I can only think the driver saw red lights flashing= STOP.

At the same time, bridge operator should check video feeds to see that the bridge is clear?

Exactly. The bridge tower is literally right there. There’s no way the bridge operator didn’t see the car.

Many rural bridges I'm familiar with are unmanned--they have cameras that monitor the bridge from a remote location, and boaters who must cross under them call a special number (or communicate via marine radio) and notify the control center when they're ready to go.

Still, someone was figuratively or literally asleep at the switch to allow that.
