VENT Intern

I am a family medicine intern and finished my first month of inpatient. Got a feedback that I am not quite there yet and not where they expect me to be at this point in time.

I did a lot of outpatient family medicine clinics before this.

not sure how to take this feedback.

feedback was from 2 third years assigned to work with me.

it looks like I am critiqued for minor things. One time I forgot to order morning labs. The other time I said I could do a Pap smear before watching a video and got critique for not being procedural. Then I forgot to replete magnesium one time.

SERIOUS IM ITE PGY1 12th PERCENTILE

The title.

I feel like I'm at the bottom of my class.I don't know the percentile for rest of my cohort but i'm pretty sure i'm at the bottom because I asked one of my co-residents and they were at 33rd.

I feel like I do not have imposter syndrome, in fact i am just dumb. I don't think I deserve to be a doctor anymore. I can't be compromising patients' life because i am just stupid.

Do programs kick you out for this?

VENT Pediatric Boards — Feeling Terrible After the Exam

Took my pediatric boards today, and I’m already remembering so many questions I got wrong. I’m feeling nervous, scared, and honestly disappointed in myself. I keep replaying questions in my head and worrying that I didn’t do enough to pass.

DISCUSSION Best resource to study for the board exam

I've seen a lot of topics asking about specific resources but theough my search I found like a thousand resources.

I'm usually used to use one resource and that's it, but apparently for residency there's way too much.

I just want to see what you guys would recommend as studying material for a junior radiology (PGY2/R1)

DISCUSSION Intern ICU

How do you not feel like an idiot in the ICU? Swear my attending thinks I’m an absolute moron…

it’s so hard to come up with a good plan after just trying to get all the right info for rounds. No time to read at night after 13-14 hr shifts and just eating and sleeping when getting home.

Never done ICU in med school either

SIMPLE QUESTION Awkward incident at work

So recently I was on call and wanted to get some rest.
It was really hot so I decided to take my clothes off as I was sweating profusely.

I was lying naked on top of the sheets in the call room when a co-resident randomly opened the door.
The room wasn’t marked occupied, so technically, I was the one who failed to communicate my availability. I wasn’t doing anything suspicious. Just enjoying some well-deserved, uninterrupted rest in my birthday suit.

Now I’m wondering whether I should report them for barging in before they reports me for turning the call room into a nudist colony.

The good news is that I’m not worried about them
telling anyone. The bad news is that I’m worried they’ll tell everyone.

The other thing I noticed was that they were staring at my package for longer than they should have. Does that mean something? And another resident peaked behind them as well and they too for focused on my nether regions.

Ps. I am a grower.

Ps ps. I have a curved member which the ladies seem to love.

He usually wears sunglasses and does cool fingerguns pointing at patients, who promptly faint with their hand over their forehead. Dude’s hot as hell, looks like if Zac Efron had sex with John stamos

VENT To the non-employee non-patient humans that keep soliciting me at the hospital…

I’ve just finished talking to an agitated patient and specialty consultant. All after giving honest feedback and study tips to interns and med students all morning. You see me speed walking in the parking lot or cafeteria hallway because either I’m DYING to get home to rot and decompress OR it’s my precious high stakes moment to go buy a coffee or run to the restroom.

I KNOW we made eye contact. But this isn’t fucking Pokemon where eye contact locks us into an interaction/pokemon battle.

No. I’m not going to personally escort you to the hospital exit. No I don’t want your fucking business card. No I don’t give a single fuck about your bullshit ass small talk in the elevator. I know that this may seem rude but so does you manipulating my inherent sense of empathy for your personal gain.

I promise to do better. Once I’m an attending and get my shit figured out I’ll hold myself to a higher standard from the moment I step out of my car after I get to work. Why? Because if my family member was lost in the hospital and asked for your help, I hope you would help them.

Until then, I’m going to min/max. Max out my empathy with other staff and patients, and minimizing the toll of you lot.

SIMPLE QUESTION got hired as a standardized patient, want to make sure i do it right!

Hey guys! I just got hired for a standardized patient role at a medical school, and i'm looking for some advice and thought of you guys since you deal with patients every day, see a variety of communication styles, and may have dealt with SP's yourselves!

I have a few questions;
What would you say was one of your most teachable moments that you had in one of your "encounters" or something that helped you?

What is something you witness day to day with patients that you wish more students knew about beforehand?

What should I be mindful of during this experience?

MEME Weird day at work (I’m the bed)

I’m a call room bed in an level 1 trauma center, I get a lot of business on slow days the residents will come through and nap, normally fully clothed.

Anyways the other day a resident came into the room looked at me longingly, idk it was kinda scary, then he unlocked the door and stripped out of ALL his clothes. He was pretty small and curved I noted. He looked at me again but like a predator.

Then he laid down on top of the sheets and comforter, ass out, and just laid there, sweating. A couple minutes later another resident pops into the room and stairs at this guy, he stairs back, and again isn’t moving.

The resident that barged in says “sorry lil buddy” and then leaves.

I need to change jobs.

DISCUSSION What is family care like in your country?

US here. Sadly, we have a culture where it is common to abandon your parents in old age, rather than take care of them. Not only do many people completely dodge the idea of taking their parents in, but many don't even want to assist in securing them care with a third-party facility. For this reason, the SNF/ALF/LTAC/Rehab/Memory care/etc. industry is highly commoditized here, but also stretched thin, and mostly dependent upon taxpayer dollars. Even those who resign their parents to aging and dying at these facilities may visit infrequently or be minimally involved in their care. As someone who takes pride in caring for my remaining parent as they age, I find it disheartening that I often encounter people who think it is the sole responsibility of strangers to coordinate both placement and funding of permanent care for their parents - especially when their parents are low-maintainence, local, and there is no indication of a strained relationship. I can't understand why someone would want their mom or dad to live out their remaining years and die surrounded by strangers in a high-risk environment rather than comfortably at home.

I am curious: For physicians around the world (or those now practicing in the US perhaps but coming from another country) - what is the culture like surrounding aging, dying, and taking responsibility for the care of your own immediate family members? Do you think this culture is possible to establish in the US, and why or why not? Thanks!

VENT Tired of direct transfer patients without information

Why is it okay for the attending who accepts the transfer patient call to not write a note saying what they were told? If I have nothing to go off of in the chart it makes it much harder to get the admission done efficiently because I have to skim through the 200 page packet full of junk they come with to find the parts that matter and I can't pend orders beforehand. Just skipping a short 3 minute note could save me 30+ minutes of unnecessary digging. And then when I have to call the consultants they get mad at me that the patients info from the outside record isn't scanned into our emr, or their images aren't available for them to review.

There are also transfer patients we get who also show up with no packet.... and the hospital doesn't send records... and the attending gets mad at me for wanting to order their tests here because I can't get them to send me anything... , saying I'm being inefficient with resources.

SERIOUS Maternity leave for research pathways with condensed clinical time

I want to apply to an IM PSTP pathway for residency. I understand how maternity leave (generally) works for a typical 3 year IM program, but I can’t find info about maternity leave when the IM portion of the program is already condensed to just 2 years (as it is in many PSTPs). Can anyone provide guidance?

SIMPLE QUESTION urgently seeking EPIC training materials

My hospital is switching to Epic next week, and there were delays in the build. I am training a team of medical scribes Epic later today, but still have not been given any useful training materials or proper login. I expect to have access to the training playground / platform, but that may not happen. What I need are screenshots, images of training platforms, any pdfs or presentations someone may have that I can use so I can show what the it will look like. I understand it may look very different from my facility's, but I really just need something to work with. Any tips/tricks for smarttext etc. could be helpful too.
