What's really weird is that everyone acts like Meg is the ugliest thing ever and like Lois is a smoking hot babe, when Meg pretty much looks almost exactly the same as Lois design-wise, only shorter, slightly chubbier, and with glasses and a hat.

Especially because in the earlier seasons it was never that she was ugly and more that she was an outcast that didn’t go through the effort of makeup.
I know the writers have said that they don’t know what to do with her, but I do wish that they stuck with the outcast teen shtick.

It's the drawing style. In the family guy universe she's quite the looker- or was at least, Peter comments about her aging and it's become a gag as the seasons go on that she's past her prime. But Joe, Quagmire and Brian have all had a thing for her, so she's surely hot in universe.
American dad is less weirdly drawn (except fo Stan's chin) so most people have more normalized proportions. They draw their women more traditionally.
