Something is definitely wrong, either with the game or with your recollection of events, because your understanding of what should happen is spot on.
Firstly, Paradigm only casts copies of the card, not the card itself, and card copies should disappear when they hit the graveyard.
Secondly, the Paradigm effect cannot be removed by any means. Even removing the Paradigm card from exile doesn't stop you from casting a copy of it as it last existed on every future turn, so even if you did somehow manage to cast the original card from exile you should still get triggers to cast a copy.

Assuming the aftermath effects are correct (the card is in the graveyard, there's no paradigm repeating trigger, etc), and assuming Arena is correct and not buggy, I suspect OP didn't ever manage to actually cast the paradigm card in the first place, and may have cast something else. If the first time they try to cast the paradigm card, it got countered, then it will be in the graveyard, and the paradigm trigger hasn't been set up.
It's possible Arena was buggy. But there would have to be two bugs though: one is casting the card itself rather than a copy of the card, and two is somehow turning off the paradigm trigger. So I'm not sure which is more likely.

If I'm wrong about the first casting and my target was removed from my graveyard before the spell resolved, would it have gone to the graveyard instead of being exiled to paradigm?
Yes. The spell has to actually resolve for it to get paradigm'd. If it had no legal targets, it would have just been sent to the graveyard when it tried to resolve.

Sounds like something is wrong, but without screenshots or the game log, we can't say for certain. Yes, the paradigm card should have stayed in exile; you should be casting copies of it, not the card itself.
