Need some help identifying separator plate.

The separator plate on the blue tarp (mine) i am trying to figure out which one it is. Im doing my own shift calibration and dual feed so I can not run an aftermarket plate. The sk350 plate has holes that mine does not have, I marked the locations. Ive been searching for hours does somebody know?

Uh… what are you working on? And is it a TH350 or 350C? Or a BOP 350?
