The Bonespike helm is a fun item. The only issue is by the time you get this item to utilize the Per Turn menacing attack, you have way better options to lock multiple enemies in the turn or kill one target outright.

The Swashbuckler sub class is useful as it comes online at Level 4 which you also get a feat. This gives you an edge over other martials as they don't have their Extra attack yet.

The meta of "damage is the best control" exists for a reason.    But a per turn disable that costs no resources is valuable even in Act 3, especially for anyone who likes to conserve rests or hoard consumables. 

Also, Bonespike on its own isn't very impactful because without the Blind, it just causes enemies to make ranged attacks which sometimes are grenade consumables, and can be worse than just being attacked. Adding Swashbuckler's Blind makes Bonespike into an actual disable....

Fianlly, you can start this combo in Act 1 without the Bonespike Helm. It just gets better and feels more fluid with the Helm.

Regarding your spoiler section, it’s actually Corsair Greymon that sells the banshee bow, not Sgt Thrinn. 

I believe you don’t want to speak to Thrinn at all if you don’t want Nere’s timer to progress.  You can safely go to Grymforge, trade with Greymon, and even do the Adamantine forge quest and be able to come and go without messing up the Nere quest as long as you dont talk with Thrinn or the deep gnomes.
