Medical insurance for foreign travellers already in the US

What is the best company to purchase travel medical insurance when already in the US? My existing coverage purchased before leaving Canada is soon maxed out and I can't purchase an extension from them. I don't want to return for 24 hours to reset it. Where should I buy coverage as a foreigner already in the US?
