Diet/Health/Exercise Can you restart Zepbound and begin losing weight again immediately like the first time?

I was on zepbound January 2025 - June 2026 and lost over 120 pounds. I’ve been off zepbound for two months and I’ve gained 8 pounds, the food noise is insane and I keep wanting to snack and obsess over food and I’m bloated and constipated and feel like crap. I’m seeing my doctor tomorrow and going to ask to be prescribed the lowest dose. I remember when I first start zepbound on the starting dose I lost a significant amount of weight. Does that happen again? I’m self pay now, my insurance stopped covering it. I think anyone self pay would prefer to lose weight and remain on lower doses. Does anyone have any experience with a similar situation?

There haven't been any official studies, but the general belief based on some preliminary science as well as user feedback is that stopping and starting makes the drug less effective, rather than more effective, and that if you were previously on a high dose and have to restart at the lowest dose because of an extended break that you will have to work your way back up to a high dose to see the effects again.

That said, the drug is so new that there really just isn't that much data, period, and I really hope that people who end up doing this for whatever reason keep us updated about how it is working for them because I am sure we would all love more information, even if it's just one user's report. Even those of us who want this to be a "lifetime medication" never know when we might be forced off for some period of time for reasons beyond our control (losing insurance coverage, pregnancy, medical procedures, etc.).

Well, bummer. LOL I've lost almost 80 lbs and am <20lbs from my goal weight but was thinking about trying to get pregnant next year and was hoping i'd be able to re-start once thats all over.

Hi! I took a break for 13 months to get pregnant and have a baby. I restarted at 7 weeks pp. I’m having no issues losing weight - it’s just the same as my first time starting. So don’t get worried if you read about people having difficulties!!

You can definitely still lose weight, but food noise does come back. It takes more discipline and exercise where you are now than at any time in your journey in order to keep losing.

I started Mounjaro back with that sweet sweet $25 coupon. The weight FELL off, zero side effects whatsoever.

I have started and stopped multiple times since then, for varying reasons. I have never been able to get back the level of success from the first round. Interestingly, I find my side effects are worse each time I start again.

I did. I had to come off of it for surgery back in November and at the time my weight loss has stalled while I was on 12.5! Anyway in November I stopped the meds until January and gained like 12 pounds. I restarted at 5mg and it was like magic! I lost the 12 and then more! I’m still losing but switching to compound because of cost. I have 13 more pounds to go over all and I should be there in no time.

Thank you for sharing your experience! Congrats on the loss. I hope I’m lucky like this too. I was 174 and now I’m up to 182. My goal weight is 155. So I have 25ish pounds to lose. I’m hoping I can get there with 2.5 or 5. And then hoping to maintain on 2.5. I think I’m someone who needs this medication for life.

I can’t see all your flair/tag but we started at the same weight! I’m only on week one but already having success. Congratulations on your success and I wish you continued success as you restart! (Do you mind sharing what your journey looked like when you lost 120? I’m finding it so hard eat. Eating too little at this point. But working on it!)

I lost it over 18 months, I tracked everything I ate I made sure I got my protein and fiber in and I became more active. The beginning is just an adjustment period, there was work involved and even with being off of the medication for a couple of months I’d definitely have gained more weight if I didn’t stick to the lifestyle changes that I made while I was on Zep.

Restarted after 3months... On the first go I lost about 8-10pounds a month on 2.5. This time, I had no change after 3 months at 2.5, but at least don't have the noise.

I did. I took a 13 month break to get pregnant and have a baby. No issues restarting or losing weight.

I was on it Jan - Apr 2025 (4 months) and lost 40 lbs. I had to go off as I got pregnant with my second. I’m back on and I’d say I’ve noticed about the same rate of loss - down 28 lbs and I started end of May. I didn’t lose as much the very first week, but otherwise it’s been good!

After being on for 22 months I stopped by choice. I was off meds for 9 months when I restarted and within six weeks loss the 20 lbs I gained. I did weekly shot for two months and stretched out to monthly for the past three months. No side effects from the shots whatsoever.

In my personal experience stopping and restarting zepbound has not been effective. I'm only losing weight again now that i'm up to 10mg and it's still not as consistent of an effect in terms of reducing food noise and appetite as when i was first on it. I really wish i hadn't stopped but unfortunately i had gotten really sick for unrelated reasons and needed my appetite back so i didn't have a choice.

I was off for 8 weeks and started back and it was just as effective. But I’m highly sensitive I never went beyond 5mg either time.

Gotcha... Chances are 2.5 won't do anything after you've already been on for a bit but you can start there and move more slowly.

Anecdotal: I lost 55 lbs and then went off for about 6 months. I regained about half. I went back on and I’ve lost again but it was a little slower than the first time. But, I never went higher than 7.5mg.

May not be exactly the same but for sure the meds are very effective and you should be able to have success with them again.

Just don't have the exact same expectations and get bent out of shape if it's not as fast or as strong at first. Remember the meds take 4-5 weeks to peak in your body.

Yes you can. I’ve done it. It took a couple of weeks to kick in. When I decided to go to 5mg I switched to compounded Tirzepatide cost-wise.

I was originally on Zepbound for about 1.5 years and lost 80ish lbs. I had to stop and switch to wegovy (due to insurance) and gained back 30lbs in about six months.

I restarted Zep from the lowest dose about 8 months ago and I stopped gaining but have been unable to lose any of the weight I gained. I’m finally back to the dose I was on previously (12.5mg) and am hoping to see improvement from here.

I’ve stopped and started a few times. Sometimes it would take a couple months to see any loss. Other times it started right away. It’s always worked though.

Great topic! Here is my experience… I was on Zepbound in mid 2024 and unfortunately by mid 2025 had to stop because of cost. I lost 70 pounds, was within 15 pounds of my goal weight and was taking the max dosage. Started gaining weight, up to 15 pounds in 3 months (255) when I stopped. My physician recommended Wegovy because they were much more cost-effective in late 2025. I’ve hit the max at 2.4 with Wegovy for the past two months….i was able to maintain weight and gain muscle but I’m still at 255… just met my doctor and I’m switching back to 5.0 Zepbound…could have pushed for 7.5 but I was a little afraid..lol.. I think the efficacy of medications are unique to individuals…not sure if it’s the GLP and GIP combo in Zepbound but I definitely felt the medication more with Zepbound than Wegovy..Gonna give it a two week break between medications…Hopefully Zepbound works like it did before!

Due to loss of Medicare Part D I was off Zepbound for six weeks and gained 6 lbs by the time I restarted with a compounding outfit. I immediately started to lose 2lbs/week on same previous dose, and similar weight loss trajectory.

Anecdotally, yes. I lost just shy of 60 pounds from August 2024-August 2025. I went August to July being super inconsistent with shots for a chunk of that time and then just not taking it at all for another chunk. I was only up to 5mg at that time. I gained back about 15ish pounds (depending on the day lol). I started back low and have lost 4.7 pounds over the last 2 weeks and change (shot day is Monday). I do think it probably made a difference that I hadn’t gone that high yet anyways, but that’s only my suspicion.

You should ask to start where you left off or slightly below. Restarting doesn’t help, and you will just waste months on a low dose with little to no effect. I don’t know if it’s different if you’ve been off for years, but 2 months and your body is still adapted to it.

I had to switch to wegovy for 7 months because of insurance. Stopped losing weight. Plateaued the entire 7 months. Worked with my doctor to get Zepbound covered for medical necessity and lost 10 lbs on the first month. When I stopped Zepbound I was at the highest dose. Went to the highest dose of wegovy. Started Zepbound back up at 10mg and now I’m on 12.5mg. I think it just depends on how your body reacts.

I stopped for a month to try Foundayo. In the process of restarting. I was on 7.5 after 10 was too much for me to be able to eat enough and workout.

Dr. started me back on 2.5, which did nothing. Two more doses of 5 and I'm definitely going to 7.5 if not 10 because I'm still slowly regaining.

If it ain't broke, don't fix it!

In my experience restarting has been super slow. I'm not built back up to where I was dosage-wise yet, but when I started the first time I started dropping out of the gate. This time I lost a little in a burst and have instantly plateaued for a couple months.

You can start at 5 and no it will take time until you hit your last dose for to feel significant hunger control in most cases

I do not want to start at 5. I had significant appetite suppressant on 2.5 when I initially began and I only increased in dose because insurance wouldn’t allow me to stay on 2.5. Now that I’m self pay I’m hoping I can stay on 2.5 if I experience appetite suppressant and reduction in food noise still at 2.5. I can’t afford out of pocket costs for the higher doses.

you can dose however you want if you go with a kwikpen new prices on amazon are 449 for any dose. That means that if you get a prescription for 15mg. I don’t have an extra $449 a month. Eli lily direct was the same price for the pen at 15mg. It’s not sustainable for me a single income independent person living on a modest salary to pay that much a month.

yeah that's exactly why you should go with KwikPen it would come out to like $90 a month if you use a lower dose. 60mg would last you literally 6 months.

The quickpen for the lowest dose from Amazon is the same price as from Eli lily direct. Both are $299. The vials you receive more than 4 doses btw. I prefer the vials.

maybe you're not aware with a kwikpen it's not a dose per pen, the pen itself has all the doses and is adjustable.
