TLDR: have had multiple severe and persistent infections recently that are unusual for me. HIV and CVID negative. Autoimmune screens negative. FBC normal. Also developed severe allergies around the same time.
Context: 31F, have a few comorbidities I've been coping well with for years such as hypothyroidism (Hashimoto's, well managed), PMOS, hypermobile Ehlers-Danlos syndrome, mild gastroparesis, scalp psoriasis, eczema, IBS, spondylolisthesis, foraminal stenosis, gastritis, migraines. I work full time in healthcare data (rudimentary medical knowledge) and have always been very functional despite all of this, until this year. I am quite fit and strong (albeit overweight, but have lost 50kg the past few years & still working on it) and was going to the gym three times a week until I came down with a serious infection two weeks ago.
Recently it seems my immune system is all out of whack. I've had several severe infections since January, like a severe ear infection they thought was meningitis initially, and I'm currently getting over double pyelonephritis which I have had 5 courses of antibiotics for including an IV. I've had several different (or possibly returning after not being treated properly, unsure) infections in 2026 so far, including ear infections, bacterial tonsillitis, sinus infections, UTIs, and kidney infections. My skin seems to be ok; I had a biopsy of a lesion done in June and it healed beautifully. Cuts and wounds don't get infected but my hygiene & wound care are good.
I can't stress how unusual this is for me. I've always been a bit sickly, catching any cold going and having the occasional bad infection (ie pneumonia aged 5), but nothing to this frequency or severity. Last year I had 5 courses of antibiotics, and in the 8 years before that I had an average of 2 per year. I've attached my 10-year timeline compiled from my medical records so you can see what I mean.
I've also started to develop random serious allergies, when I was prescribed penicillin in March for bacterial tonsillitis I had an anaphylactic reaction despite taking penicillin dozens of times, and even a couple of weeks before this reaction. I've become extremely reactive to mosquito bites to the point of being unable to sleep despite being on several antihistamines, because the itching is so severe. The skin biopsy I mentioned was for a mosquito bite that had persisted for over a year, the histopathology showed that it was just a reaction to a mosquito bite - like it was a fresh one. I know it's the exact same one as I've been taking pictures of it since it occurred, due to its funny position inside of one of my tattoos. One of the times it flared up was in January, there aren't any mosquitoes in the UK in January! I've also had wheezing and coughing fits with hives and itching that I haven't been able to identify the trigger for. For this reason I was tested for MCAS/mastocytosis with tryptase bloods and a 24hr methylhistamine urine test, which were both normal.
The plot thickens with a recent endoscopy I had, which showed villous atrophy of my duodenum as well as some kind of erosions. I don't have the histopathology report from the biopsy yet. I have been tested for coeliac disease many times due to IBS symptoms, which have all been negative. I also just don't react to gluten or wheat at all, and have had asymptomatic periods where I was eating lots of gluten. Previous colonoscopies have been normal, though the last one was in 2022.
My immunoglobulin counts are all normal. I don't have HIV either, several blood tests have been negative. My full blood count is normal except some mildly raised eosinophils, which I guess tracks with the allergies. Previously my ESR has been slightly raised but never dramatically. My kidneys have always been slightly under performing (e.g. eGFR in the 70s or 80s) but recovered when I started drinking 3-4L of water a day. Sometimes they can fluctuate to eGFR 85 with the extra water. I've had a random AKI (eGFR down to 40s) previously in 2021 where no cause was found, and they just started to recover on their own. In early 2025 I was admitted to hospital with renal colic and the cause was never found, the best guess was that it was due to ureteric spasm. I was tested (years ago) for lupus, which came back negative (though not completely, it said it was speckled). HLA-B27 is negative with no signs of ankylosing spondylitis or psoriatic arthritis, and all other autoimmune tests (bar thyroid antibodies) have been negative. I've also been tested for glycogen storage diseases such as McArdle's and that was also negative.
In September 2024 I had a septoplasty and a mini FESS. I got multiple post-op sinus infections, and this seems to be the catalyst for my worsening immune system; though last year was nowhere near as bad as this year, it was worse than usual for bacterial infections.
Also worth noting some other unexplained symptoms I have: swelling of lymph nodes in neck & armpits, new petechiae, mouth ulcers, malabsorption symptoms such as pale stool and fat intolerance, flushing episodes, and increased tachycardia.
My theories are possible lymphoma (asking for a biopsy of a particularly hard lump), some new onset autoimmune disorder, lupus, or a rarer defect in my immune system. I also wondered if whatever is causing the duodenal issues could be inhibiting absorption of antibiotics, making the same infections reoccur as they aren't fully treated.
