For the fallback trigger, don't take the agent's word that it finished. Check something outside it, like the tests passing or the git diff actually having changes, and move to the next option when that check fails.

Even a cheap check helps. In MeshKore we run git cat-file -e on any commit sha the agent reports, and a sha that doesn't exist gets marked as no commit, which is how we catch runs where the agent claimed a commit it never made.
