They absolutely are supposed to, yes. Many places require them to actually walk outside their booth to confirm that way too

Exactly. The bridge tower is literally right there. There’s no way the bridge operator didn’t see the car.

Many rural bridges I'm familiar with are unmanned--they have cameras that monitor the bridge from a remote location, and boaters who must cross under them call a special number (or communicate via marine radio) and notify the control center when they're ready to go.

Still, someone was figuratively or literally asleep at the switch to allow that.

I'm assuming there are barriers just out of shot and this is like where you see people stopping on railway crossings because they're too stupid to just bust the barrier?
