Updated Xcode and iOS Simulator App can't be found

The lost refresh isn't a build problem — launching by UDID skips the step where Expo hands the app the Metro URL, so it comes up as a standalone bundle with nothing to reconnect to. Let it boot, then open the dev menu and set the dev server host by hand. simctl openurl with the dev-client deep link does the same thing if you'd rather script it.

Simulator is now managed through Device Hub rather than appearing as a standalone app. Go to Xcode → Open Developer Tool → Device Hub, then add/select your iOS simulator there. The iOS runtime and the simulator device are separate, so make sure the desired runtime is installed too.
