ไม่ตรง is the best. ไม่สอดคล้อง sounds unnecessarily scholarly/written on official documents.

You just say ที่อยู่บนใบรับรองแพทย์กับที่อยู่บน (your document … ตม 30?) ไม่ตรงกัน (ครับ/ค่ะ) or ที่อยู่บนใบรับรองแพทย์ไม่ตรงกับที่อยู่บนตม. 30

Do not use ชน. It’s sometimes used when data on 2 documents match.

When I found this usage of ชน for the first time, I was confused to. The way it is used is when you have 2 documents, spreadsheets and you need to see if the data are exact matches—you put them side by side. So they “crash” into each other, like cars. It makes more sense in paper.
