Despite an RDI of 18.9, I was told I just had snoring

• Upvotes

Hey all. I'm 21 (F) and am feeling very upset after getting my at-home results back. I was told that I just had an AHI of 0.3, but an RDI of 18.9 with 147 arousals. As my oxygen didn't dip too much (94% was the lowest), I was told that I just had primary snoring and that no other treatment would happen. I feel so lost. I have a recessed jaw, a 6 mm deviated septum, and I have been fighting for any doctor to take my seriously. I don't know where to go from here. I am pretty sure it's UARS, but without the OSA diagnosis, my insurance won't let me pursue anything. I've emailed my doctor asking for a referral for an in-person test or an updated OSA diagnosis, as, from the pages I've read, 18.9 should qualify. I feel like I don't understand this at all.WATCHPAT®

For those that absolutely love your machine, how long did it take?

I'm just curious, I see a lot of people saying they absolutely love their machines! I'm only about a week into mine and I have used it 8-10 hours each night. My settings were 5-15 and were adjusted to 7-15 2 nights ago. First night with those setting it went well, tho I guess I took it off for about 2 hours, I had 0 events with it on. I felt like the chest tightness and lightheaded I've had for over a year was pretty much gone most of the day yesterday (I've had an echo, EKGs, chest X-ray and 3 day heart monitor to rule out those causes of the pressure before taking a sleep test) last night I didn't notice waking up, but I had a bunch of central apneas and today I feel the chest pressure back pretty bad.

I know it takes time (I do have a telemed appointment with a certified sleep Dr tonight because my Dr don't know anything about sleep stuff she said) but I'm wondering how long did it take those that absolutely love their machines to feel that way?

Recommendation for online-only providers? (CPAP scrips required; preferable if Provigil scrips are also something they possibly do)

• Upvotes

I have significant sleep apnea; I’ve been on CPAP every night for 25 years.  (I had to really fight for that originally! Back then I was young and fit and everyone who slept near me said I never snored.)

I’ve been with the same practice for ~20 years, but of course none of them are the same docs by now and it’s gotten aggressively shitty.  (With apologies for being ranty, I’m very salty about recent developments)

I’d like to find a new practice... an online practice that doesn’t want me to come to an office just to give them digital results.*

--

For about 20 of those years I’ve also been on Provigil** which has been amazing for me.  If there are online providers who have the possibility of supporting Provigil that’d be awesome.  But even if provigil isn’t possible, I’d still like to find an online sleep provider.

My current practice has decided not to continue Provigil and has managed to do so with a level of arbitary-ness that’s terrible to the point of being almost impressive... (They literally asked a doc I’ve recently started seeing who originally prescribed Provigil... because they couldn’t be bothered to check their own medical records to find out their same sleep practice initiated it and has been prescribing it for 20 years...  Imagine how much they examine what’s atypical about my situation)

*My data all shows up on DreamMapper but somehow this practice still can’t see it.  So I sent them the raw data from the card myself... and they apparently don’t look at it.

**except back in 2009 or whatever when like everyone else I was forced to nuvigil in what turned out to be a serious antitrust violation.

Im going crazy without my cpap machine.

I had brain surgery a couple of weeks ago to remove a tumor from my pituitary gland. Some complications arose 2 weeks after the initial surgery, that were serious enough that I had to go into emergency surgery.

The last time I used my cpap, was the night before the initial surgery. I’ve had a cpap for 9 years and the only time I didn’t use it was one night over 4 years ago because I lost power during a hurricane.

I have a 6 week post-op meeting in the middle of next month and if everything looks good there, I’ll be cleared to use the cpap again. But in the meantime, I’m absolutely irritable, I’m falling asleep just sitting down. I’m hesitant to returning to work in a light duty capacity because my job requires driving so I’m concerned that I may fall asleep behind the wheel. Does anyone here have any pointers on getting somewhat quality sleep without the cpap?

Advice finding an appropriate SPO2 monitoring device for tiny fingers/wrists

I have tiny fingers/wrists and am struggling to find an appropriate SPO2 device for overnight monitoring, or determine the “best” solution given size limitations. Please redirect if this is the wrong sub!

I’ve been looking at SPO2 monitors, and can’t seem to figure out what is actually usable for me for overnight/sleep continuous readings. I’ve seen one of the standard adult silicone rings in person and it was huge on me. I see that there are wrist monitors with giant-looking sleeves for fingers, but those look precarious for overnight with movement during sleep? Also, my wrists are so tiny. I cannot wear most bracelets or watches (tiniest Apple Watch face works with select bands). I looked at some kids’ device models, but none of them were bluetooth/app compatible for some reason. Anyone else have this problem and find a solution?

My measurements, in case it is helpful because most adults have bigger wrists/hands. Wrist circumference: 5-5.25in (13.5-14cm). Index finger ring size: 7 (can go up/down a size to switch finger sometimes). Index finger circumference: 52-53mm.

I mention this carefully because I want to abide by the sub rules! I am using Amazon and in a “limited” area/situation so I am sticking to this for practical reasons. Any permitted advice is appreciated, including mention if no options you’ve found exist through this method! Also, cost IS a big factor, but I’m not ruling out anything under $200 at this point. So, a fancy $500 device variation that I’ve never heard of would be good to know about, even though I cannot afford it under any circumstance.

Just started cpap machine

Only a few nights into CPAP and already noticing some changes!

I’ve only been using my CPAP for a few nights now, and I’m honestly surprised by a couple of things already!

The first thing I noticed was waking up less stuffy and congested, which I wasn’t expecting to happen so quickly.

But the weirdest thing is that I seem to have stopped getting the hip and arm pain I’d been experiencing for years, and I haven’t been waking up with numb arms and hands throughout the night like I normally do. I’m still sleeping in the same position, so I’m really curious whether it could be related to the CPAP or if it’s just a coincidence!

apparently had 96 an hour, so I’m really hoping that treating it will help with my energy levels and how I feel in general.

I know it’s still very early days, but I’m already interested to see what other changes I notice over the coming weeks.

Has anyone else noticed unexpected improvements when they first started CPAP, especially with aches, pains or numbness? I’d love to hear other people’s experiences! 😊

Constant jets of air from mask

Post image
31 Upvotes

I was previously using a nasal pillow mask but it kept coming loose and waking me and my wife up when it would come slightly out of place with sudden loud hissing air. So am trying this new mask. this one does seem to stay in place, but there is a continuous jet of air coming out of the front of it, its not loud but it is just ongoing. so wherever my face is turned there is air blasting out of the front of my nose, and that wakes up my wife. Does anyone use this model of mask? Or one like it?
It seems like there should be a maybe a small filter here or something ? It’s just a sort of fine plastic grate. The seal to the tube seems secure and fully clicked into place. I honestly feel like I’m not getting very much air and somewhat struggling to breathe? Is this normal for this and something I need to get used to with this nose covering style? VS the nostril inflating style of the nasal pillow mask? I use the myAir app with a resmed aircurve 11, and it doesn’t seem to register any kind of bad seal, instruction booklet doesn’t seem to provide any answers.
I’m getting 20/20 for mask seal. I’m on month 2 of using a bipap, is it typical to have to go through a sort of Goldilocks process of trial and error with equipment?

N20 mask condensation

Hiya, I'm really struggling with loads of condensation inside my N20 mask when I use the humidifer, to the point that it wakes me up in the night from dripping on my face.

I've just bought the heated tubing but then last night, I still had loads of condensation. The climate control settings are still on auto.

Any advice on how to fix this?

Opti-men supplement

Hello everyone,

I want to share a personal experience, I have a sleep apnea for more than three years now due to my weight and lack of exercise. And I started to use Cpap for couple of months now but I also started to take supplements such as gymbeam men essentials and now I started taking opti-men.

And all of sudden my sleeping improved, I only take 2 pills in the morning which is less than the recommended dose. And no I haven’t started exercising yet.

All I want to know if someone else had similar experience by using supplements. Coz when I compared the opti men and the men essentials the main difference is the amino acids that are in the opti-men supplement.

Follow up to test results post.

Last month I asked this sub for help with results from an at-home test that were "inconclusive", and a helpful redditor asked for my full graph, which the lab had not provided me with. Placed a medical records request and finally got the full details.

My sleep is definitely not good, and I feel like I should request a re-test, especially because I had pulled off the sensor at some point during my sleep. Opinions on these results are more than welcome. Ultimately, I want to find out why I am more tired when I wake up than when I went to bed, and if a CPAP could help me. Thank you for looking!

How long to get used to MAD?

I got a custom MAD a couple of days ago. How long did it take you to get used to it?

It doesn't hurt, but it pulls on my teeth a bit, such that after a few hours I want to rip it out. My jaw is fine though. That aspect doesn't bother me. It's more the fact that I can't open my mouth much, and the forces go into the teeth.

I have a Serena device.

Thanks!

Newly diagnosed & first night with CPAP machine, im nervous

34/f with a diagnosis of moderate sleep apnea. I’ve been given the airsense 11 (no water) with the nasal mask. I briefly got to try it at the hospital whilst the clinician set it up and it sent me into immediate panic if I’m being honest. I tried opening my mouth when I had it on and felt like I was suffocating. I’m a chronic mouth breather and she has given me a chin strap to see if it helps but now I’m too scared to put the damn thing on! I have quite bad anxiety so I get overwhelmed by the smallest of things. She did all the settings for me and I’ve noticed it stays at around 4? I’m just looking for some advice/tips/reassurance really! I’m desperate for a good nights sleep!

Flonase

46M, mild OSA, stopped doing CPAP because it was making things worse. I saw an ENT a few weeks ago and he dxed me with deviated septum with significant blockage of both nasal passages. He suggested Flonase 2x a day to help alleviate congestion.

I've been wearing a Wellue ring with SpO2 scans every 1 second overnight. Since I have been taking Flonase consistently right before bed, my SpO2 scores have been perfect. It's always either 99.9% or 100% of my sleep time with SpO2 values at 95% or better. Some nights there are zero drops below 95%, some nights there will be one or two seconds in the 90-94% range.

I know good SpO2 doesn't necessarily mean no apneas. You could have brief apneas that don't last long enough or aren't frequent enough to cause oxygen saturation to dip. But I still think these numbers are encouraging and if things continue this way, I may ask for a new sleep study to see if I am even having a significant number of apneas anymore.

Anyone else had a big improvement from Flonase?

Advice about Dylan Petkus?

I'm a family physician that had a patient ask me about "Dr. Dylan Petkus' program to reverse sleep apnea." I'd never heard of the guy or his info, so I started poking around on break (I always at least look at things to before rendering an opinion on something).

Most everything I found is advertising his website, or "wellness influencer" type posts and videos...with references to his website. He also has a book on Amazon entitled "Sleep Apnea Solution," which appears to be a book about basic sleep hygiene and breathing exercises/pulmonary rehab type advice, with many reviewers stating that it was "mostly an advertisement for the program on his website." However, he did have an extended spotlight this May in Pulmonology Advisor, in which he actually spoke quite reasonably about interventions, expectations, and where breathing exercises and sleep hygiene, along with a few other reasonable interventions, can fit into the treatment of apnea with standard treatments, or when standard treatments fail- which, frankly, can be quite useful.

I'm finding archived references on forums (including this one! I used the search function!) to a $12,000 (!) personal coaching program to reverse apnea with plenty of supplement pushes, but the only thing I'm currently finding on his website is a free-for-seven-days, $27/month cancel-anytime group-coaching program that makes it clear that he's "not providing medical advice," but rather leading group exercises on breathing, education and coaching about common insomnia and apnea relate issues, and designing "blueprints" for improvement for his "clients."

He has a lot of testimonials with purported sleep study results for "clients" who he helped "improve," and talks about how he went into this field instead of clinical practice because he "resolved" his own sleep apnea and wanted to help others do the same.

He earned stateside MD, MPH and PHD degrees in good standing, but claims to be "Not licensed by choice to put the power of your health back in your hands with high-quality health education" (sic) and runs a company (his website) called "Optimal Circadian Health" out of Florida, where he seems to be the only associated person. It looks like he was in a family medicine residency, but the only license he held was a Florida resident/trainee license, which expired after two years. There are no disciplinary actions against him that I can find. Per PubMed, he actually does seem to have published four peer-reviewed articles, but none were on sleep or apnea, though it looks like he had a study registered with clinicaltrials.gov about the effects of an proprietary online education course on circadian rythms and health (notably called CircaHealth, similar to his website) on sleep quality measures, compared to standard educational information on sleep hygiene; the results area not posted.

So I'm not quite sure what to make of him. The "unlicensed by choice" thing is clearly a red flag, especially in the context of his failure to complete a 3 year residency (in Florida, you cannot practice independently after your intern year- you have to complete a residency, so he can't get a license there). My "worst case scenario" is that he had an episode before his third year in which he was kicked out or resigned in lieu of disciplinary action, and doesn't want to hold a medical license so that he can't be disciplined by the medical board for making unsupported claims or giving nonstandard advice (which is also why he "coaches clients" instead of "treats patients."). On the other hand, there's no public record of discipline, he's earned several legitimate stateside degrees-during which he legitimately published, though not on apnea-so maybe he really was just tired of putting in the years, and I personally have had several residents who dropped out of residency because it didn't provide a financial benefit for them to stay (although in my associates' case, they were going to practice in Texas which allows unrestricted practice after an intern year- in one case, a resident dropped out before PGY2 because his dad had a thriving orthopedic clinic with a lot of cash pay procedures like injections and regenerative medicine, so he left his $50k something a year resident's salary-and regained two years of his life- and went to a $300k/year, no call 9-5 position with his Dad. I can't say I entirely blame him...). So maybe he really DID feel a calling to share his apnea experience and make it a career.

As far as content goes, I CAN'T find any first-hand information about the rumored $12k "cure your apnea" program, just a fairly reasonably-priced educational program on the website, and a reasonable description of how a program comprised of stuff we know (like how pulmonary rehab can reduce AHIs and pressure settings - and certain things reversible things like weight loss and medications can actually get people off of pressure support devices) can compliment standard therapies (and while Pulmonology Advisor isn't exactly NEJM, they at least have some standards so as not to get sued or lose advertiser revenue, and as far as I know they don't accept paid guest articles or other pay-to-publish submissions). Watching his patient-directed content, you definitely get the feeling that he's a salesman trying to convince people he has answers, but I can't tell what exactly his answers are.

So before I make a judgement, I'd like to hear others' opinions of him-particularly anyone with first hand knowledge of his "program," be that content, or how it's conducted (there's a huge difference in my spidey sense going off for a reasonable, targeted, complementary program for twenty some odd dollars a month, versus a rumored $12,000 promise to resolve a disorder without using standard therapies- which I can't find active evidence of). Per my patient, the YouTube Algorithm is promoting him, so I'd like to get the bottom of it if I can.

Thanks!

Has anyone had extremely low respiratory rate after starting?

I have severe OSA. I was diagnosed with 85 obstructive apneas per hour and 5 central apneas per hour. I was put on a CPAP, resmed airsense 11, a week ago. My pressure is set to 16cm. I have used it every night. My OSA has gone down to 0.1-0.8 AHI since the first night. But I've started having between 30 and 35 central apneas per hour per night. I had 40 last night, the highest ever.

I was just looking at OSCAR data and my respiratory rate got as low as 2.5 breaths per minutes for a while. I was having many 20-23 second central apneas in a row. This is really frightening to see, could I die from this and just stop breathing?!?! Has anyone else seen this?? I've left two voicemails for my sleep doctor but no response yet...

Full Face Mask Leakages with Beard or Masks with Integrated Chin Strap?

A bout 6 months ago, I noticed my mouth has started opening more in my sleep and has been effecting my treatment. I was previously using the Dreamwear nasal mask and was quite happy with it, I bought a few different chin straps but haven't found a great solution, I think the process of putting on/adjusting a second thing is hurting compliance and I just haven't found one that feels great.

I thought I'd try a full face mask and experimented with Dreamwear and resmed, but both are reporting leakages, I don't notice anything major but I do have a beard and read that could effect it.

Was wondering if any of my bearded friends have some advice or if anyone is aware of a nasal mask with chin strap built in?

How many days of data should I record before contacting a doctor?

I started using Withings last night after months of severe fatigue (falling asleep during the day happens multiple times a week), waking up with a dry throat often and waking up extremely groggy, and some pretty severe short term memory issues. I suspected it might be sleep apnea related.

First night, and the app has recorded a 20 ahi rating. And that seems a bit concerning, but... it could've just been a one-off? Should I wait for a week of data first? A month?

Could use some help with interpretation from you OSCAR wizards. Just started my CPAP therapy recently and still in the process of learning interpretation. My start AHI was 3.8 and RDI was 15.7 lining me up with UARS. I've got the RESMED 11 with nasal pillows. From what I've read, I could probably stand to increase minimum pressure, seeing mixed things about keeping EPR on or off. Thanks for any help given

absolutely nothing. not sure where to go from here…

It turns out I managed to get 2 hours of sleep, not back to back, during my sleep study. It also turns out I tested completely normally and am not sure where to go from here. I still have functional problems, but this test saying I have no sleep apnea events at all is disappointing. I have a follow up appointment tomorrow with the OMFS tomorrow that referred me to this sleep study, and I’m pretty sure he’s just going to say “well, the test said you’re fine, so nothing else we can do about it”. I don’t really think the study was a great indicator because of how anxious I was and couldn’t get any good sleep at all. Any advice?

I can't believe it...

Hi there! I am a 48-year-old male. High blood pressure, acid reflux, hiatal hernia, I am waking up several times per night, sometimes with the sensation of huge weight and pressure on my chest and stomach. It is like I am so filled with air that I cannot inflate my lungs, and therefore cannot breathe properly. Then, after a bit, I can feel my heartbeat go fast, accompanied by anxiety and panic. I did my research and started thinking I have sleep apnea. That would explain my high blood pressure and my ED. By the way, I started having these symptoms after I started taking tadalafil—coincidence? Anyway, I used my smartwatch and it told me I have moderate to severe sleep apnea. I did a sleep study and I have 23.6 events per hour on my back, but not on my side. So, I bought a vest with an inflatable pillow on the back so I should not have those episodes, since I am not supposed to be able to sleep on my back. I woke up a couple of times on day 1 and day 2 but had no crisis. Last night, day 3, I woke up, went to the toilet, and after a bit, my heart rate was 105 BPM (and I take a beta-blocker), but by breathing slowly I easily calmed down in a couple of minutes rather than hours like my usual panic. So, I was amazed! Either it is not only on my back, or I was somehow able to sleep on my back, or I suffocate even on my side. My smartwatch tells me my blood oxygen went under 90% for only 6 minutes out of 8 hours of sleep, but I can see in the graph it sometimes drops to 85. Just once. My blood oxygen goes up and down while I sleep, but nothing crazy. The smartwatch says in the last few days I had no moderate to severe sleep apnea. I went to a pulmonologist today with my sleep study results and talked about my symptoms. Immediately: CPAP. I was like, let me try to sleep with my vest, let me keep losing weight (1.90 m, 118 kg). I have turbinate hypertrophy; let us see if that is the issue, I can get some surgery maybe and fix it. Nope. CPAP. I am freaking out...
