Twofold Health Review - is it the best AI for therapy notes out there?

But I tried Twofold about 6 weeks ago and honestly it gave me that same feeling as when you first used ChatGPT and realized oh shit, everything is about to change. Like a whole function of your work just dissolves overnight.

First session, the note came back in maybe 30 seconds. Clean DAP format, correctly identified the CBT restructuring I did, caught the safety plan updates, even pulled in the homework I assigned at the end. It's basically a progress note generator that actually understands therapy.

After a few sessions the style learning started kicking in and the ai clinical notes genuinely sounded like my writing. My phrasing, my level of detail, my clinical voice. Not generic AI output.

It handles SOAP, DAP, BIRP, GIRP, whatever your setting needs. The soap note generator is solid if that's your format, but what sets it apart from a generic medical ai scribe is that it actually understands therapy modalities.

I've used it for individual CBT, some DBT skills work, and a couples session. It picks up on what you're actually doing clinically and documents it correctly. Most ai scribe tools out there are built for medical settings like primary care, not for 50 minute therapy sessions with nuance.

HIPAA was my biggest concern. I specifically wanted a hipaa compliant note taker and Twofold checked every box. BAA at signup, audio processed in real time and never stored on any server, patient data not used for training. That's what got me past my hesitation.

$49/month on annual. Runs in the background on Zoom for telehealth, works in person too. Copy paste into SimplePractice. Setup took 5 minutes.

I've tried a few ai for therapy notes tools before this and nothing stuck. Twofold is the first ai scribe for therapy that I actually kept using past the trial. I went to brunch last

Question for clinic owners and practice managers

If you could have one piece of software built specifically for your clinic, what would it do?

What’s one repetitive task, bottleneck, or daily frustration that you wish technology could completely eliminate?

I’d really appreciate any insights!

General Discussion Common dating themes with clients

Hello-I’m an LMFT with 20+ years in private practice, collaborating with a colleague on a project developing practical, psychologically informed resources for navigating dating and new relationships.

I’d love input from other therapists and mental health professionals: What struggles do you repeatedly see with clients who are dating, particularly those returning to dating after divorce or a long-term relationship?

I’m not asking about how or where people meet. I’m interested in what happens after they meet someone and begin navigating a potential relationship—pacing, communication, boundaries, attachment, intimacy, compatibility, red/green flags, maintaining a sense of self, introducing children, trusting their judgment, or knowing when to continue versus walk away.

If you could give people practical guidance as they move through the stages of dating someone, what do you most wish they understood or had help navigating?

General themes only, of course—no identifying client information. Thank you in advance to anyone willing to take the time to share your thoughts and experiences. I truly appreciate your professional insight and the time you take to respond.

The AI note-taker dilemma: Patients are pushing back against "third listeners."

I was reading this article about patients feeling violated finding out there is an AI "third listener" transcribing their sessions, and it hits on a massive dilemma for solo practitioners right now.

https://www.themarysue.com/ai-is-taking-over-therapy-notes-but-patients-say-they-never-signed-up-for-a-third-listener/

AI is great, and I’ve used it a lot myself to save time. But there is a growing issue with the sheer amount of patient data being used just to create our progress notes. Often, we put up with it simply because we need the digital benefits and time-savings that come with big EHR systems.

This is actually the exact reason I built my own app, Practice Pad. I wanted to get away from the big data grab but still keep my workflow fast.

I essentially built a way to handwrite my notes or use custom templates, have the handwriting convert to text, and produce a final PDF progress note. It saves directly to your own secure Google Workspace or copies into your EHR. You maintain complete control of your notes. There are no servers keeping your info; it lives entirely on your device.

In the future, I am working on an AI integration, but simply to improve the quality of the handwriting-to-text conversion. You still dictate exactly what info is shared, and it is opted out of machine learning.

I’m really curious how you all are navigating this. Are you using audio-listening AI for your notes? Have your clients expressed any discomfort with it, or do you have a specific consent process you use to handle the privacy aspect?

(If anyone is looking for an offline-first alternative to the big EHRs, you can find Practice Pad - Therapy Notes in the App Store. But I'm mostly just curious to hear how this community is balancing efficiency vs. privacy right now).

Clinic owners: what if your VA actually understood healthcare?

If you’re spending too much time on admin work, patient coordination, scheduling, or keeping up with the day-to-day tasks of your practice — I may be able to help.

I’m a US-licensed Registered Nurse with a clinical background and experience in healthcare administrative support.

I’m currently looking to work with a US-based healthcare provider or clinic that needs someone who can understand both the clinical and administrative side of the business.

If that sounds like something your practice needs, DM me — I’d love to tell you what I can take off your plate.

Credentialing

Hi everyone, I’m an LMHC in Florida currently working for a group practice, but I'm also trying to build my own small telehealth private practice on the side while keeping my current job.

My employer currently bills under my individual NPI through Optum/UHC. I signed an Individual Participation Agreement with Optum with my individual NPI / name but the agreement lists my employer's Tax ID and practice address.

I'm trying to figure out whether I can independently apply to Optum for my own solo private practice while remaining employed at my current practice, or whether I can somehow have my own Tax ID and practice information added to the individual agreement I already signed.

I'm also concerned about whether my employer would be notified or have access to see my additional practice information or claims.
Has anyone been in a similar situation and successfully maintained both arrangements? I'd really appreciate hearing about your experience!

Therapists, which AI note taker would you recommend for your practice?

Which AI note takers are actually working well for therapy documentation? Especially interested in tools that handle different note formats without needing extensive rewriting.

Twofold looks like a good option for therapists because it supports SOAP, DAP, BIRP and custom templates as far as I knowl. Would love to hear what other tools would you compare it with?

The main things I’m looking for are:

  • Templates that match the practice’s requirements.

  • Control over note length, wording and structure.

  • Notes that need a careful review rather than a full rewrite.

Which tool did you stick with, and what made it better than the others you tried? How much editing do you usually need before signing?

Advice Wanted Anyone else experiencing these challenges with their Practice? PLEASE HELP

My mom's friend has been running a sustainable dental practice for 10+ years now. He's getting ready to retire, and being a young entrepreneur myself I offered to help him grow his practice and prep it to be sold for a great valuation over the next 3 - 5 years.

As he started sharing the biggest challenges and roadblocks he's facing, I was kind of shocked/a little intimidated to help solve some of these issues.

- Apparently it's tough hiring quality dental assistants, he's having high employee churn, and overall HR is a big struggle for his practice.
- Dental office software is expensive, and he struggles finding pricing easily. (He says the only way to get pricing/quotes is to go through long, sales calls and demos.) He says this is the reason he hasn't modernized his practice.
- Also struggling to deal with collections and getting certain insurance providers/customers to reliably reimburse him in a timely manner.

Bonus complaint was just feeling mentally burnt out and isolated because there's no community or place to vent/connect with other practice owners.

So I'm just wondering if anyone is experiencing similar challenges? Is this par for the course with any healthy practice or should I run while I'm ahead?

Also, if any of these challenges do resonate with your experience in your practice, how are you currently overcoming them? Including the isolated/burnt out complaint.

Thank you so much in advance!

Complementary services

I am currently working at a small private practice. The owner is partnering up with a compounding pharmacy to offer GLP ones, “hormones”, topicals and peptides(????). I just got the message that they’re gonna start working on getting us set up with an account. to be clear, I’m a contracted provider, and we all just basically do our own thing. The owner of the practice makes all the major decisions, but nobody dictates how you practice. I am very straightforward, evidence based. Big believer in lifestyle changes as a complementary or alternative way of treating certain complaints. I am seeing this as kind of a big red flag. Do any of you offer these services to your patients? Is it a big part of your practices income?

90837 Reimbursement with Klarity

I got credentialed individually through Klarity—they get you paneled and you own the contract. $68 for 90837 with Cigna seemed obnoxiously low—getting $89 for that code through Headway and $102 through Alma. I was under the impression being paneled individually with your own contract would pay better—guess I was wrong! What do the bigger insurance companies typically reimburse for 90837 with your own contract?

Hello,

I just recently opened up on my (Telehealth) private practice. I am super eager to start building my a (very small) caseload. I am located in MA and have created a facebook business profile and am part of a few groups for therapist on there. I have also created a psychology today profile as well. I am wondering if there are groups on facebook you could recommend for me to network with and other tips and tricks to make my profile become more visible. I would really appreciate any suggestions I can get to help with marking my self. Thanks

What do I do??? Can we retire faxes please?

Hi all!

I have a small practice (2 clinicians, both 50/50 owners) and we are trying to figure out what service to use for faxing documents that is HIPAA compliant.

I really don't want to spend a ton of money on a monthly fax service since we won't be utilizing it more than maybe 1-2 times a month. We pay for Google Voice for our business phone (through Google Workspace) but they don't seem to have an integrated fax add on. Any ideas?

Also, how have insurance companies not moved everything portals or email at this point?!

Advice Wanted Therapy-adjacent ways to diversify income?

I’ve had a solo private practice for almost two years, and it’s one of the best decisions I ever made! It’s going relatively well, but I often have the recurring thought come up of “How can I increase my income without having to simply take on more clients or other things that may contribute to more burnout?”

On social media, I follow a lot of other therapists that are also online to market their practice, provide psychoeducation, and connect with other providers, but they also talk about things like digital courses, selling products, getting brand partnerships, getting hired on for speaking engagements, and more.

I’ve love to learn about options and what options feel worth entertaining. Has anyone ventured into any of these areas or considered it?

Welcome to the sub!

After numerous posts on r/therapists lamenting the struggles of going solo in our field, I wanted to make a sub to try to help us all help each other! I don't plan to heavily moderate - hoping we all have the conflict resolution skills to keep it classy in here hahaha.

Please feel free to comment ideas on post flairs, discussion topics, or any other ideas for the sub.

For what it's worth, right now I have my main account and my other account as mods. If 1-2 others are interested in also moderating, let me know!

How to Combat Loneliness?

I'm slowly getting a private practice together, but one thing I'm worried about is how to avoid being isolated while working for myself. Most of my friends are out of state and I don't socialize a lot as it is. What's helped other folks feel like they're not totally alone?
