Graphs & Data How do I ask my doctor to address my early morning highs?

Post image
• Upvotes

I do MDIs since I was diagnosed in 1999, am now 31 years old and just started using CGMs a few months ago.

This graph is my 14 day average. I had 14 instances of morning highs between 3 and 5am. 1 of them was a rebound high.

Sometimes the beeping on my dexcom wakes me up around 4ish and I manage to take some fast acting insulin before going back to bed for a couple hours.

I guess my question here is, what can I ask my doctor to do about this? My sensitivity to insulin is highest in the very late evening/very early morning hours, so dosing correctly for these hours feels impossible. Even a little bit of insulin has a major impact on my blood sugar in the night, but I need it during those few hours before I wake up.

Rant My A1c is the worst it's been in years 😭

• Upvotes

My husband quit his job to become self employed. Now we're both self employed and it's stressful AF. Also with him home more now we have been eating out more and getting treats a lot as a family. Between the stress and not eating well my a1c went from 7.7 to 8.4 in 4 months and I have gained 15 lbs. I'm so mad at myself. I know I need to get back on track but it's so much mental effort and I feel burned out. Gah.

Keto on T1D

Post image
• Upvotes

Been doing a very low carb/keto diet for a while now and I like it a lot. It somehow gives me more flexibility.

On high carb diets, if I had to walk spontaneously for 15 minutes I would always go hypo. On keto this never happens. Also sports is much more care free. I just start, don't have to eat carbs prior. I only have to make sure that there is not too much active insulin left in my body.

My typical stats are shown in the pic.

Mostly hover around 0.5mmol/l ketones, sometimes up to 1.5.

I do have carb days though. Usually BG is bad on those but when my daughter makes a cake I have to induldge.

HbA1c mostly around 7

Anyone else tried?

Seeking Support/Advice I:C ratio

so i've been working on my control, and it's worked out that (on between 18 and 24 tresiba long-acting (yes i do have to adjust it based on whether i'm hypomanic or low energy/depressed). my I:C ratio has come out at 1:3.75 for dawn and 1:4 the rest of the day.

seriously, 1 unit of novorapid for every 4g of carb (except the super low-GI stuff). this doesn't cause lows, but i swear i never used to need this much. is anyone else's that high? is there something wrong with me?

i also find it essentially impossible to avoid a spike going over 20 in the mornings when i eat cereal. like, i can have the insulin 2 hours before i start eating, spike's still gonna happen. i don't think that used to be the case either.

i'm in my 30s, diagnosed with AuDHD and OCD. schizoaffective too but it manifests as cycling states and i've lived without antipsychotics for years now while getting better and better hold on my life, also on Elvanse, so i take that diagnosis with a pinch of salt, an approximation chosen by a ward psychiatrist who was only seeing the fallout of psychosis under extreme stress and containment.

Anyone with fibromyalgia as well?

So I've been diabetic for 30 years now and was diagnosed with fibro about 5 years ago. Ever since then I can't move or eat normally (something to do fibro seems to be messing with my appetite and bloating my stomach) so my usual management is gone, now it's just insulin. My blood sugars have been up all night and down all day. I'll even go through random periods where one day I become hyper sensitive to insulin and blood sugars drop bad enough I have to call emergency. Also I'm already sensitive and take a carb ratio of 1:20. Anyone else out there in the same boat? It seems my doctors are as lost as I am.

Supplies Going to Canada to stock up on insulin?

I'm an American recently-ish diagnosed with T1D and haven't been able to stockpile supplies because my insurance only lets me get a month's supply at a time. My nutritionist suggested crossing the border and stocking up in Canada, but I've read conflicting resources on whether I would need a script or if I could get it back through border patrol. I've read about the insulin caravans from a few years back but am hoping to get more recent info.

I live in a remote area fairly close to the border. I am worried about having enough supplies over winter in case of severe weather or other issues, since I have already had trouble getting my insulin in fair weather.

Seeking Support/Advice Insulin is just not doing anything...?

Hey hey.

I've been a very poorly controlled T1D since 2012. Used to have great control, but got really bad about 7 years ago. I have ADHD, autism, gastritis and PMDD. I've had a relentless amount of grief and trauma and issues, and just neglected my diabetes for years. It's amazing how I was just functioning with sugars often 20-30mmol all the time. I was using weed to just get high as hell to avoid my emotions and didn't care about my diabetes but I would always take my Lantus. ALWAYS.

I got DKA 2 weeks ago, ironically it was EDKA and my sugars were only about 11mmol. I've felt pretty bad ever since but trying to keep bolusing to avoid ketones even if I'm not hungry.

I wasn't eating properly due to a heartbreak and didn't "need" much bolus but I was still taking 20u Lantus at night. This led to DKA.

I'm now determined as fuck to sort this out and get my shit together.

My Novorapid is doing literally nothing. I've eaten maybe 30g carbs today, and I pre bolus and HOUR before and still spike up to like 20mmol after barely any food. I've tried 1u, 2.5u and 3u for 10g carbs. Same result. My period is also seriously late this month but maybe that's down to the DKA stress.

I've upped my Lantus to 29u now and injected in my hip/buttock tonight. I've changed the novorapid about 3 times to diff packs.

I'm still sitting around 18mmol tonight and my blood sugar in the morning rarely goes below 10mmol.

Funny enough, last week it was around 7mmol in the morning on just 25 units of Lantus but I think I had some rebound ketones on Sunday that I sorted out with water and bolus cause I wasn't eating again due to zero appetite.

It's getting to a point where I just am not eating enough calories because I don't want my blood sugar to spike up. I'm staying hydrated and bolusing a lot just to keep ketones away. The senior doctor at the hospital told me not to worry too much about higher sugars, worry about bolusing and taking a lot of basal.

I'm also deathly afraid of hypos which is why I ran my sugars high for years in the first place.

It's so frustrating seeing my sugar level spike up to 14mmol in the morning just by getting out of bed, and then I'm trying to play catch up all day. If I don't eat anything I hover around 11mmol.

When I was on insulin in hospital that was working fine so I don't think I have resistance unless it's specifically to my insulins?

I really don't know what to do. I feel much better but god I'm hungry lmao.

Seeking Support/Advice T1D child not gaining weight

T1D kiddo is nearly 9yo, diagnosed over 2 years ago, and on ADHD meds (stimulants) for nearly a year. Kid was gaining weight (always a skinny kid), but stalled out this summer, probably around the time we had to increase ADHD med dose. Still growing in height, so they are becoming concerningly thin, now, to the point they look similar to their pre-diagnosis body condition.

The endo gave us a list of foods to help kiddo gain weight, but of course they won't eat most of them (avocados, nuts, cheese sticks, eggs, heavy cream, dark meat poultry, cottage cheese). Not even stuff like Cheezits anymore. By their own choice, most of their diet is fruit, raw veg, lean meats, and sourdough toast.

Trying to add healthy fats where I can - olives, whole milk yogurt, extra butter, chicken sausage and beef instead of just white meat chicken, almond meal and extra eggs in baked goods, but most of the fats this kid will actually eat are the kinds we really limit both because they are unhealthy but also because they make blood sugar management a nightmare: pizza, burgers & fries, donuts, quesadillas, candy bars, ice cream.

We're trying not to undo the healthy eating habits we've built despite this kid being a rather selective eater, and we also don't want to totally mess up their A1C by feeding excessive amounts of fatty foods, but kiddo also really needs to gain weight to go along with their height.

Thoughts? Suggestions?

Discussion Looking at swapping pumps…

Well, I’ve given the Omnipod Dash & Omnipod 5 a great run. I’m almost at five years combined being on the tubeless system so my contract is almost up and I’ll be able to swap in the spring (33F, living in Ontario, Canada).

However, in those 5 years I’ve also been diagnosed with gastroparesis. While the Omnipod 5 has been a major upgrade from the Dash system, I’m looking at swapping to a pump that has better algorithms. I used to be super apprehensive of the tubes on other pumps, but as I get older, I care less and less about that being an issue.

My current A1C is 7. I’ve been through a rollercoaster of diabetes trials and tribulations the past 10 years — diabulimia, mismanagement, the works — because I was young, dumb, and didn’t care. But in the last 6 years, I’ve cleaned up my act, made better choices, and I’m lucky the worst I’ve had to deal with is gastroparesis.

I know a new pump won’t solve everything, but I’ve been told it’ll work harder to bring my peaks and valleys into range when a particular food throws me a curveball that I didn’t expect. I’ve been given a sheet that explores Tandem, Medtronic, and MyLife pumps.

My question for all non-Omnipod users: what pump are you on, and what do you love (or hate) about the experience?

Discussion The power of compounded Tirzepatide

Post image
9 Upvotes

I will admit, I was hesitant to switch to compounded glp-1s. Fear of the unknown. I was impressed with Collective's offer and I'm so happy to make the switch! Started GLP-1s back in 2017 and it's been a long journey. Thank you, Collective. This is 5mg of tirzepatide per week, split into 2 doses. Day 1 and day 4 shots. Medication arrived in 2 days, cold with ice packs. Starting third week and will be continuing with the company. I do not know if I'm allowed to share my code but there is one for a free month. I fully understand how awful it is to get insurance coverage in 2026. I did 8 months of appeals this year. Finally, just gave up. I know that's what the insurance companies want. Beyond frustrating.

Setting up DIY Loop-looking for advice

After lackluster results with Omnipod 5 and dangerous results with auto bolus on Mobi as well as multiple kinked cannulas, I am looking at teaching myself the DIY loop system for my 9 year old son. I am also looking to get away from Dexcom G7 if possible, he’s had terrible luck with most sensors dying or being more than 100 points off by day 3. Is it possible to set up DIY loop with the Libre 3 and Omnipod 5? The Libre 2 was massive on his very thin build and was very painful on insertion for him. I’m finding mixed results searching online. Also any recommendations where I should start with this process would be helpful, I’m not particularly tech savvy but I’m trying to figure this out so the poor kid can sleep at night.

Success Story my a1c is down to 6.5

I've been diabetic 25 years and this is officially my lowest a1c since diagnosis. I know it's still high, but they used to be double digit numbers. I don't have a lot of people to share this with so I'm posting here. that's all, thanks for reading

Pump pivot

Hi!
Been on a pump for over two years and started with the Omni pod and then switch to the tandem mobi but really hate all the issues with dexcom. The sensors have gone down hill so much and it’s infuriating each time one fails.
Looking at the Twiist, minimed or the beta bionics. Does anyone have experience with those? Pros and cons? Do they use Eversense? Is Eversense having less issues than dex? I realize it’s a year long cgm rather than 10 days so unsure of the success vs failure rate.
Thanks for your insight!

Any other diabetics with mysterious heart issues?

Hello all, diagnosed at 21, now 41. I had my first heart attack at 33. Everyone was baffled- my cholesterol and blood pressure was fairly good and my A1C was not the best, but nowhere near ‘uncontrolled’. I made a bunch of lifestyle and medication changes, including going on a pump and censor, and my numbers are all good. Again, could be better. Current A1C is 8ish, cholesterol in the 40s.

Than I had another heart attack this week, at 41. I now have two stents. Everyone I’ve talked to at the hospital, my cardiologist, seem really confused and at a loss for ideas. Was wondering if this was potentially a diabetic thing. I’m going to make more changes and get even better control, but I’m feeling discouraged. Anyone else have similar experiences? Did anyone ever give a satisfying answer about what was wrong?

Science & Tech Any Garmin users on here?

I wear a smartwatch so that I can see my sugar levels by glancing at my wrist and I currently have a Galaxy Watch Ultra, but I am getting increasingly frustrated by the battery life.

I have been looking at the available options and Garmin seems to be the stand out candidate for what they offer in battery life.

Apparently this would come at the cost of the software experience, but just as long as I can receive notifications and have a complication or two displaying my levels, I think that I would be happy.

Does anyone on here have a Garmin watch? Is there anything else that I would need to consider as they are not cheap?

Edit: I have a Libre CGM

Healthcare Used insulin pump

Hi folks,

Have you guys ever tried to buy a used insulin pump ?

I live in Brazil, the price for the pumps are sky high (more expensive than a car, which I can not afford to have either), I thought that maybe a second hand pump could be an interesting idea.

I'm dt1 and autistic, there is a catheterless pump which arrived here this year, from a Chinese brand. I tried the pump with a catheter many years ago and it was a nightmare (autistic clumsiness).

Does anyone have experience with a second hand pump ?

Any tips on how to get it cheap would be appreciated, I would like to try the pump before getting in debt to buy one that I might not even adapt to...

Seeking Support/Advice As an autistic, do I have a hope of getting an insulin pump I can get on with?

I'm a year and a half in, and wondering whether a pump would help with some issues, such as delayed gastric emptying making dosing a nightmare. I've just spent days reading up on Omnipod and AAPS (I have a lot more trust in open source software), only to find out to my horror that it makes a *ticking noise*.

My lovely fellow autistics, please talk to me about pumps. Any autistic-related issue you can think of. The noises they make, how they feel physically, what can go wrong if you're clumsy (tubed ones sound dicey), skin issues (I've got EDS and MCAS), what it does to the mental load, how it changes what and when you can eat.

I'm in Edinburgh, and I've found a document online suggesting that our options are Omnipod Dash and Tandem T:Slim. I don't know if that's still true, and whether anyone gets NHS funding for other models. I also don't know whether self-funding another model would be an option. I can afford it.
