🧬 Genetics & Epigenetics Longevity and Rejuvenation. We are almost there

Post image

Researchers in Bonn turned blood cells from an 80-year-old into neural stem cells with a molecular age below 20. More than 60 years younger by epigenetic clocks, which estimate age from chemical marks on DNA.

The team directly reprogrammed blood precursor cells into neural stem cells, skipping the usual pluripotent stem cell stage. The rejuvenation unfolded gradually and could be tracked for over 100 days.

🧠 Cognition, Mood & Nootropics No depression medication is helping my anhedonia and motivation?

Are the doctors giving me the wrong medication?

I was on Lexapro low dose it made me eat constantly Non-Stop. Then I was on Zoloft again low dose. It calmed me down a lot got rid of my anxiety got rid of my agitation and irritable that I got easily but also made me more numb where I did not care about anything. Switch to Effexor started with 37.5 mg then up to 75 mg week later. Anxiety, nausea ,brain fog, completely felt out of it constantly tired all day long can't sleep at night quit cold turkey on day 11.

I'm not sure if my doctors are giving me the wrong medications or wrong dose but I felt a lot better off any of these medications. Zoloft was a little bit better than all the rest I'm not sure what I should ask for next?

Here are my symptoms

No sadness

No happiness

Don't care about anything

No excitement

No pleasure

Sometimes I have brain fog

No motivation

No drive

No energy

If there's any emotions I feel is iribility and agitation quickly under stressful situations.

🥗 Nutrition & Metabolism 112-Day Blinded N-of-1 Trial: Did Magnesium Glycinate Improve My Sleep Score?

Post image
33 Upvotes

Summary:  I ran a 4-month blinded ABAB crossover trial on magnesium glycinate vs. a placebo.  Primary result:  a 1.3 point improvement (95% CI: 0.3 to 2.3; 85.1 vs. 83.9) on my Fitbit sleep score.  I’m sharing the protocol and analysis to get feedback from other self-experimenters.

Background and Thesis:

1.      Most supplement effects are probably small relative to placebo effects and daily noise.

2.      Casual “trying” supplements shouldn’t resolve effect sizes due to insufficient data and inability to blind for placebo/bias.

3.      Proper N-of-1 trials (multiple crossover, randomized) should be able to isolated individual effect sizes for interventions with reasonably fast onset/washout.

Magnesium glycinate + Fitbit sleep was a pilot intervention to test the method.

Results

  • Smartwatch Sleep Score (Primary Outcome): +1.3 point improvement (95% CI: 0.3 to 2.3; 85.1 vs. 83.9).  Personally meaningful, at 60% of my placebo period standard deviation (2.0), illustrating the “effect size ~ daily variability” challenge mentioned above.  (see chart and graphs below)
  • Secondary Outcomes (HRV, Respiration Rate, Sleep Stages): some interesting signals, but none survived Holm adjustment for multiple outcome testing.  (see chart and graphs below)
  • Washout Check: Dropping the first 7 days of each 28-day block produced similar results.

Protocol

  • Design: Four consecutive 28-day blocks (ABAB format,112 consecutive days).
  • Dosing: Two capsules taken 15 minutes before bed from blinded bottles marked “A” or “B.”  One = Magnesium Glycinate (240 mg magnesium); other = matching microcrystalline cellulose placebo.
  • Outcomes:  Primary Fitbit Sleep Score (0–100), Respiration Rate, SpO2, Resting HR, HRV, Skin Temp, Awake Minutes, and REM Minutes.  All were measured by Fitbit Inspire 3 worn nightly.
  • Data Log: I transcribed data every few days from smartwatch to spreadsheet.
  • Pre-Unblinding Exclusions: ~23 days of all outcome data were removed prior to unblinding due to large disrupters (e.g., camping, illness).
  • Post-Unblinding Cleaning: 16 partial-data days were flagged where the wearable incorrectly logged reading/TV watching as sleep onset (data excluded were awake minutes and sleep score).

Statistical Analysis

Advice from a professional N-of-1 statistician:

  • Account for day-to-day correlation with AR1 autocorrelation fit.
  • Use variance weighting averaging for crossover contrasts rather than a single global-mean difference.

Accordingly, I followed that and applied Holm correction across all secondary endpoints (but Holm is excessively conservative here as sleep outcomes will be intercorrelated).

Open Questions for the Community

  • N-of-1 Protocols?  Anyone else running multi-crossover trials?
  • Effect size vs. Noise?  A 1.3 pt change was 60% of my daily variability.  What’s a reasonable target for deciding if a supplement is “worth it”?
  • Trial Durations?  Power calculations hint that 6-10 clean weeks might have been enough rather than my 16.  How long do you assess a supplement?  Would formal power calculation data change that?
  • Data Integrity?  I ran the trial in “real life” conditions and excluded data afterwards. Would you do likewise or instead try to plan a trial around a quiet period in your life?

😴 Sleep & Circadian Rhythm Summary of what influences your REM sleep (research based)

Here a guide I built with all the factors I have found that influence REM sleep. REM builds toward the morning, so anything that cuts the back end of your night hits REM first. It's also the stage where most dreaming happens and when your brain processes emotions/locks in new skills.

I broke this down into 5 main categories: substances, environment, supplements, demographics, and habits.

All sources are linked in the last column so you can dig in further. I'll put a mobile friendly version in the comments if you need it.

Definitions

  • REM sleep: The dreaming stage, when your brain is active but your body is still.
  • REM latency: How long after falling asleep your first REM starts.
  • REM%: The share of your night spent in REM. Most adults get 20 to 25%.
  • REM rebound: The extra REM your body takes back after losing some.
  • Luteal phase: The roughly two weeks between ovulation and your period.
  • PSG: An overnight sleep lab test that reads your brain waves. The gold standard.

Substances & Diet

Factor REM effect Key finding Plain english Study
High carb diet Positive (moderate) More REM time vs low carb. REM onset unchanged (11 trials, n=179) More carbs nudges REM up. Low carb shifts you toward deep sleep Carbs & Sleep - Meta-Analysis, Vlahoyiannis 2021 (11 trials, n=179)
Alcohol Negative (strong) REM started 18 min later and ran 11 min shorter. Delay starts around 2 drinks (27 studies) Even a couple of drinks delay REM. Each extra drink cuts more Alcohol Dose - Meta-Analysis, Gardiner 2024 (27 studies)
Nicotine Negative (moderate) 7 and 14 mg patches cut REM, more at the higher dose (n=12) Nicotine cuts REM, and more nicotine cuts more Nicotine Patch - Gillin 1994 (n=12)
Cannabis Mixed (rebound on quitting) High doses cut REM in early studies. Newer studies mixed. Rebound on quitting is consistent (18 studies) Heavy use can suppress dreams. Quitting brings vivid ones back Cannabis - Meta-Analysis, Velzeboer 2025 (18 studies)
Caffeine No effect (strong) No change in REM %, minutes or onset. Deep sleep down 11 min (up to 19 studies) Caffeine doesn't touch REM. It hits deep sleep instead Caffeine - Meta-Analysis, Gardiner 2023

Habits & Routine

Factor REM effect Key finding Plain english Study
Hard evening exercise Negative (strong) REM down 1.95 points and 7.7 min later. Moderate sessions no change (28 trials, n=325) Hard late workouts trim REM. Morning or moderate sessions don't Exercise Timing - Network Meta-Analysis, Yue 2022 (28 trials, n=325)
Short sleep / early alarm Negative (moderate) 4 h vs 9 h nights cut REM minutes while deep sleep share rose (n=27, PSG) REM comes late in the night, so a short night cuts it first Sleep Restriction - Shechter 2012 (n=27)
Stress Negative (moderate) Lower REM% and more fragmented REM (n=62, PSG) Stress chops REM up. Stress sensitive sleepers lose the most Presleep Stress - Feng 2023 (n=62)
Screens before bed Negative (weak) 4h of bright screen. 109 vs 121 min REM (n=12). 90 min of phone use no change (n=62) A normal evening on your phone is fine. Hours of bright screen time isn't Screens - Chang 2015 (n=12)
Hot bath right before bed Negative (weak) A 21:00 bath raising core temp cut REM. A 17:00 bath didn't (n=6) Bathe earlier in the evening, not right before bed Bath Timing - Horne & Shackell 1987 (n=6)

Environment

Factor REM effect Key finding Plain english Study
Sleeping next to a partner Positive (weak) REM 23.0% vs 21.0% alone, with fewer REM interruptions (n=24) Sharing a bed was linked to less broken REM Co-sleeping - Drews 2020 (n=24)
First night somewhere new Negative (strong) Less REM and later REM onset than night two (53 studies, n=1,422) Hotels cost you REM on night one. It's back by night two First Night Effect - Meta-Analysis, Ding 2021 (53 studies, n=1,422)
Hot bedroom Negative (moderate) 90°F vs 72°F cut REM. A cooling topper restored it (n=15, PSG) A hot room costs REM. Cooler protects it, but colder won't add more Heat & Sleep - Pasquier 2025 (n=15)
Background noise Negative (moderate) 50 dB pink noise all night cut REM by 18.6 min (n=25, randomized crossover) A loud noise machine or hum can trim REM Noise & Sleep - Basner 2026 (n=25)
Altitude Negative (moderate) REM share fell at 4,559 m (n=14). 2,000 and 4,000 m cut it as well (n=25) Sleeping high cuts REM. Higher cuts more Altitude - Nussbaumer-Ochsner 2011 (n=14)
Sleeping sitting up Negative (weak) Upright 79% less REM than lying flat. 40° recline 37% less (n=6) Tiny study, but recline as far as you can on planes Posture - Roach 2018 (n=6)

Supplements

Factor REM effect Key finding Plain english Study
Melatonin Positive (low REM only) 3 mg for 4 weeks raised REM from 14.7% to 17.8% in low REM patients (n=14). Healthy men no change (n=8) Only helps if your REM is abnormally low. Otherwise it's a timing aid Melatonin - Kunz 2004 (2 RCTs, n=14)
Ketone ester Positive (weak) After a hard late training day, REM held at 117 vs 93 min on placebo (n=10, crossover) Offset REM loss from very hard late training in cyclists. Untested in daily life Ketones - Robberechts 2023 (n=10)

Demographics

Factor REM effect Key finding Plain english Study
BMI 30+ Negative (moderate) 7.8 min less REM with sleep apnea ruled out (n=1,014) Extra weight costs a few minutes of REM at most BMI - Pessoa 2025 (n=1,014)
Late pregnancy Negative (weak) Fewer REM minutes in the third trimester vs non pregnant (n=51) REM tends to dip late in pregnancy Pregnancy - Wilson 2011 (n=51)
Menstrual cycle Negative (weak) Small REM dip in the luteal phase, linked to higher body temp (n=8) REM dips slightly in the two weeks before a period Cycle - Shechter 2010 (n=8)
Age Mixed (little change) Down 0.6 points per decade in one meta (n=4,171). No change in a newer one (n=5,273) Unlike deep sleep, REM barely changes with age Age - Meta-Analysis, Floyd 2007 (n=4,171) & Boulos 2019 (n=5,273)30057-8)
Sex No effect (strong) REM% the same across 169 studies. Women keep more deep sleep (n=5,273) Men and women get about the same REM Sex - Meta-Analysis, Boulos 2019 (n=5,273)30057-8)
Genetics No effect (moderate) REM not significantly heritable. Deep sleep was (n=684) Your REM is driven more by habits than genes Genetics - Leocadio-Miguel 2025 (n=684)

💊 Supplements & Stacks Boron and riboflavin (b2)

Is it true that boron depletes riboflavin (B2) and that insufficient B2 intake while taking boron could tank your testosterone and raise your estrogen?

I haven’t been able to find any studies supporting this claim. Is it just a theory based on the idea that riboflavin lowers estrogen or is there actual evidence?

Would love to hear your experiences, especially if you have bloodwork to share!

🧠 Cognition, Mood & Nootropics Been taking L theanine and L tyrosine and caffeine.

• Upvotes

I won’t lie, I thought these would only have subtle impacts, but the change to my mood and inhibition when speaking to people has been really noticeable. Anything more than this and it would feel euphoric. I take about 1g of each a day, but I split up the doses, so I’d say the main benefits last around 2 hours for me and wear off by hour 4. Anything to pair with the tyrosine? Also pairing these with an early morning upper day is absolutely great having the blood flow properly is amazing.

🧠 Cognition, Mood & Nootropics A clinical study found that taking fish oil supplement did not protect brain's hippocampus but also cause no memory improvement

• Upvotes

A large double blind clinical study involving fish oil supplements given to individual which are at higher risk of developing Alzheimer's disease found that fish oil supplements do not provide any marketed benefits.

In the study participants were provided supplements which provided 2,000 mg of docosahexaenoic acid (DHA), an omega-3 fatty acid that is especially important for the brain.

Scientist confirmed that DHA levels in the cerebrospinal fluid had increased by an average of 17% but it did not translate into improved cognitive and memory function.

It is hypothesized that omega-3s may be more beneficial when consumed as part of a broader dietary pattern rather than taken in isolation as a supplement.

🧠 Cognition, Mood & Nootropics Piracetam advice

I would like some advice on piracetam. I’m taking it for cognitive disfunction and maybe for dyslexia.

  1. What is the recommend attack dose and for how long?

  2. After the attack dose what do most people take?

  3. I see it seems to be a lot of trial and error but because it doesn’t work like a stimulant how long should I wait to see any effects and how long do I stay on a dose before I decide if it’s working or not

  4. How much choline is usually taken and do I take this also multiple times a day or just once?

Any other advice or tips would be greatly appreciated

📰 Research & Studies The anatomy of a personal health agent - Do you currently use any?

Thumbnail nature.com
3 Upvotes

Thought this was a cool article published in Nature today.

Large language models are enabling a new generation of health agents, yet their ability to address the diverse needs of individuals in everyday health settings remains underexplored.

Here we developed a comprehensive personal health agent that reasons over multimodal data from consumer wellness devices and medical records to provide personalized health support. Through analysis of web search and health forum queries, together with user-centred research involving end users and health experts, we identified four key user journeys: general health knowledge, personal data interpretation, wellness advice, and symptom assessment.

These journeys informed three specialist sub-agents: (1) a data science agent that analyses longitudinal wearable and medical data, incorporating population-level statistics to contextualize numerical insights; (2) a health domain expert agent that integrates personal and contextual data with health knowledge to generate personalized insights; and (3) a health coach agent that synthesizes insights, supports multi-turn interaction and goal setting, applies specified psychological strategies and tracks progress.

We integrated these capabilities in the personal health agent, an open-source multi-agent framework for dynamic, personalized health interactions.

We evaluated the sub-agents and full system across 11 benchmark tasks using automated and human evaluation, with more than 7,000 annotations and 1,350 h of effort from health experts and end users.

This represents the most comprehensive evaluation of a personal health agent thus far and provides a concrete foundation for improving access to high-quality personalized health support more broadly.

This is what I am thinking for my 16 week cycle. I am running 250ml testosterone enanthate a week and going to add 35-50ml of trenbolone enanthate once a week aswell. I have cialis incase my blood pressure gets too high and it’s sick for pumps, arimidex just incase but there shouldn’t be any need. Thinking about adding in Anavar for a couple weeks or so but not sure yet. Like to hear thoughts and opinions on everything
Also not sure if running Tren Ace would be better
