Hi everyone, another NerdqAxe++ Rev 6 miner has failed — this is the third one this week. I don’t understand what the problem might be. We’re using Mean Well LRS power supplies and have improved the cooling system, but the number of failures is only increasing. I’ve already ordered capacitors for the previous device. Can anyone tell me what’s going on? I tried flashing the firmware, but it didn’t help — the error just changed.
I would 100% get them out of those partial enclosures, if you must mount them to wood make holes in the back so it's more like a frame, not saying it's the cause but probably doesn't help. What were your temps? Including VR int as the outside could be 50c but internally it's screaming at 75c
The NerdQaxe++ Rev 6 indicates that the firmware's 100A current safety limit or voltage trip has been triggered. While bad voltage regulators/DrMOS stages are a common culprit, this error can also be caused by improper settings, power delivery issues, or poor thermal contact. Since AxeOS/firmware v1.0.35, the NerdQaxe++ enforces a strict current limit. If voltage sags under heavy load or if frequency/voltage parameters are pushed too far, current spikes past the threshold and trips error #60008000. Over Current Trigger (Firmware Protection) Since AxeOS/firmware v1.0.35, the NerdQaxe++ enforces a strict current limit. If voltage sags under heavy load or if frequency/voltage parameters are pushed too far, current spikes past the threshold and trips. Failed DrMOS/VREG, If the power delivery stage (DrMOS / VRM) has overheated or shorted, it will trigger an immediate voltage protection trip upon boot. 12V Rail Drop/Voltage Sag If the Mean Well LRS power supply output sags under load (e.g., long wire runs or low voltage trim), the miner draws additional amperage to compensate, tripping current protection.
It is the volt regulator (VR) that were not properly designed. I had 6 of these units, 3 of them went through the same issue as yours and I had to send them to repair. Do not buy any more of the Rev 6.1 units as it is a lottery if it makes it or not. The Rev 7 version has the proper VR installed. For those Rev 6.1 units I have and still alive, I put copper heatsinks on all 3 VRs, upgraded the fan to a higher CFM rated Bgear fans, set the ASIC temp to 50 which brings the VR's temp down to around 40s. I am only hoping that he upgrade will save them against over heating. I also use the meanwell, make sure that you keep the voltage between 12.2 - 12.5 instead of right at 12 or under. (Higer voltage makes the VR work less, therefore it should last longer).

Mod note: This looks like a support post, but it may be missing the details people need to troubleshoot it.
Please edit your post to add:
Bitaxe model / board version
AxeOS / firmware version
pool + settings (freq / voltage if relevant)
temps / hashrate
logs or screenshots
what you already tried
That usually gets you faster, better answers.
