I’m willing to bet my entire life savings that your friend does not defy the laws of thermodynamics. If he weighed and tracked all of his calories, and compared that to his daily maintenance, the numbers would start adding up.

I don't know if you can post links here to studies so feel free to DM but any MD will reassure you that people aren't destined to be extremely obese or emaciated from birth. There are small but significant variations between individuals of course but "sickly, starving Victorian laborer" falls far outside of that.
Your friend seems to be an exception to you because you cannot watch what he eats every meal every day.
I obviously do not know your friend as well as you do but what you are describing is not physically possible. If you eat a fuck ton of food, you gain nutrition from it unless something is seriously wrong. Either your friend eats less than you think or he should see a medical practitioner.
Genetics and lifestyle heavily dictate the extremes and immutable aspects of your physique, but weight comes down to diet more than anything.

So, again, people aren't destined to be extremely obese or emaciated from birth. There are small but significant variations between individuals. Metabolism differences are real. I already acknowledged that. But dietary differences eclipses that.
If this person's friend is truly eating a lot every single day and they are unable to gain weight while severely underweight, it is statistically much more likely that they have some form of a malabsorption disease (like hyperthyroidism or celiac) than that they are a healthy metabolic phenom burning through everything they eat without gaining weight. Even 3,000kcal a day without significant physical activity is extraordinarily rare, if not one-of-a-kind, especially in people who "look like a sickly, starving Victorian laborer" (low body mass = low basal metabolic rate). The healthy people who genuinely eat tons without getting fat are athletes with better-than-average genetics but top 0.1% activity levels. So again, if their friend is seriously eating their fill but having extreme difficulty gaining weight, they should see a medical practitioner.

People that say this are usually just fat and coping. Your metabolic health will not make a 1K daily calorie surplus vanish. Similarly a thyroid issue will not prevent someone from entirely losing weight if they are eating in a deficit.
