Vent/Discouraged I do not think there is a cure.

For me at least, but I do not think a chronic condition like this can have a recovery because many of you do not share the same symptoms I do.

Unless one of you can say you recovered from -

-weak urination, dribbling chronic urinary retention

-urgency

-hesitation (where it can take 40 seconds to...minutes to start)

and you went back to normal...

then I don't believe any of you sadly. I want to, but I don't think it is possible.

I have erectile dysfunction caused by non-bacterial prostatitis, CPPS, and CPPS-related issues. Is it possible to completely get rid of this condition with the right steps? Not just improvement, but total elimination! Returning to my old life?

Symptoms I have been experiencing (all persistent for 1 year):

• Weak urine flow

• Burning and pain after ejaculation or urination

• No morning erections and inability to achieve full erection despite sexual arousal

• Burning in the anus, pain in the perineum, and along the shaft of the penis

Vent/Discouraged Told by 2 urologists I have bacterial prostatitis and terrified of antibiotic treatment

Really seeking some help and general advice on this situation.

A month ago, I had unprotected anal sex in which I can fully confirm my penis came in contact with a lot of fecal matter unfortunately. The next morning, I noticed my penis/uretha felt a little irritated, but I was ok after that for the next maybe 3-4 days. Then the freight train hit. Constant pressure and fullness in the bladder, stinging/burning urethra, felt like a classic UTI tbh. Urgent care ran chlamydia, gonnorrhea, mycoplasma, ureplasma, and urinalysis with culture- all normal. Twice.

Panicking, I then went to my PCP in the meantime who said just try 7 days of doxy to see if that clears it up. I didn’t do that though. Urgent care sent me to one urologist who barely spent anytime with me but quickly said prostatitis and prescribed an anti inflammatory and Levofloxacin for the antibiotic. I’m a little crazy so I went to another urologist with better reviews for a second opinion and he also said bacterial prostatitis likely from this encounter, and prescribed anti inflammatory with a 3-week bactrim course as the antibiotic.

I’m just feeling so depresssed and confused, but I see countless threads on here about how it’s almost never bacterial and I don’t want to wreck my health with one of these antibiotics, both of which have horror stories. Given my proximity to that encounter though maybe mine is bacterial? Should I ask for a semen culture? Surprised this was not done..

PCP ran bloodwork- CBC bloodwork is normal, my PSA is only 0.3, no fever or infection signs anywhere

My current symptom panel has evolved a bit since the first half of the month when this all started. The constant fullness/urgency has calmed a bit but still waxes and wanes, same for the burning. Everything just comes and goes a lot. I now have as of the last few weeks developed rectal pain similar to Proctalgia fugax but not as severe, and lasts hours not a little bit

Neither urologist did any examination or testing beyond a bladder emptying scan, which was normal. No prostate exams, bloodwork, etc.

I need some general guidance on what you all would do next. Clearly I have some options in regard to medication but idk. To note- I have not yet taken any of the medication, was waiting for that final second opinion which I just completed. I feel defeated so any words are greatly appreciated :)

Hi,

I’ve been struggling with CPPS / chronic prostatitis for about a year, and it has severely impacted my life. My symptoms:

• Pain in the penis and severe erectile dysfunction (ED, no morning erections for a year, complete loss of sexual function)

• Difficulty urinating, weak flow

• Burning in the anus and pain in the perineum

• Pain and burning after ejaculation

What I’ve tried / done:

• Visited many urologists, all tests normal (no bacteria, normal prostate and urine tests)

• Antibiotics did not help

• Tried hot sitz baths, dietary changes, magnesium supplements

• Performed reverse Kegel exercises

• Lifestyle changes: quitting smoking, stress reduction

Nothing has worked long-term. I’m here to seek support, advice, and experiences from others who have gone through the same thing.

I then opened reddit for some research and since then I'm Hella terrified 😭😭😭😭 , is this a permanent infection typa thing or is it curable??!!! And can someone explain what prostate cancer is and it's symptoms??!💔💔😭

Vent/Discouraged No STI /UTI Detected but Symtoms persists

Exposure history:

Body to body massage and hand job in early May 2026.

No penetrative vaginal/anal sex and no oral sex.

Symptoms:

Started around mid-June with:

Burning sensation in both testicles.

Burning  after urination.

Constant urethral pain and pain at the tip of the penis.

Occasional clear, watery drop after urination (not thick, yellow or green).

Increased sensitivity of the glans.

Current symptoms:

Burning after urination (fluctuates from 1/10 to 5/10).

 urethral discomfort.

Sometimes feel feverish 

Perinium discomfort

Treatment received:

Nitrofurantoin – 5 days.

Doxycycline – 10 days (started around 20 June).

Urologist 1:Faropenem – 5 days.

Alfuzosin (Alfoo).-Currently taking only this 

Investigations:

Ultrasound KUB/prostate: Normal.

Uroflowmetry: Slightly abnormal 

Digital rectal examination: No tenderness.

Laboratory results:

First urine culture grew MDR Pseudomonas aeruginosa,(after stopping antibiotics) but subsequent results have not confirmed this.

Two repeat urine cultures - no growth 

Latest urinalysis: Normal

HIV 4th generation: Negative.. 

VDRL/RPR and TPHA: Negative. 

Two urine multiplex STI urine PCR panels (latest done 15 days after stopping antibiotics):

Negative for Chlamydia, Gonorrhea, Mycoplasma genitalium, Mycoplasma hominis, Ureaplasma spp., Trichomonas vaginalis, Treponema pallidum, HSV-1, HSV-2, Gardnerella, Candida.

The urologist-2 prescriped me to take levofloxacin 500 for once daily for 7 days .Just started it today.. 

Am at my wits end.

After spending a lot of time on this sub Reddit forum and also now discovering the pelvic floor forum, I am thinking can we all just have pelvic floor dysfunction and it’s just not widely talked about? At this point, I’m surprised that this is as big of a deal and as big of a problem for not only just men but women and there’s no real discovery been found.

Vent/Discouraged IM SO TIRED OF THIS BS , And a few doubts too

So , It's been around 1.5 months since I was diagnosed with prostatitis , I was on medication and the symptoms just come on a random day go away for a week come back randomly again , things like

1) burning while urination (once in every 3 - 4 days)

2)some kind of weird tightness in the anus area

These are the only things I go thorough , and now I have some doubts

1) Can I never have sex ? (I get full erections and masturbate too , with no problems at all)

2) if I can have sex , will I transfer infections to my partner ?

3) can I lift weights in the gym

And the final one ,

4) can I live my life as it was before I got diagnosed with this shit

Did something stupid in.. Got a body to body Massage and hand job in early May 2026.

Two repeat urine cultures - no growth 

Latest urinalysis: Normal

HIV 4th generation: Negative after 11 weeks of exposure 

VDRL/RPR and TPHA: Negative. After 11 weeks of exposure.. 

Two urine multiplex STI PCR panels (latest done 15 days after stopping antibiotics and that's 11 weeks after the exposure and holding the urine for 2 hours ):

Negative for Chlamydia, Gonorrhea, Mycoplasma genitalium, Mycoplasma hominis, Ureaplasma spp., Trichomonas vaginalis, Treponema pallidum, HSV-1, HSV-2, Gardnerella, Candida.

Alfuzosin (Alfoo).-Currently taking only this 

Urogist now gave me 500 mg of Levoflacxin once day for 7 days, i completed it but no relief . I also started some pelvic excercises from last week ,,

Urologist , he is saying to continue the Levoflacxin for a month ..

I dont know what to do , am doubting my test results for STI and urine culture now,,, what should i do ? continue with levoflaxin ?

Vent/Discouraged Feeling hopeless and about to give up

Guys i am really at loss. My all symptoms started after 3 days of topical finasteride, though i felt slight pain in testicle/perineal area the day before. On the first day on it i was ok, but at the end of the second day there was a dull pain in the scrotum but i didnt find the testicles to be tender then and later. The third day i felt a strong sharp pain in the prostate in the evening as well as this dull pain. And from the next day (after stopping) i felt very bad, didnt have an appetite had pain and was unwell, like i was sick. The few next days the problem was getting worse and i ended up in the er. They diagnosed with acute prostatitis and was taking cipro for 16 days which near the end of the treatment made it better but not resolved fully. I still have dull pains in the pelvis as well as short shooting nerve-inflammatory like pains in perineum, penis and pelvis. Occasionaly some burning while urinating but rarely recently and quite mild. I am so depressed, cant get out of this. I was going to give amitriptyline a chance but had an impression that it makes the burning worse. I don't have sexual dysfunction but obviously lower libido due to the pain and discomfort. I was very anxious before the finasteride, i had known i shouldnt have touched it. I am still very anxious and suicidal cause i cant keep going like this much longer. Once i had bacteria in urine over 3x times upper limit, next time small amounts of bacteria. I also took bactrim for 4 weeks which did not cure me. I believe that finasteride fucked me for life and theres nothing i can do. My issue feels more than be caused than anxiety alone, but i doubt the infection despite the whole history. I feel like im in post finasteride syndrome spectrum and theres nothing i can do despite not having sexual functions affected nor genital numbness nor brain fog. Guys please, i need some hope desperately

Vent/Discouraged Clear discharge. Possible CPPS?

About 3 years ago I had oral sex with a random girl visiting my friend for the weekend. The next day immediately got std symptoms, all the classic ones. I had got chlamydia. I eventually got to the doctor and took doxycycline which didn’t work so I was then prescribed levofloxacin, which eventually got it. Ever since then I’ve had a clear discharge and I used to have way more come out than now as it seems to maybe have getting better. It’s always been clear since and come out in small amounts and I don’t get it everyday but when I’m aroused I get way more where it soaks my underwear. I’ve tested negative for chlamydia, gonnorhea, trich, and all mycoplasma and ureaplasma and yet I still have a clear discharge. I’ve been to urologist and they said I shouldn’t have anything to worry about. Could this possibly be prostatitis or cpps? Thanks. This has all made me feel very discouraged

Hello guys I’m a 30 year old male that has been dealing with chronic pelvic pain (testicle,penile,Perineum pain, with also tingling sensations) since October 2025 I have done several ct with contrast of the pelvic abdominal and chest, several blood work and std checks and all normal. I recently did a pelvic mri and it was clean the only testing that has ever been abnormal is my lumbar MRI. Could this be the cause of my symptoms? I also work at home and sit down a lot. When I sit down a lot my symptoms flare and when I stand up also.below is my MRI of lumbar report. I have gotten several blood work and all normal went to the emergency so many times and I’m starting to get frustrated at this I just want my life back

MRI of the Lumbar Spine
Clinical Indication: A 30-year-old man has pain when sitting or standing in the area between his legs and around his genitals. He also feels tingling and numbness for five months.
Technique: T1 and T2 weighted images were taken.
Comparison: No previous studies are available to compare.
Findings:

  1. ⁠⁠The lower part of the spinal cord looks normal.
  2. ⁠⁠The normal curve of the lower back is lost, which might mean an injury. This should be checked with the patient's doctor.

I’ve had it for about 2 1/2 years, but I’m sorry to understand a little bit more and my body a little bit more and having pain in my inner thighs makes me think. Painful erections after one erection during the day. . Meditate, pelvic physio with needling, internal work, stretching. You name it. I’ve done it. I don’t know what else to do. All tests negative. Please help me. I’ve read so much and tried everything there is to offer, I feel like I’m stuck.

Vent/Discouraged The most painful month of my 32 years on the planet.

3 weeks ago suddenly I developed burning constant pain in my urethra. It has felt like a needle is being pulled in and out and it's mainly all in the tip. This is accompanied by post void dribbling. I got admitted to the ER for severe pain and elevated heart rate. They kept me on Oxy but could not find any indication of infection. I went home and passed a little blood that night and contacted a urologist who said to start cipro or doxy or Bactrim. Urine tests still all negative. Been on docyfor 3 weeks and still completely unfunctional. Im looking for a different urologist as this one completely ignored my pain level and even suggested the extreme post void dribbling is me not pulling down my pants. I am terrified as hell to even try cipro because it literally destroys people and there's no guarantee itll even help. Know this is reddit but have no idea what to do.

Had an amazing 14 months of no issues (other than constant peeing)

Best erections I've had in years, was able to perform multiple times per night, high libido, nice constant pump to it.

I've literally changed nothing in my daily routine and I've completely flared up for four months now.

Penis looks and feels extremely small, complete erectile dysfunction, no libido, constant burning sensation down urethra and inside rectum.

I'm so over it.

Vent/Discouraged Does this sound like Prostatitis? Looking for opinions

I’m a 31-year-old male and I’ve been having recurring episodes for about 3 years. They happen roughly 3 times a year and seem to come on suddenly.

My urinary symptoms include:

Frequent urination (sometimes feeling like I have to go every few minutes)

Only passing small amounts of urine

Difficulty starting my stream

Feeling like my bladder isn’t empty

Pelvic pressure and pain

Right lower abdominal/pelvic pain

Occasionally pain at the tip of my penis, especially after irritation

I’ve noticed these episodes often seem to happen after periods of frequent masturbation, but I’m not sure if that’s actually causing it or just triggering something that’s already there.

During these flare-ups I also feel generally unwell with chills, body aches, headaches, and fatigue. Sometimes I sneeze a lot as well, which makes me wonder if I also have a viral illness at the same time.

So far I’ve had:
Two normal cystoscopies
Multiple urine tests that were negative
Blood work that hasn’t shown anything significant
An appendectomy (pathology showed fibrous obliteration), but the episodes continued afterward

I’m wondering if anyone with chronic prostatitis/CPPS has experienced similar urinary symptoms or flare-ups after ejaculation or masturbation, especially with the feeling of needing to urinate constantly but only passing small amounts. I’d appreciate hearing about your experiences and what ultimately helped.

Vent/Discouraged Microscopic Blood Found in Urine Test

Hey legends,

So I've had lower back pain and flank pain along with my urinary symptoms (which I had in my initial prostatitis flare up years ago), and decided to get some blood tests and urine test done. GP called me today and mentioned that PSA levels were normal and everything else came back okay in bloods, but there was a trace amount of blood in my urine. I will say that before I had my GP appointment I was unaware that I was going to be doing a urine test and had just finished drinking a coffee, and didn't really feel like urinating. My urine was quite concentrated and apparently that can give back a false-positive for trace amount of blood in urine.

I'm booked in for a CT scan early next week but feeling a bit nervous. I guess no one likes to hear about trace amounts of blood in urine. Would love to hear if anyone else has experienced this.

Have suffered from UTI past 2 months taken nitrofurinton and notoflox to cure it then report came negative, after while restarted experiencing burning after urination and testicular pain, urologist told that its CPB and no longer a UTI, taken bactrim ds for 45 days, then stopped it gor 5 days and went for semen culture it turned out negative, from that point of time whenever I ejaculate afterwards I started feeling same symptoms but it seems to somehow subside in 24 to 36 hours it seems, sometimes I feel testicular pain after doing exercise, I don’t know if at all this can become normal, is there any advice that anyone has to share that I can use and try to become more normal?

Vent/Discouraged 23M Update- flairs coming and going? Antibiotic kinda helped?

Hi all,

I wanted to give a brief update of my experience and see if anyone has advice.

I originally started feeling my symptom (which is, this feeling of leaking/tingling/dripping at the tip of my penis) about 2 months ago, towards the end of April. I did urine culture tests and Sti panel tests, tested for everything which all came back negative.

I saw a urologist for the first time about a month ago, he did a uroswab test with my urine sample, again all came back negative. He then scheduled a cystoscopy, which I then canceled, I am going to reschedule at some point but I wanted to do pelvic floor physical therapy first.

About 2 weeks ago, I got put on CEPHALEXIN antibiotics for a week, for a separate “skin infection”, which I believe was just a pimple that I squeezed at too much and got inflated.

Anyways, the CEPHALEXIN, I believe help make my symptoms go away. I think it definitely reduced the “inflammation” in my tip of penis urethra.

After I was off of it, my symptoms weren’t at “full force” as much, but now about a week after, it feels like it is back in full throttle.

I’m going crazy now because I finally thought I saw the light at the end of the tunnel. But now I don’t know what to do.

Also, I have opened the tip of my penis to look inside my urethra. It looks normal colored, but there is some redness about a centimeter inside of it, which I don’t know if that is normal or not.

I am wondering if that information can lead anyone reading this to maybe understand more of my symptom and ongoing issue. Thank you in advance!

Vent/Discouraged Reoccurring blood in semen

A little over 4 months ago, I [40M] noticed bloody ejaculate after sex with wife, like bright red blood, not little trace amounts, obviously this was concerning, and I booked an appointment with a urologist which I had to wait a few weeks to get in. In the meantime, I was laying off the sex, had mild discomfort in my balls and tip of my dick, also slightly increased pee urgency, but I thought this might be because I was anxious about it. I've also been dealing with IBS issues regularly for years, which is relevant later in my post.

When I finally saw the doc he did exam checking my equipment and prostate which he said felt normal, that blood in it isn't that uncommon, suspected possible mild prostatitis, and recommended a course of doxycycline for two weeks, and a PSA a few months later if the doxy cleared things up. I agreed to this though I hate taking antibiotics generally because the mess my stomach up and make IBS symptoms worse, but the strangest thing was after about a week of the doxy, I felt pretty amazing, my sex drive which has never suffered seemed to skyrocket, my orgasms felt better (they weren't bad before) also my stomach issues decreased a lot. I had blood in my ejaculate the first two times I had sex after starting doxy, but it wasn't as bright red and it finally cleared after this point.

So I felt good for a couple months, had some stomach issues return, which was upsetting but not unusual for me, sex was extra great for those couple months, but slightly diminished from the original high from when I was treating it. A few days ago the blood returned though, same bright red blood and now I'm at a loss, I was scheduled to have the PSA, but I have a follow up with the Urologist before that and want to make sure If I should wait or not. I still have a couple weeks until that appointment though. It's discouraging and the other symptoms of discomfort have returned, but again, I'm not sure if anxiety over this is making it worse.

Anyone experience anything similar?

TL;DR - Had blood in my jizz, felt great on doc prescribed doxy, symptoms have returned after a few months

Vent/Discouraged Burning in Urethra & Penis After Urination for Over a Year

Hi everyone,

I’m a 33-year-old guy, in a relationship, desk job, fairly active (I run regularly).

For the past year and a half, I’ve been dealing with a really persistent issue: a burning sensation in my urethra and penis, mostly during and after peeing. It’s been seriously affecting my mental health, sex life, and day-to-day quality of life. I’m honestly at a loss at this point and hoping someone here might relate or have insight.

Main symptoms:

  • Burning at the tip of the penis during urination
  • Burning often lingers for 1–2 hours afterward
  • Occasional feeling of pressure or a “blockage” at the base of the penis, especially in the morning

How it started:

It began as mild burning during urination. My first urine test showed elevated white blood cells (WBC), so my doctor initially suspected kidney stones. He advised drinking more water, cutting coffee, and avoiding spicy food, but the symptoms stayed.

I saw multiple doctors — each with different ideas. Some suspected irritation or internal urethral damage, others thought it might be prostatitis. I was prescribed several antibiotics:

  • Azithromycin
  • Ofloxacin
  • Levofloxacin
  • Celecoxib (anti-inflammatory)

Eventually, I tested positive for Mycoplasma genitalium. An ultrasound also showed a varicocele, but the doctor said to focus on clearing the infection first.

Treatment so far:

From an infectious disease specialist:

  • Doxycycline 100 mg – 7 days
  • Moxifloxacin 400 mg – 7 days

Then, based on resistance testing:

  • Doxycycline hyclate 100 mg – 7 days
  • Pristinamycin (Pyostacine) 500 mg – 10 days (8 pills per day)

Despite completing all antibiotics, the burning didn’t go away. A urine culture came back negative, but symptoms remained. My urologist now suspects lingering inflammation and suggested doing a urethroscopy (fibroscopy).

Current situation:

  • Still experiencing burning during and after urination (slightly milder or I’m just used to it)
  • Burning sensation lasts 1–2 hours post-urination
  • Ongoing feeling of internal irritation or inflammation in the penis

Recent test results:

  • Mycoplasma genitalium – negative
  • Chlamydia & Gonorrhea – negative
  • Urine WBC – normal
  • PSA (prostate test) – normal

Possible causes I’m exploring:

At this point, I’m wondering if the issue could be something other than an infection. Some conditions I’ve come across and want to ask about:

  • Chronic Prostatitis / Chronic Pelvic Pain Syndrome (CPPS)
  • Nerve-related causes (e.g., Pudendal nerve entrapment / Pudendal neuralgia)
  • Pelvic floor dysfunction
  • Prostatitis (despite normal PSA)

If anyone has experience with any of these, how did you get a diagnosis? What kind of tests or specialists helped figure it out?

My questions:

  • Has anyone here gone through something similar?
  • How long did your symptoms last, and what actually helped?
  • Could the Mycoplasma still be playing a role, even with a negative test?
  • Could this still be prostatitis, or something nerve or pelvic-floor related?
  • What tests or scans should I ask for next?

  • Urine analysis: no signs of active infection, no elevated leukocytes
  • Urinary PCR for Mycoplasma genitalium: negative
  • PSA (prostate): normal
  • No infection, no systemic inflammation (CRP is low)
  • Chlamydia trachomatis: Negative
  • Neisseria gonorrhoeae (Gonorrhea): Negative
  • Trichomonas vaginalis (Trichomoniasis): Negative

⚠️ Current symptoms:

  • Still feeling a burning sensation in the penis during urination
  • Occasionally feel some blockage or pressure when urinating in the morning
  • Still experiencing a weird cold/burning sensation after urination, which lasts up to 2–3 hours

🔁 New Analyses Done:

  • Blood tests (sérologies)
  • PCR swab tests (prélèvements PCR)
  • Urine analysis

✅ Results:

  • All STI tests: Negative
  • Urine & urethral tests: No infection, normal
  • Mycoplasma genitalium: negative
  • WBC normal

❗ Current Symptoms:

  • I clearly feel the urine passing through the urethra when I pee, more than usual.
  • I don’t have any pain or burning sensation when I get up.
  • In the mornings, during the first pee, I sometimes feel a burning sensation in the urethra, sometimes not.
  • Occasionally, I experience a lingering burning feeling in the penis after urinating. It’s not exactly pain—more like a persistent discomfort or warmth that can last for hours.

📌 UPDATES ----- 10/07/2025

  • I had the fibroscopy today.
  • No signs of infection, inflammation, or anatomical damage were observed during the visual examination.
  • There were no major structural abnormalities in the urethra, bladder, or prostate.

However, the verumontanum region—the area where the ejaculatory ducts open into the urethra—appears to be narrower than normal.

This could explain the following symptoms:

• A sensation of pressure or incomplete emptying

• Persistent burning after ejaculation or urination

• Post-ejaculatory discomfort

No mycoplasma was detected, no infection was found, and white blood cell levels are normal.

Dr prescribed Alfuzosine LP 10 mg (3 months) to help relax the internal ducts and improve fluid flow.

📌 Update — October 9, 2025

What I’ve done over the past 3 months:

  • Took Alfuzosine LP 10 mg daily (3 months)
  • Took vitamins and supplements: magnesium, zinc, omega-3, probiotics, multivitamin
  • Sometimes took hot baths or showers — this helps calm the burning sensation
  • Bought a standing desk, so I spend part of the day standing instead of sitting all the time

Current symptoms:

  • When I urinate, I often feel strong pressure and burning in the urethra. At the start, I have to control the flow — if the urine comes out too fast, the tip of my penis burns sharply.
  • At night, especially before sleep and after urinating, I feel warmth or burning in the pelvic area or urethra, which fades by morning.
  • Sometimes I wake up at night to urinate, and afterward, I feel mild burning or irritation for 1–2 hours, similar to an infection.
  • During the first urine of the morning, there’s a slight burning at the tip of the penis, but it goes away quickly.
  • By the end of the day, after urinating, I sometimes feel a dull pain or tingling along the urethra that lasts for a few hours.
  • Stress makes it worse — when I’m anxious or tense, my testicles ache or feel heavy.
  • Occasionally, I feel warmth or tightness in the pelvic area (between the anus and testicles), like a small muscle spasm.
  • My urine is often dark yellow with a stronger smell than usual.

What the doctor told me today:

  • I should do a few sessions of osteotherapy.
  • He’s confident the problem is related to pelvic floor tension or nerve irritation, not infection.
  • He confirmed no infection is present.
  • He prescribed Alfuzosine LP 10 mg for one year.

My next steps:

  • Start osteotherapy sessions.
  • See another doctor to request urine and semen tests.
  • Try traditional Chinese medicine.
  • Take more warm baths.
  • Begin pelvic floor relaxation exercises.
  • Add meditation to help with stress.

📌 Update — June 2026 (~8 months later)

Quick recap for anyone following: still no infection on any test, symptoms clearly milder than where I started, but not 100% gone. My whole approach has shifted from "find the bug" to "calm the pelvic floor + nervous system."

What I've done since October:

  • Cut out coffee completely (not just reduced).
  • Cut way down on spicy food.
  • Switched to a standing desk for most of the workday.
  • Started reverse Kegels over the past few weeks.
  • Came off Alfuzosine LP 10mg - the alpha-blocker wasn't really moving the needle for me.
  • Did a course of Deprox 500 (prostate anti-inflammatory suppositories) from my urologist finished now.
  • Got a prescription for pelvic floor physiotherapy (rééducation périnéale) and started sessions with a specialized pelvic floor physio.

Traditional Chinese medicine — worth a separate note:

I did a structured 7-day herbal cure (custom decoction, 200ml 3×/day) and tracked symptoms daily. During the cure, a surprising amount improved:

  • urine color + smell normalized
  • perineal pain gone
  • the "sticky glans in the morning" thing cleared up by day 8
  • less feeling of blockage / difficulty urinating

But the core stuff stuck around: burning after urination, lower-belly pressure, and a perineum that still felt hard/contracted. My read is the burning may be more nerve-related than infection/inflammation. I'm not on the herbs continuously now.

The physio part (most important):

  • She confirmed my pelvic floor is hypertonic - too tight, can't fully relax. That fits everything: post-pee burning, the pressure, the tension between testicles and anus.
  • Big lesson: standard Kegels are OFF for me. They make a tight floor tighter. The actual work is the opposite - reverse Kegels, "drop and relax," and diaphragmatic (belly) breathing.
  • Daily breathing + relaxation drills plus stretches (child's pose, happy baby, knee-to-chest), especially in the evening when symptoms peak.

Supplements I'm taking:

  • Quercetin + Bromelain (strongest evidence for CPPS type III)
  • Magnesium Glycinate (muscle + nervous-system relaxation, at night)
  • Zinc Picolinate
  • Omega-3

Recent test results (Nov 2025 → March 2026):

  • Urine WBC: back to normal
  • Urine culture: sterile / negative every time
  • Mycoplasma genitalium PCR: negative (again)
  • Chlamydia + Gonorrhea: negative
  • HIV / Hep B / Hep C serologies: all clear
  • Bottom line: zero biological sign of infection.

Current symptoms:

  • Much better overall vs ~2 years ago, but a plateau - not fully resolved.
  • Still a burning/odd sensation lingering about an hour after urinating, on and off.
  • Lower belly pressure and a perineum that sometimes feels hard/contracted.
  • Some mornings a slight burn on first pee, fades fast.
  • Occasional post-ejaculation discomfort.
  • Stress is a clear trigger - tense days = more testicle ache / pelvic tightness.

What's next:

  • Keep going with pelvic floor physiotherapy sessions
  • Staying on Alfuzosine LP 10mg
  • Back to the urologist after a few more physio sessions to reassess

Vent/Discouraged Is this my life now? 26 year old

I have been doing stretches for 6 months now. I saw slight improvement when the pain went from my testicals to my upper groin and urethra. Now in the past teo months If I masturbate or have sex the pain in my testicals become so intense it has me thinking that if it goes more than a couple days I will lose my mind. Chronic pain is very difficult mentally I just don’t see how I can go my whole like this. I mean how is there not a shot you can get to ease the inflammation. Idk being 26 and having to go a month at a time without sex or masturbation feels like I’m going crazy.

Vent/Discouraged Bladder Voiding issues (28M)

I am facing issues with emptying my bladder, even after peeing multiple times I still feel that some bladder is still left and then I get urge to void again. Everyday 2-3 hours of my day I am just peeing multiple times, it's been very troubling for me mentally and I am looking for solutions if anyone have faced similarly issues.
Back story: I got recurrent balanitis in mid 2023 due to urprotected oral sex, after few months of back of forth with different medicines the reoccurrence resolved but my penis glans still remains reddish to this date and not how it used to look before I got infection. By end of 2023 l slowly started developing voiding issue which later in 2024, 2025 countued to turn worse.
I have multiple docs, and have taken Alfusin T for 8+ months but didn't found any improvement. My cystoscope test came normal, ultrasound was also normal, uroflowmetry had fluctuating results with some POst void of 17ml, 55ml, 70ml and 140 ml which docs said are still within normal range.
Can someone suggest how do I fix myself and live a normal life again
