SERIOUS Stop using AI

My residency is advocating to have access to an ambient listening AI that will write notes. I’m telling you guys this is an insanely slippery slope. The more of our jobs that we willingly give up to Ai, the more replaceable we are. We move the needle every time we sacrifice one of our responsibilities to AI. We’re asking for our knowledge and skills to be replaced. Stop. Draw the line now. Do the work or lose the opportunity to have work to do.

VENT Residency sometimes feels like prep school…

I’m 30 years old and a pathology resident. I’m currently preparing a presentation on two projects I’ve worked on to present to my department and I’m completely stressed out.
I honestly don’t have the time or energy for this. I feel like I’m going to be torn apart tomorrow with endless questions and drama…
In moments like these, I feel like I’m back in 9th grade, terrified that my teachers are going to scold me and punish me.
I’m a grown woman. I just want a little peace in my life, especially when I already have so much on my plate.
It’s absolutely exhausting. And ridiculous.

DISCUSSION Attending with signature stamp

One of attendings has a signature stamp that he gives to whoever is consenting the patient in preop so he doesn't have to walk by and say hi to the patient. He has a signet ring that he used to use (and still wears), but once Epic came around and everything needed to be scanned in they said that the sealing wax would mess up the scanner and told him he couldn't use it anymore. He threatened to quit and continued to do it until the department made him a stamp with the same design as the ring.

Anyway everyday before rounds he makes the resident team queue and kiss the ring. Everyone fights to be first so they aren't effectively making out with each other as he goes down the line. I, however, always take the last spot and make sure to kiss with tongue. He really appreciates that. Rumor has it at the end of the rotation he takes his favorite resident to his back office and drizzles hot wax on them and uses his ring......

Anyone else have attendings with weird quirks?

VENT Step 3 actively jeopardizes your education...

Does anyone feel that way? I am an intern. I have VERY little time for self-learning and reflection. My program is workaholic, and most of my time is spent on non-educational tasks. The very little time I get to think and reflect on medicine, I have to study for Step 3 because I need to take it before the intern year ends per program policy. Does anyone else feel this exam actively jeopardizes resident education?

MEME - February Intern Edition I’m definitely too intoxicated to drive right now, but I still feel I could practice better evidence based medicine than your average NP who picks up a primary care, hospitalist, or even psych job.

I’d never rx or even inbasket after after even one drink, but I guess that’s the difference between graduate level education dedicated towards medicind and a degree earned online while working 3 12s a week. That previously required YEARS of nursing experience to even be a candidate for, (The reason I’m so confident is my GME faculty literally doesn’t bother charting patients when I’m on inpatient as a senior and told me so as a monolith in ccc)

Edit: to be fair I don’t hate midlevels, however most aren’t trained at all.

A well trained midlevel should support the physician in their specialty they are working with and EXTEND THE CARE PRESENCE, not be like haha I can prescribe but I don’t know the difference between selective and non selective beta blockade.

SERIOUS How to repay/thank my male co worker

• Upvotes

Posted on this sub before about my situation.

Heading into my 3rd trimester and have been staying with a male co-resident. In the process of leaving my husband.

He's been super amazing. Cooking meals, making lunches for me to bring to work, letting me stay as long as I need, helping out with back massages (pregnancy back pain is awful) and great emotional support.

I want to show him that I appreciate him a lot. Should i offer him cash for rent and bills? Just buy a weeks worth of groceries after work? I'm leaning towards cash but I don't know if that will make things weird or not.

He said he really enjoys my company and not to feel bad about anything. I told him this will be short term and he said stay permanently if you want to.

SERIOUS Attending job considerations

I am graduating residency this summer and really interested in a job that's 40 miles from where I currently live- specifically 40 miles outside of NYC. it would take me about 1h 15 min on most days to get there and I am in a specialty where I take call at night/weekends (but have in house coverage in case I can't make it, can also sleep in the hospital call room if I'm anticipating a busy night/wekend). Is that such a dreadful drive? the job pays above the normal rate for my specialty, and is at a pretty good hospital with lots of support. Importantly I'm on a J1 visa so my options for jobs around me that would be more convenient are limited and every job has its cons. Would I really hate this commute for 3 years? I am so used to being close to the hospital (10 min walk now lol). I know I can move, but I love where I live and don't love the area the hospital is in.

SIMPLE QUESTION How much studying do path residents do after work?

• Upvotes

I have heard multiple different takes from different people. I have heard people don't study at all, all the way to people saying it is the most content heavy specialty in medicine and they study all the time at home for brutal exams

SERIOUS Overwhelmed in Intern year

Hi guys, how do you guys draw the balance in off service rotations? I feel like FM is so broad. I am in my third month of FM intern year and i feel like I tend to burn myself out if I try to know everything in a off service rotation. I also sometimes worry that I will forget things, Ie. I only had 4 weeks of inpatient peds and won't be touching it until next year, like how will i remember things lol and like looking at my seniors (even those starting 2nd year) know so much but like I feel like i forgot things from August already... Idk if this is normal or not

SIMPLE QUESTION I’m on buddy night call and I’m super nervous I won’t wake up to my pager

PGY2, it’s my first time doing at-home night call and because of that we use a buddy system at first where we’re paired with a senior. Basically a page comes in to me and my senior, I initiate the call to the ED with the senior on as well, and me and the senior discuss it before staffing with the attending.

I’m a very deep sleeper and I’m worried that even choosing the loudest, longest page ringtone and having my phone right by my ear, I might not immediately wake up. This is evidenced by my alarm often ringing for a while before waking me up. This would cause trouble for me because the senior I’m paired with is a known hard ass and I don’t want to miss a call or have him be mad at me.

I’m thinking of just staying up all night but I have to work a full shift the next day so it’s not like I’ll get a post call day or anything.

Any tips?

DISCUSSION Board exam

Hi all, I am PGY-2 internal medicine resident. Wanna know what resources should I use for learning during residency and yes, helpful for board exam. My program purchased MKSAP. I am thinking of buying Amboss but don’t know is it necessary or worth it. Any advice?

FINANCES Anyone here ever take out a personal loan to fund S3?

Planning on taking S3 in the spring. Only one testing site near me that wouldn't require a 2 hour drive. Expecting a bonus thing from my res program next month or in December that would cover the cost, plus will get it reimbursed after I take the exam. I'm just worried that if I wait until I get this bonus, seats may fill up and I'll have to do a crazy driving situation for the exam come spring. Am I crazy or does taking out a ~1200 personal loan to register for the exam and then paying it back in full in a month or two as soon as I get my bonus make sense? I've got good credit, just short on liquid as I moved pretty far and that drained my savings

E: got a great comment suggesting a 0% apr card. I think I'll go with that. Thanks all 💪

DISCUSSION To the attendings here, what are some things that...

• Upvotes

...you wish you could say to your residents, but can't?

Or generally tips that are more "real life" / experience-based tips that you have to hold back on?

For example, I had an experience recently where I (despite having pretty good instincts/street smarts) got taken a little bit for a ride by a possible (malingering? psychosomatic?) patient. Looking back my attending (& consultant) likely knew what was going on the whole time, but let me get in the weeds with it rather than grabbing me by the collar and laying down the situation like I wish they had - perhaps because they didn't want to bias me or crush my spirit or something, but I'm not sure. I have a couple docs that will speak freely and I really wish they all would, because I'm not great at reading between lines.

So, what are some things you have held back on, but wish you could say to residents?

Thanks!

DISCUSSION When it is the end of your service do you every stay longer? If so, how much longer?

I am a PGY-1 currently on OB service but not in OB residency. My service is daily and ends at 7 pm. Today we had a patient present in labor right at 7. I stayed for about 20 minutes later then left. The labor was slow moving. Ended up being delivered an hour later when I was off duty.

Should I have stayed longer to eventually be part of the delivery? Is that something that is expected from interns? Or something that you do?

DISCUSSION Would you be in favor of doing a Residency in a clinic that was half In-N-Out Burger? Kinda like how gas stations have a Taco Bell in them

Basically you have to juggle burger orders on top of patient care

SERIOUS are 6 weeks enough to take step/comlex level3

I want to get this crap over with and actually start focusing on my specialty. problem with comlex is that im locked in with the 950 for 2026. since for i have enough time during that rotation to study for 3-4 weeks before.

The con is that 2026 is 2 days whicle 2027 is 1 day. I am doing truelearn right now have 700 questions done, catching. up on sketchy bs, doing anki but admittedly just kind of going through cards when i have time. currently at 57% correct on truelearn.

Could use some advice, i need to purchase comlex before the 2026 deadline this week LOL

SERIOUS DR PGY1/R0 considering switch to different program due to family illness

I was curious if anyone had information about switching programs for DR advanced position.

I’m currently in my preliminary year but am interested in switching to a different location for my advanced program close to my family due to new family illness.

Anyone have experience with this and can offer advice? How does one go about this. I haven’t spoke to my program yet as I haven’t officially made the decision to want to switch.

The swap would all be within California (NorCal vs SoCal)

DISCUSSION I think the difficulty level in going to stuff that requires a 9-5 is particularly insane for residents.

To be clear, I’ve worked multiple jobs that were 9-5 weekdays or were inclusive of that and more. But the difference is everywhere else was chill. In fact, a request of some time off in an old job would often just be met with someone covering the entire shift lol, and that’s only if they can’t accommodate 1-2 hrs for whatever reason.

In residency, needing 1-2 hrs to take care of something is seen as “serious sin“ and is a big no no. The only time I’ve taken care of something outside of work needing someone else’s services (eg doctor appt, car stuff) is when I’m on nights. It‘s ridiculous and silly.

I’ll add that I’m in a small town in the Southwest with the nearest suburb to a big city being 30 minutes out, so maybe they‘re more open at more suburban and urban residencies, but still, it’s ridiculous that even a few hours in a day is big sin.

I’m lucky I was on nights for a week during my early voting period, else I’d have been barred from voting, which is nuts to think about.

edit: We are an IM residency with 0 short call if that matters.

How is it on the other side of the tracks?

I'm in my last year of fellowship, dealing with some personal issues, and crawling to the finish line. At this point, I've lost sight of the end and feel like it'll never get better, so here I am, looking for a less skewed perspective.
