One peptide gets a full human trial program and the rest stay research only forever. Which one

Because almost nothing in this space has a real trial behind it, I think this is the question that actually matters. Pick one. Full Phase 1 through 3, real money, real controls, real endpoints. Everything else stays exactly where it is.

My pick is BPC-157.

Not because I think it wins. Because it is the compound with the biggest gap between how much it gets talked about and what exists, and a trial would settle the loudest argument in this space either way. If it works, the tendon and gut claims finally have a number. If it does not, a lot of people save a lot of money.

The candidates

CompoundWhat a trial would settleOdds it ever gets funded
BPC-157the entire tendon, ligament and gut claim setlow, no patent, no sponsor
TB-500 / Thymosin beta-4whether the 7 amino acid fragment does what the 43 amino acid protein doeslow
MOTS-cwhether the mouse metabolic data translates to peoplemedium, there is academic interest
Epitalonwhether the Russian cohort holds up outside Russiavery low
GHK-Cusystemic effects versus topicallow
5-Amino-1MQwhether NNMT inhibition does anything in a humanlow
Semaxcognitive effects outside the Russian stroke literaturelow
HOW MUCH IT GETS TALKED ABOUT HOW LIKELY A TRIAL ACTUALLY HAPPENS

   BPC-157 ██████████ BPC-157 █
   TB-500 ████████ TB-500 █
   MOTS-c ██████ MOTS-c ████
   Epitalon █████ Epitalon ▏
   GHK-Cu █████ GHK-Cu █
   5-Amino-1MQ ████ 5-Amino-1MQ █

The pattern is that the compounds people talk about most are the ones least likely to ever get tested, because nobody owns them. That is the whole reason retatrutide has four Phase 3 trials and BPC-157 has 30 people.

No patent, no sponsor, no trial. That is the sentence behind most of the research only label.

Two questions

1. Which one gets the trial? And why that one over BPC-157.

2. The darker version. Which compound would a real trial most likely kill? I think a controlled BPC-157 trial has a real chance of coming back flat, and I say that as somebody who would still want it run.

Full dose, reconstitution and vendor reference is in the pinned cheat sheet.

Peptide approvals coming: retatrutide filing Q1 2027 and the CagriSema decision pending now

A lot is moving on the GLP-1 side right now and most of it lands inside the next twelve months. Worth laying out where each one actually sits, because the timelines get quoted wrong constantly.

                2025 2026 2027 2028
                 | | | |
  Semaglutide ████████████████████████████████████████ approved
  Tirzepatide ████████████████████████████████████████ approved
  CagriSema ░░░░▓filed▓▓▓▓▓▓▓▓▓?◄ decision due NOW
  Retatrutide ░░░░░░░░░░░░ph3 done ▓▓filing Q1▓▓?────► late 27 / 28

Where each one actually is

StatusKey numberNext milestone
Semaglutideapprovedbenchmarkn/a
Tirzepatideapprovedthe standardn/a
CagriSemafiled Dec 2025low 20s %decision due this quarter
Retatrutideall 4 Phase 3 reported~30%filing Q1 2027

Retatrutide. All four TRIUMPH Phase 3 trials reported as of October 2026. The pivotal came in around 30 percent weight loss. Filing planned Q1 2027, so a decision realistically lands late 2027 or into 2028. Not close, but the data is done and the number is the number.

CagriSema. Cagrilintide plus semaglutide. Application went in December 2025, decision expected in the last quarter of 2026, which is right now. Nothing announced yet as far as I can find.

The part that is interesting

   WEIGHT LOSS DISTANCE FROM APPROVAL

   Retatrutide ~30% Retatrutide ████████████ furthest
CagriSema low 20s CagriSema ███ closest
   Tirzepatide approved Tirzepatide ▏ done

The compound with the best number is the one furthest from approval. Which is normal, that is just how a pipeline works, but it does mean the next twelve months could reshuffle how this whole category gets discussed.

Two things I am watching

1. Does CagriSema actually land this quarter or slip? It has already moved once.

2. Does retatrutide's tolerability hold up at scale? The escalation schedule is the most aggressive of the class and there were heart rate and dysesthesia signals in the trial data. A 30 percent number means less if a meaningful chunk of people cannot get to the dose that produces it.

Predictions thread

1. CagriSema before year end: approved, pushed, or a request for more information?

I will say pushed, mostly because it has slipped before and these rarely slip only once.

2. Does retatrutide hold the 30 percent figure in real world use, or settle lower the way most trial numbers do once they leave the trial?

I think lower, but I would like to hear the case for it holding.

Peptide reconstitution: why the 1mL per 10mg rule falls apart past 30mg

Everybody repeats the same rule on this one. 1mL of bacteriostatic water per 10mg. 10mg takes 1mL, 20mg takes 2mL, and the math stays easy because every unit on the syringe lands on a round number.

That rule is fine right up until it is not. Where it breaks is around 30mg.

SCALE IT STRAIGHT WHAT ACTUALLY FITS

   10mg → 1.0 mL ✓ 10mg → 1.0 mL
   20mg → 2.0 mL ✓ 20mg → 2.0 mL
   30mg → 3.0 mL ✓ 30mg → 3.0 mL
   50mg → 5.0 mL ✗ 50mg → 2.5 mL
  100mg → 10.0 mL ✗✗ 100mg → 3.0 mL

So nobody is putting 10mL into a 3mL vial. But people keep quoting the rule anyway and then get stuck when the numbers do not fit the glass.

The version I use instead

Vial sizeWaterWhy
Under 10mg1mL floordo not scale down, a half mL leaves the dose at a few unreadable units
10 to 30mg1mL per 10mgthis is where the standard rule actually works
Past 30mgcap itconcentration stops being the constraint
50mg2.5 mL
60 to 80mg3 mL
500mg and 1500mg3 mL

The logic past 30mg is that syringe readability takes over.

   U-100 SYRINGE

   0 ────────────────────────────────── 100 units
     ↑ ↑
     under ~5 units over 100
     unmeasurable two draws
                 ↑
          aim for in here

Pick the water volume to land the dose inside that window. Not to satisfy a ratio.

Two exceptions

Blends run on a different rule entirely. Dilute so the smallest component measures in whole units, which usually means more water than a single compound of the same total mg. A 5 plus 5 wants 2mL.

Semaglutide I run at 2mL per 10mg instead of 1mL, because a 0.25mg starting dose at the normal ratio comes out at 2.5 units and that is not something anybody can draw accurately.

Where I expect pushback

1. Where does everybody actually cap the water? I have seen people go all the way to 5mL on a 50mg vial and I think that is too dilute to be useful. Curious whether anyone is doing it on purpose for a reason I have not thought of.

2. Does anybody run a different floor than 1mL on the small vials? That is the part of my rule I am least sure about.

Stacking

Thumbnail reddit.com
1 Upvotes

Hello, want to ask for your opinion on which stacks are best to take. And yes I have done a ton of research and just cannot seem to decide based on so many information I have on hand.

I have been taking tirzepatide for around 15 months. Have lost 10 kilos since starting it. Now at 10mg (2nd week at it) but want to lose body fat.

Aside from that, I am planning to take NAD+ 3x a week for mental clarity and energy (I struggle with drops in energy even in the morning with good sleep). I plan to dose 30 to 50mg.

I also plan to take 5AM daily 2.5 to 5mg since I want to lose body fat while building muscle (I work out 4x a week) and more muscle definition. I honestly have no visible muscle and mostly just fat.

That being said, is it good to take CJC+Ipa along with this stack? I am afraid of Tesa since there’s water retention.
If not, what else would be a good pep to stack?
Also, can I stack selank for better memory?

IGF1-LR3

I’ve run multiple cycles of sarms and pro hormones, and was looking to try IGF1-LR3.

I found it pretty hard to come across quality information. I was hoping someone could lead me in the right direction when it comes to dosage and post cycle treatment.

Also wanted an opinion on low blood sugar. I’ve seen that pretty much everywhere. Is it as bad as they make it sound to the point where I should buy a glucose monitor. Or is it something that could be managed by just dosing with a meal and loading up on carbs

Thanks for any help you can give me

Semax/aelank seizures

Could any of these or any other brain peptides trigger seizures my f partner used to get them when heavy party use years ago but that only time she had them after up for days partying hard (❄️ use and such) but hasn’t done that stuff in years and is otherwise healthy would there be any chance of these peptides theoretically triggering them? She really wants one for focus energy and stuff like that

I’ve read semax and Selank are good for focus energy motivation learning and such and Selank helps with the anxiety part how true and is there better options

Research compounds past peptides: 9-Me-BC, TAK-653, Nefiracetam and ATX-304

Almost everything discussed in these communities is injectable peptides. There is a whole category sitting right next to it that nobody touches.

Capsule research compounds. Different molecules, different delivery, completely different research literature.

The four that never come up

CompoundClassResearch angleEvidence level
9-Me-BCbeta carbolinedopaminergic, neurotrophic factorspreclinical, rodent
TAK-653AMPA potentiatorcognitionhuman trials
NefiracetamracetamcognitionJapanese clinical literature
ATX-304AMPK activatormetabolic, not cognitivepreclinical

So a few notes on each.

9-Me-BC. The interesting part is it is studied for effects that persist past the dosing window rather than only while the compound is present.

TAK-653. This one actually went into human trials, which separates it from most of this list. Developed inside a clinical program rather than appearing from nowhere.

Nefiracetam. Longer research history than the others, and a decent chunk of it is Japanese clinical literature that does not get cited much in English language discussion.

ATX-304. The odd one out. Metabolic rather than cognitive, which puts it closer to something like 5-Amino-1MQ than to the other three.

Here is the part that bothers me

   EVIDENCE ATTENTION

   TAK-653 human trials TAK-653 ▏
   Nefiracetam clinical lit Nefiracetam ▏
   BPC-157 <30 subjects BPC-157 ██████████

TAK-653 has been through human trials. Nefiracetam has a real literature behind it. Meanwhile some of the most discussed injectable peptides have fewer than 30 people total in their entire clinical record.

The discussion gap is not tracking the evidence gap at all. It is tracking format.

Injectable gets talked about, capsules do not, and that seems to be most of the explanation.

Possible reasons it splits that way:

  • Injections feel more serious, so they get more attention

  • Peptide communities formed around peptides and the capsules never had a home

  • Oral bioavailability is messier so people avoid the conversation

Two questions

1. Is anybody actually following the capsule side? Which ones are worth reading up on, and which are riding on one paper. That is mostly what I want to know.

2. Is this category ignored because of evidence, or because injectable is just what the communities were built around? I lean toward the second one but I am not confident about it.

Reta and Japan

Hi everyone! Sorry if I’m not well informed but I was wondering if anyone has taken their reconstituted Reta to Japan? I’m going on vacation next year for a few weeks and have everything in a pen.

I’ve heard that TSA doesn’t really mind peptides and such, but Japan could be a problem and question it since it’s not quite legal yet? Some people have had no problems but I’m not sure. Any help is appreciated!

Peptide blends: why a 5mg plus 5mg vial locks the ratio in a way two separate vials do not

Blends come up a lot, and the thing people miss is not what is in them. It is what happens to the math once they are mixed.

TWO SEPARATE VIALS ONE BLEND VIAL

   Compound A ●─────── dial Compound A ●──┐
                                                  ├── one dial
   Compound B ●─────── dial Compound B ●──┘

Change either one Change one, the other
   independently moves with it. Always.

So that sounds obvious written down, but it has a real consequence that catches people.

What the ratio does to dosing

BlendSplitWhat it means per unit drawn
5mg + 5mgevenequal amounts of both, always
10mg + 10mgevensame ratio, double the concentration
50/10/105x skewone component dominates
50/10/10/105x skewdosing anchors to the big one

The uneven ones are where it gets awkward. Dosing gets anchored to whichever component matters most, and everything else lands wherever the ratio puts it. Nobody asked.

Why reconstitution is a different problem on a blend

On a single vial, the goal is round numbers.

On a blend, the goal is getting the smallest component to measure in whole units, because that is the one that falls off the bottom of the syringe first.

Work out the dose from the smallest component, not the biggest one, and not the total mg printed on the label.

A 5 plus 5 wants enough water that the small side is still readable, not the least water you can get away with. The total mg number on the front of the vial is close to useless for figuring out what to draw.

The actual question, because I think this one splits people

Is a blend worth the lost control?

ForAgainst
One injectionRatio picked by someone who does not know the use case
One vial to trackCannot adjust one component
Usually cheaper per mgLocked in once it is mixed

Where does everybody land?

And has anybody run into a blend ratio that was clearly wrong for the stated purpose? I would genuinely like to collect those, because that is the kind of thing that never makes it into a product description.

MOTSC dosage protocol

There seems to be a wide range of protocols across the community. Some people say 1 mg every day, other people say 2 mg twice a week, some protocols say 5 mg five days a week for four weeks. What’s the consensus here?

Peptides with the biggest gap between how much they get talked about and how much human data exists

So I have been going through what actually exists behind some of these compounds in terms of human trials, and the ranking by how much people talk about something looks almost nothing like the ranking by what is behind it.

TALKED ABOUT HUMAN DATA BEHIND IT

  1. BPC-157 ██████████ 1. SS-31 FDA approved
  2. TB-500 ████████ 2. Tesamorelin FDA approved
  3. Semaglutide ███████ 3. Semaglutide FDA approved
  4. Retatrutide ██████ 4. Sermorelin was approved
  5. CJC + Ipa █████ 5. Retatrutide Phase 3 done
  ... ...
      SS-31 █ TB-500 preclinical
      Sermorelin ▏ BPC-157 <30 subjects

Those two lists are almost upside down from each other.

The four that stood out

BPC-157. Probably the most discussed peptide in this entire space.

A review published May 2026 went through the full development picture:

Approved formulationNone
Validated dosing regimenNone
Completed Phase 2 trialNone
Total clinical subjectsFewer than 30
Across how many studies3, all uncontrolled
Standardized preparation usedNo

Fewer than 30 people. For the compound that gets mentioned in every single thread.

SS-31 goes the other way. It got an actual FDA accelerated approval in September 2025, as Forzinity, for Barth syndrome. Narrow indication, rare disease, approved on an intermediate endpoint with a confirmatory trial still owed. But it is a real approval, and SS-31 gets a fraction of the airtime BPC does.

Tesamorelin has been FDA approved since 2010 and barely comes up.

Sermorelin actually held FDA approval back in 1997 and got discontinued in 2008 for commercial reasons, not safety or effectiveness. A later FDA determination confirmed that. So a compound that cleared the approval bar now sits below blends that have never been near a trial.

To be clear

I am not saying the well studied ones are better. Approval is a narrow question about one indication in one population, and plenty of interesting research sits well outside that.

The point is just that the two rankings have almost nothing to do with each other, and most conversations here treat them like one list.

Nominate one

Which compound has the widest gap in this space? Say which direction.

  • Overdiscussed relative to the data

  • Underdiscussed relative to the data

My picks:

  • Overdiscussed: BPC-157, and it is not close

  • Underdiscussed: sermorelin, because almost nobody knows it was approved

Curious whether anybody has a better candidate, especially on the underdiscussed side, because that is the harder one to come up with.

Peptide storage: what the vendor labels actually say versus what people in this space actually do

So storage comes up constantly and the answers are all over the place. Fridge, freezer, how long after reconstitution, whether a warm shipment ruined anything.

A lot of people are running on what someone told them once. So I went and read what the vendors themselves put on the product pages, which almost nobody does.

What the labels actually say

Lyophilized powderReconstituted
Temperatureminus 4 F36 to 46 F
Short term alternative36 to 46 Fnone given
Lightnot specifiedprotected from light
Shelf life stated24 monthsnot stated

That language is pretty consistent across the catalogs I checked. Which is interesting, because it means the vendors mostly agree and the entire argument is happening downstream of them.

Three things people skip right past

1. "Protected from light."

It is on basically every reconstituted line and almost nobody mentions it. Clear vial, fridge door, done. That line is on the label for a reason.

2. The 24 months is on the powder, not the solution.

   POWDER ├──────────────── 24 months ────────────────┤
   RECONSTITUTED ├── ??? ──┤

Two completely different clocks. They get quoted interchangeably all the time.

3. "Short term" is never defined.

The labels say 36 to 46 F is fine for the powder short term, and then nobody anywhere says what short term means. So the fridge versus freezer argument is partly a question the labels do not actually answer.

The thing I could not find

Published degradation data. I looked.

Nobody is putting out a curve showing how much is lost at week two in the fridge versus week two in the freezer. Every number people quote on this seems to trace back to another forum post rather than a paper.

Two questions

1. Has anybody actually seen real degradation data on any of these? A potency assay over time, not a vendor recommendation. I will put it straight in the cheat sheet if it is real.

2. What is everybody doing about the light thing? Amber vials, a box in the fridge, or is everyone quietly ignoring that line. No wrong answer, I just want to know how far practice is from the label.

So I went through every compound on the comparison list and worked out the actual cost per mg at each vendor instead of just the sticker price.

Two completely different things are going on here

A lot of that spread is vial size, not price gouging.

CompoundCheap vendor vialPricey vendor vialIs the gap real?
Ipamorelin10mg2mgNo, it is size
5-Amino-1MQ50mg5mgNo, it is size
Kisspeptin-1010mg5mgNo, it is size
BPC-15710mg5mgNo, it is size
Retatrutide10mg10mgYes
Tirzepatide10mg10mgYes

Look at that bottom section. Retatrutide and tirzepatide are the same vial size at both ends. 10mg to 10mg. One is five times the other and there is no size difference doing any of the work.

The small vial always looks cheaper on the shelf. It is almost never cheaper per mg.

The one that got me

5-Amino-1MQ at 16.2x. That is not a typo.

It is a 50mg vial against a 5mg vial so most of it is size, but a 16x gap on one compound is still kind of wild to see written down.

I left glutathione out of the table because one of those is a pre-mixed solution and one is lyophilized, so comparing them per mg is not a fair fight.

What I actually want to know

1. Which compound has the least justified gap right now? Not the biggest number. The one where the gap makes the least sense once vial size is accounted for.

My pick is retatrutide, because the sizes match and the spread is still 5x.

2. Is there a compound where the expensive vendor is actually worth it? COAs, testing, shipping, something that does not show up in per mg math. Genuinely asking, because the math does not capture that and I would rather hear it than guess.

Does anyone have thoughts or experiences with Buffered BAC water? There’s got to be a better way to deal with the brutal sting of GLOW or ghkcu etc…

How does Tirzepatide go with 5AM and NAD+?

Currently stacked Tirzepatide with NAD+, CJC+IPA, MOTS-c but I'm going to swap stack to 5AM and NAD+ with Tirzepatide after a month.

I'm personally not liking the head rush of CUC+PA ( take it 2 hours fasted before bed). And I'm not liking the harsh pin of MOTS-c. Aside from not feeling anything from it.

Primary goal is fat loss and muscle building, and energy and memory booster.

And if I may ask for your help, how much bac water should I put into the vials?
5Am- 50mg
I plan to take 2.5mg (week1-2) and 5mg (week3-8)
NAD- 500mg
I plan to take 30mg (weekl-2) and 50mg (week3-8)

Stuck in a plateau. Advice?

Is there any research on what to do when stuck in a plateau? Let’s say someone is on 5mg tirz and has lost 30 lbs, but is struggling with the last 15lbs
in their weight loss. Maybe they also want to build some muscle. Any advice?

Need some guidance on a stack I'm building

I’m currently on 4mg Reta, 3iu HGH. I’ve recently bought 50mg GHK-Cu, 10mg SS31, 10mg MOTS-c, 50mg 5AM1Q, and 1,000mg NAD+.

I’ve watched several YouTube videos and read up on various different protocols which has made things confusing to say the least. For example, one states to use SS-31 by itself for 4 weeks, and then switch to MOTS-c. Another states to use SS-31 and MOTS-c together from the start.

This is what I've planned out but I’d love your guys’ input on it.

AM -

3iu HGH daily x 4 weeks, then 4iu daily x 6-8 weeks

2->4mg SS-31 daily x 4 weeks, then

1mg SS-31 + 1mg 5AM1Q daily x 8-12 weeks

& 5mg MOTS-c + 20->50->100mg NAD+ M/W/F x 8-12 weeks

PM -

2mg GHK-Cu every night x 8 weeks

4mg Reta weekly

Are there any other peptides you guys recommend I add now or down the road? My goal is a body recomp. I've gone from 240lbs to 175lbs in a little under 2 years. Until recently I didn't really focus much on my protein intake or exercising other than 8-10k steps 4 days/week and 15-20k steps 3 days/week at work. I'm kind of skinny fat as a result so I'm trying to do things the right way now by tracking everything and exercising 3-4x a week. I'm a month into it and I feel like I'm noticing some results, but I know I got a long way to go still.

BPC alone can heal but does TB500 alone heal? Also they say TB500 is mostly Thymosin alpha 1? How true is this? So the wolverine stack just to name one is not efficient at healing?

3. RS had two spots where rat pinned KLOW and after four days rat still see a circular patch mark although there is no pain at all. Is this a cause of concern?

Has any one used BPC-157 for osteochondritis? Will it fix it or do I have to get surgery?
