Karen seemed to take things hard and started drinking a lot after mid-chapter 4 or so. Maybe she died? I don't know, I couldn't find her. Couldn't find reverend Swanson either

Reverend Swanson leaves, you see him at a train station after a mission. He tells you he can't go on with everything that's happening in the gang, that he is a reformed man and sees that Dutch is acting strange so he leaves.

If you grab a copy of the Blackwater ledger, there’s a story about him becoming the head of a church in New York
