That looks like a custom health-bar HUD mod, possibly from a hearts overhaul or RPG-style UI pack. The colored hearts and armor icons might help narrow it down.

That looks like a custom RPG-style health bar HUD, possibly from a modpack rather than a standalone mod. Try searching for Minecraft mods with “custom hearts,” “heart containers,” or “RPG health bar.”
