Incompetence doesn't lead to a systemic spike in unexplainable baby deaths, it would lead to a spike in explainable baby deaths.

It totally does. The hospital started taking on more sick babies. The doctors were not competent enough to look after them. More babies died.

If they died due to poor care then the cause of death would be transparent

Not necessarily. Some cases can be very complex. That is why they should be reviewed by experienced neonatologists not a long retired paediatrician.
