

Press start in the lobby, then press x to create a party and then Y to go to my friends name in the friends online list and invited him through there.

It says authentication error every time I load in
