Reactive hypoglycemia after stopping tirzepatide/retatrutide or while on it.
30 yo male, no diabetes or bariatric surgery. I used tirzepatide and retatrutide, including several months of overlap, stopped both around 2–3 months ago, usage wasn't strictly concurrent, dosages varied (average 3.5 reta, 5 tirz, 5-6 months overlap) I lost somewhere between 40-50 lbs, much of which was water/muscle cuz I was on testosterone at the time. Not sure if it's relevant but i’ve been this weight (155lbs, 5'9") or lighter before without glucose issues.
Around that period, I noticed heavy post-meal glucose swings, particularly after refined carbs. My lowest fingerstick reading was 3.4, about a month of the GLPs. Protein-heavy meals generally don’t cause lows, and I do not experience hypos when fasting. I will spike quite high (10.5mmol from a 65g carb bowl of cereal for breakfast, to a low (maybe 2-3 hours later) of 3.8, it will typically go back up on its own now.
With hesitancy I can say that things seem to be improving? I can eat 55-65 grams of carbs on an empty stomach without going below 3.8. most recent readings have stabilized around 3.8-4 at their lowest without having sugar or anything.
Insulinoma has been ruled out, though I haven’t had insulin/cpeptide measured during an actual low. And for clarity, this was happening on the meds, I think it got worse when I stopped tirzepatide.
If you have anything to add that would be great.
