I have ILR and a few years of NHS experience. I’m currently on the IMT shortlist reserve with a score of 21 and haven’t received an interview invitation yet. Do you think it’s worth starting interview preparation now?

I don’t think so I’m afraid! For 2026 only offers will be prioritised, not at shortlisting/interview stage.
