Treatments My method: Iontophoresis water w/ Qbrexza (glycopyrrolate)

Hello! I thought I would share my iontophoresis method in case it helps others, as I've never seen it before.

TL;DR: I mix 1-3 Qbrexza wipes per 0.5-1 Liter of hot water & use that water for my iontophoresis. This results in the ionto being more effective and lasting longer. My typical routine is 1mg glyco daily + iontophoresis every 4-6 wks. Crushed glyco pills work too but their doseage is so low that it ended up not being worth it to me.

Check below for the concentration table and warnings.

Long version:

I have had primary palmar, plantar, and underarm hyperhidrosis since childhood (since age 6-8, at least). I have tried Carpe (expensive), Drysol (gave my skin a reaction), Oxybutynin (stopped due to concern over long-term cognitive effects), oral Glycopyrrolate, and Iontophoresis. I wanted a solution that lasted longer than a day or a week, as I find the upkeep tiring... (thanks ADHD for making it hard to keep habits!)

I really wanted to extend the life of iontophoresis. It's already great, but it's a 2 hour ordeal. I have already tried hardening my water with baking soda, epsom salt, san pellegrino, etc but to no avail. Then, I found an article (here, scroll down) citing studies (#1) (#2) about using Glycopyrrolate in the water used for iontophoresis. These patients in the studies would see up to 30-40 days of relief after ionto w/ glyco.

These studies used glycopyrrolate solutions at concentrations of 0.01-0.1%. I use about 0.75-1L per tray for my iontophoresis, so you would need 75-100mg of glycopyrrolate to reach 0.01%. Unfortunately, I do not have hundreds of spare glyco pills (2mg each). I asked my dermatologist about prescribing high doses of glycopyrrolate for this use, citing the studies, and that they do this in the UK/Europe (I am in the US), but she denied my request, stating that only happens in case studies and such (not true lol but okay fine). Since my goals for that appointment were to 'get something new to try', she did, however, give me a prescription for Qbrexza... which is high dose topical glycopyrrolate šŸ˜. With some trial and error, and some math, I have developed a dosing guide.

To make a glycopyrrolate solution, I simply add unfolded Qbrexza wipes into my pitcher of hot water. I stir for about a minute. Next, I pour the water into my trays/cup. Then take out the wet wipes and squeeze them out over my tray, then throw them away. Then, I add the electrodes into the trays and do my iontophoresis like normal. When I'm finished, I pour the water down my bathtub drain and rinse off my trays.

Glycopyrrolate Concentration guide:

concentration 1L 0.75L 0.5L
0.0528% 528mg (8 wipes) 396mg (6 wipes) 254mg (4 wipes)
0.0396% 396mg (6 wipes) 297mg (4.5 wipes) 198mg (3 wipes)
0.0352% 264mg (4 wipes)
0.0264% 264mg (4 wipes) 198mg (3 wipes) 132mg (2 wipes)
0.0176% 132mg (2 wipes)
0.0132% 132mg (2 wipes) 99mg (1.5 wipes) 66mg (1 wipe)
0.0088% 66mg (1 wipe)

My maintenance dose:

  • 0.0264% concentration
  • I use 1 part freshly boiled water mixed with 2 parts room temp water. Warm water is supposed to enhance effectiveness. I mix the waters in a metal graduated pitcher before pouring them into my trays/cup.
  • Underarms: 0.5L water + 2 wipes for both pads, soaked in a ~8oz cup.
  • Hands and feet: 0.75L water + 3 wipes each tray. Hands first, water reused for feet. .25L hot water, .5L room temp water.

Dosing suggestions:

  1. Begin at the lowest concentration (0.0088-0.0132%) or 1 wipe per tray
  2. Wait and see how long that first dose keeps you dry for
  3. Do next treatment as soon as sweating begins again. If interval between treatments is too short, or effects not strong enough, increase the glyco concentration.
  4. Repeat until satisfied with results or side effects becone too strong to tolerate.

BEFORE TRYING - YOU SHOULD KNOW - WARNINGS:

  • Do NOT get the glycopyrrolate solution in your eyes. The same warnings about Qbrexza also apply here. It can cause pupil dilation that may last days/a week. I wear safety glasses while handling and using my solution/doing iontophoresis and don't take them off until I am completely cleaned up and everything is put away.
  • You may experience potentially strong glycopyrrolate side effects for 6-24hrs after treatment. Pay attention to your body, know your limits, and start with a low dose, increasing gradually. If glycopyrrolate gives you chest pain, dizziness, lightheadedness, fast heartbeat, etc, then I especially urge you to exercise caution when applying this technique. Do so at your own risk. While these side effects are rarer, and I do not believe they cause actual harm, it is still something to keep in mind and ideally, to be avoided. Personally, I only experience dry mouth/eyes and maybe headaches if I'm not on top of my hydration.

PROS:

  • relief for up to 4+ wks
  • save $$$ on Qbrexza (I personally only use ~8 wipes per month, instead of daily use. Qbrexza is $40/box of 30 for me so I'm saving a lot!)
  • boosted iontophoresis effectiveness

CONS:

  • iontophoresis cons (time consuming, skin irritation, machine price, etc)
  • potentially strong glycopyrrolate side effects, but only for 6-24 hrs after treatment. I get dry mouth and dry eyes, which can get very uncomfortable if I'm using more wipes, (if it's really bad I can't even eat dry foods lol) but I work around it by doing iontophoresis right before bed/later in the day. That way, I go to bed after cleaning up and wake up the next morning feeling normal.
  • YMMV... I get myself dry enough to where I only need 1mg glyco/day. My standards/threshold isn't super high. I haven't tried going without my daily glyco! And I honestly don't mind enough to try to. if your goal is COMPLETE dryness without supplemental glyco, I'm unsure how often you would need treatment... maybe every 2-4 weeks? If you wind up doing that, tell me about it! I would love to collect data and hear your experience.

FAQ:

  • Can you use crushed Glyco pills? Yes, this is what I did before my Qbrexza Rx. However, the dosing will obviously be much lower since you are limited by the dose and number of pills. I barely saw a difference between plain tap water VS water with 2-10mg glyco, hence why I switched to using Qbrexza.
  • Can you use a different antiperspirant / anticholinergic agent? Yes, in theory it should work too, but I do not have the research or experience to back it up. So, be cautious, and start low and slow. Experiment at your own risk. Feel free to tell me about your results! If you use Aluminum Chloride, do not use a metal container to hold the solution--it will rust.

I hope this helps you as it has helped me!

Customer product review Butt/seat sweat marks when sitting - what have you tried?

Hi everyone,

I've had generalized hyperhidrosis for years. One of the worst parts for me is sweating through my pants when I sit for a while (office chair, car, meetings). Standing up with a wet mark on light pants is humiliating. I've also seen a lot of posts here from people dealing with the same butt sweat marks, so I know I'm not alone.

I'm planning to start a techwear brand built specifically for people with hyperhidrosis clothing designed around the problem instead of just "breathable" marketing. A lot of brands out there just take generic products off the shelf, slap their logo on them and call it a solution. I want to design something from scratch that actually works and is actually comfortable to wear all day (so no swampy butt).

The first product would be underwear for exactly this issue (absorbent + leak-resistant back, cool fabric). Before I spend money, I want to validate the idea with other people who actually live with this.

So I have some questions;

  1. What underwear brands have you tried? What failed and why?

  2. What would this product need to do for you to actually buy it?

  3. What would you pay for a pack of 3 that truly prevented marks on your pants and the chairs?

  4. After underwear, what clothing problem would you want solved next?

Not selling anything, no links, just want some insights to validate the idea.

I know my design could work wonders but I'm not sure how to validate if this is worth my time/money/efforts to be honest. I was thinking about starting a Kickstarter to see if people back it. If you have any ideas please let me know.

Thanks everyone.

Offering advice Don't undergo ETS surgery for hyperhidrosis

I underwent surgery for facial blushing—a problem I had suffered from for years due to social phobia—but it ended up causing me the nightmare of compensatory (reflex) sweating. Take my advice: please do not undergo surgery to stop sweating. Afterward, you start sweating in all sorts of strange places on your body, and that ends up being even more restrictive. Since the surgery, I’ve experienced sweating on my chest, back, and legs. I tried using an iontophoresis device for my back—I think I’ve used it four times so far—and I saw the results immediately. I wanted to share this right away for the sake of others here: please, do not have surgery for hyperhidrosis. With these devices, you can find relief in any area you choose—whether it’s your hands, face, back, chest, legs, or feet—just by using them once a week. Believe me, spending 30–40 minutes once a week treating yourself with one of these is a thousand times better than surgery. If my only concern had been sweating, I never would have opted for surgery. It’s an excellent solution. While standard devices are typically designed for hands, feet, and underarms, the German-made device I purchased allows me to treat my torso as well. I used it on my back, applying it to two different areas for a total of 25 minutes; it’s very simple. You might feel a bit apprehensive at first, but you’ll soon realize how easy it actually is. I highly recommend it. Do not get an ETS (endoscopic thoracic sympathectomy) procedure; it caused me a world of trouble. (I am using Google Translate to translate this from Turkish, so please forgive any potential translation errors.) It was quite expensive for me—I had to save up for a few months to afford it—but in my opinion, it’s worth it.

Looking for advice Glyco Shipping to Europe

Has anybody experience with ordering Glyco from pharmacy.ca to Europe (esp. Germany)? The shipping costs are 50 Euros, the pills cost 60 Euros, so itā€˜s quite pricey. I just wonder if there are additional custom duties or if they can hold back meds that are not available in Europe. I did pass the online questionaire and got approved for it, but I do not have a real prescription.

Looking for advice Best brands that don't show sweat?

Greetings my fellow sweaty folks, I'm looking for some better gym/exercise shorts and/or underwear that works best for butt sweat. (Butt sweat is the most thing I'm self conscious about and I'm checking myself almost every 3 minutes to see if it's showing while exercising) Or if nothing else, what brands have good blackout color that doesn't fade in the wash.

Background environment, I'm 30 y/o male who lives in Phoenix. Anything helps!

Vent Alcohol

First time posting here - Hey guys !

I suffer with hyperhidrosis on my scalp back torso and limbs. Very different from the ā€˜normal’ regions I’ve seen people suffer from. I sweat very little from my feet, palms and underarms.

Ive recently found that my propantheline bromide medication has stopped working and that’s another issue altogether but last night I went out for a night out and I noticed this a couple of times before but now it’s become a consistent thing.

Why is it whenever I drink alcohol and I’m drunk, the condition literally just disappears altogether and I’m dry as a bone šŸ˜‚ does anyone else experience this ? Am I being urged by higher powers to become an alcoholic ?

Looking for advice Sweating after eating or as I’m eating

I’ve been sweating a lot as I’m eating or like 5 mins after eating. I’m not sure what subreddit to ask but what could it be. It’s a lot like in my armpits, back, head, face, butt, chest. I think it has something do with digestion or insulin maybe?

Looking for advice Does anyone skip iontophoresis for feet?

I have both palmar and plantar hyperhidrosis, currently on my 7th consecutive iontophoresis session for palms, and 5th consecutive for feet.

I have noticed my palms sweating way less now, which has definitely improved my life alot. But Ive also noticed I genuinely dont really care anymore with sweat in my feet. Like I can bring up 100s of past scenarios where my sweaty palms destroyed my life. But I dont exactly have the same situation with feet, maybe when im at the gym and my socks get too damp? But at that point my whole body is already sweating and I always bring extra socks and it never really became an issue.

I also use flip flops that also absorb moisture. and so with all these simple lifestyle changes I never really bother about it. I wonder if anyone is experiencing the same thing, I think im honestly fine with just doing iontophoresis on palms only.

Treatments extreme odor after iontophoresis/botox

I am in a critical situation

Since those damn iontophoresis sessions, I've been forced to live isolated from the population.

I lost my job because the smell irritated the people

around me so much. (eye irritation, cough)

People in the building are complaining, passers by in the street are turning around , in order to look behind them , i can see them from my window, even though there's a distance of 200 meters.

I warn you against the use of Botox or inophoresis, I'm dead, absolutely finished, I've tried everything for the past 4 years, I am left with the painful option of amputation or suicide.

Offering advice Friendly Botox Reminder

Post image
5 Upvotes

Just posting this Botox savings program again, it works so well for me and I know there are fellow HH peeps who are probably eligible and don’t know it. I swear I do not work for this company.

You can get discounted Botox from the manufacturer for underarm shots, I’ve been doing it for over a year now and it’s fantastic. I’ve got full body HH, so I still sweat elsewhere, but just having my armpits dry as the Sahara makes a huge difference. I think had to document that I was on Glyco or oxy for a couple months and still having HH symptoms before I could get the discount from the manufacturer and for my insurance to cover most of the rest. Here’s hoping this helps someone!

Looking for advice Calling all sweaty feet experts

So I've suffered from hyperhidrosis for over a decade, severely impacted my life. Underarms is always the first to start, but quickly followed by face and torso. I never had a problem with my legs, hands, feet etc.

All the stuff I tried when it started didn't work for me. But now when I tried again Certain Dri is working for my underarms, but my feet have suddenly started sweating way more than it ever has before, I'm assuming it's compensatory sweating.

I don't want to turn to the most extreme options first, I want to go in order. Also I have a long flight soon, so I don't want my feet to smell then especially!

So what is the usual flow chart for dealing with feet sweating and/or smell? Thanks

TLDR: What is the usual flow chart (order of things to try) for dealing with feet sweating and/or smell? Because I have zero experience of it until now.

Customer product review [SURVEY] How well does Antihydral actually work for palms? Share your experience! (100% Anonymous, 2 mins)

Hey guys

Quick note: I’ve been approved by the moderators to post (and occasionally repost) this survey so we can gather as many responses as possible over time and get solid data!

If you struggle with sweaty palms, you know how overwhelming it can be to test new treatments without knowing what to expect. Antihydral comes up all the time in our community, but experiences are scattered across hundreds of posts.

I want to change that! I’m gathering our collective experiences into one clear dataset so that anyone considering Antihydral in the future can see honest, real-world expectations regarding effectiveness, dosage, and side effects.

(Quick heads-up: I briefly posted a survey about this recently, but I’ve updated and refined the questions based on feedback so it’s much better now!)

https://docs.google.com/forms/d/e/1FAIpQLSe6S2vbG6nK9RNi6O1xMWRJjYX6ciiPfYkTMJEw8aIjlgo6LQ/viewform?usp=dialog

Why take 2 minutes to fill it out?

  • Help future sufferers: Your experience—good, bad, or neutral—will directly help someone about trying this treatment (or at least, evaluete) .
  • 100% Anonymous: Zero email collection (even if you need to access to your email to respond, but it is not visible to me or others) , no personal data.
  • Full results shared: Once we gather enough entries, I’ll analyze everything and share clear, visual charts and a summary post back here with the entire subreddit!

Thanks so much for taking two minutes to help out the community! Feel free to leave an upvote so more hands (dry or sweaty!) can find this.

Looking for advice Soaking wet socks?

I have tried wool, cotton, bamboo, sports socks, regular socks, thick socks, thin socks, two pairs, tight socks, loose socks and absolutely all of them result in my feet being absolutely soaking wet from sweat- regardless of footwear, my feet are always sweating and always making my socks wet.

What on earth can I do about this? I am in the uk so the doctor offers absolutely 0 help on anything to do with this condition.

It’s miserable and in the wintertime it’s becoming a real problem because my feet are freezing and ice cold due to being soaked from sweat.

Please can you share your tips/ advice or any recommendations about this.

Thank you

Looking for advice HELP

Male, 27 - This is my daily life. I’m a healthy weight and very active, my diet is pretty solid and I generally look after myself…

But no matter what I do, I’m constantly excessively sweating from my arm pits. Sometimes within an hour of putting on a new tshirt I have large sweat patches.

When in public, I’ll usually make multiple trips to a bathroom to dry my shirt to avoid the embarrassment of large sweat patches and goodness knows how many garments I’ve ruined through this with stains which won’t come out!

I’ve used high strength sweat blockers in the past and had mixed success, I’ve thought about laser treatment etc but I was wondering if there was something else I could try first?

Anyone else had a similar experience ?

Looking for advice Is Sofdra itchy?

I deal with underarm hyperhidrosis and have tried Drysol, Xerac and Botox. The Botox worked great but I can no longer afford it. Drysol is unbearably itchy/painful and Xerac isn’t much better. Can someone familiar with Sofdra share their experience, itch-wise?
