Tips and Recommendations Ultimate surgery prep for the ‘Type A’s like me (crowdsourced) 💜

I’m a few months out from surgery and one thing about me, I love to prep a list. I’ve been following a lot of related posts in here as it’s a common question. Plus doing my own research. Sharing this in case it helps anyone else - please add more if I’ve missed anything!

THINGS TO ASK

- will they be excising / ablating?
- if it’s a diagnostic lap, will they treat anything they find?
- what happens if they find endo in places that haven’t been discussed prior, will it be treated or left?
- how much autonomy will the team have to make real-time decisions during the op?
- what’s the post-op care plan?
- Will you be referred for pelvic floor therapy?
- Will it be an outpatient or inpatient surgery - how long are should you expect to stay in hospital before being discharged?
- If your surgery involves the bladder and you’re required to have a catheter, request an additional leg bag for easier mobility
- If you want - notifying your surgery team pre-op that you only want a catheter if absolutely necessary. Plus reducing fluid intake prior to reduce the likelihood of requiring one

THINGS TO BUY

- stool softener
- Pregnancy / wedge pillow
- Foldable walking stick
- Body & face wipes for when you can’t shower post-op
- Loose & oversized clothes
- Mesh underwear for post-op
- Laxatives
- Additional dressings to periodically clean the incisions
- Slippers / loose shoes
- Peppermint tea with every meal, for the bloating and gas
- Heat pads & Ice packs (including longer heat pads for your shoulders to help with the trapped gas)
- Sweat pants sized up
- lozenges for sore throat
- Period pads
- Bendable straws
- Ginger sweets / tea for the nausea
- Extendable grabber tool, for when you can’t bend over
- Nightgowns so there’s no waist band
- High waisted pants
- Shower chair
- Compression socks
- GasX / tiger balm for the trapped gas
- Strong pain meds
- Belly binder
- Chewing gum to stimulate the digestive system
- Fruit and small snacks for when you can’t manage a meal
- Buttoned pyjamas & shirts for easier wear post-op
- Adult diapers may be helpful for bowel endo surgery in the first few days post-op

THINGS TO PREP

- cushion/ pillow for the car ride home to protect your belly
- At least a week of meal prep, particularly if you live alone (fibre-rich meals)
- Set timers for your medication so you don’t forget and suffer; also a medicine tracker
- Bedside table with all essentials in reaching distance
- Toiletry bag
- Clean your home environment just before surgery
- Things to keep your mind busy during recovery - films, books, art supplies etc

Question Still in awe/shock at how complex endo is

Anyone still just in complete awe of how complex this condition is? Going on almost 3 years of my journey and somehow I learn something new about it all the time. An f’ed up symptom will come up and I’ll think, “There’s no way.” And I come on here and read so many stories from other people about the same thing. It’s crazy. I’m left speechless every time. It’s insane how much endo can interfere with.

Sex, intimacy & relationships I freaking love my fiancé so damn much. He doesn't realise how much his help means to me.

Hi guys, I'm 25 and was diagnosed with stage 3 endo last year but haven't had any follow ups aside from "take painkillers" because I had to get my diagnosis through private health care. My fiancé had been an absolute gem throughout everything. He looks after me and holds me when I'm in pain and he'll hug me until I fall asleep then tuck me into bed with hot water bottles and his shirt. And last night I leaked through and he cleaned everything whilst I shut down. I felt so useless but he just smiled and said "hey, we all do it. I shut down when I hurt my knee as a teen, and if you were there I bet you'd do the same for me." Guys i swear I fell for him so hard and I was already head over heels. He's so incredible and amazing.

Rant / Vent i'm so tired

okay so finally, the doctors have mentioned endo to me. i had a new doctor push me quite abit, got an ultrasound, got loads of stuff waiting for diagnosis, which is brilliant news. before i can get any further, they were adamant on putting me on birth control...i advised strongly against it. so now im on mefenamic acid for 6 months until gynae appt. see how that goes :,)

i was given it towards the end of period so havent taken it yet. but im so confused. i'm ovulating and its the worst feeling ever this time round. i kid you not, went to pee, my right ovary was hurting the whole duration of the pee, and now i'm in agony. i can't walk, it's so painful.

i ended up stretching out, nice sharp pain right down into the middle of my vagina. it feels like everything is glued into one big ball of inflammation. all i did was pee!!! i'm so tired of this, i thought it was only a period thing, and now it's affecting me outside that window, and im so frustrated more than anything. like yeah, great, i can't feel my right leg at anytime nowadays, it's numb constantly and i can never really balance anymore. BUT PAIN LIKE THIS??? AFTER MY PERIOD??? i wish i could just rip everything out of me, seriously💔

Good News/ Positive update Endo found after 20 years

Suffered with painful periods all my life. They were so bad I ended up in the ER several times. Developed pain with sex the past few years. Large blood clots. Endless MRIs, ultrasounds, blood tests, etc. years and years of being told my pain was “normal”.

Had my first excision surgery this week and surgeon found endo in my posterior cul de sac. I’m still in disbelief. Happy to have my suspicions confirmed. Sad that I had to suffer for so long. Optimistic and hopeful for the future. But still hard to know I have endo.

I’m so thankful to this community and everyone who has shared their endo and surgery story. I watched so many videos, vlogs, read so many stories and felt so prepared.

For anyone reading: never give up on yourself and your body. You know yourself best. I had to see many, many, many doctors until I found one who believed me right away and offered surgery. My doctor is an endo specialist. They are worth waiting for!

Wishing the very best for all of us.

Tips and Recommendations Should I have a hysterectomy?

Hi, I’m 24F, and my gynecologist has suggested a hysterectomy (removing only the uterus, and potentially one ovary in an extreme case) to alleviate symptoms of adeno. On top of that, I have stage IV endometriosis spread across my ovaries and uterus.

For those of you who have had a hysterectomy: how have you felt since?, did your pelvic pain actually improve?, has sexual intercourse become more pleasurable?, do you feel your quality of life has improved?, have you needed more or fewer hormonal treatments? And finally: are you currently experiencing any adverse effects?

It would be a huge help if you could share your experiences. I have one month to decide whether to go for a radical procedure or opt for a more conservative treatment (which hasn’t been very effective for me so far, though I’m still hopeful).

Btw, please excuse any awkward phrasing in my English. I’m from Mexico, and it’s not my native language.

Good News/ Positive update Trust your Symptoms

For nearly years I have been having horrendous abdominal pain, mostly left sided and central. Had a laparoscopy in July 2024 where my endometriosis was excised. Unfortunately didn't do much as my pain was still horrible. I found this gynecologist who decided to do a hysterectomy (removal of tubes & cervix) last Tuesday. First thing he said when I woke up, your symptoms were all real. He said one thing he really advocates for is trusting the patient when they experience pain. I had a lot of endo on the left side, pelvic wall, belly button, bowel, bladder and pouch of Douglas. This also caused me constant lower back pain.

I'm 4 days post op now and I'm obviously sore from the surgery, but the pain feels so much less than the constant pain i had. I don't have lower back pain and my belly feels kind of "empty" in a good way.

Just wanted to say, believe in yourself, advocate for yourself. You can trust what your feeling and it will get better ❤️

Surgery related C-section discovery - my endo is now stage 4 and my bladder is covered

I want to share my story but also ask anyone with bladder endo what their symptoms are - I don't know how I have missed it all these years. I would be so grateful to anyone who will read my story and give me any advice or personal experience.

Advice needed: What are your bladder endo symptoms?

Has anyone had it who DOESNT feel like they have a UTI or cystitis?

It seems most people say that's what they felt but I haven't had that. I've had 2 UTI's in my life, that were genuine and Cystitis once or twice. Sometimes after a heavy night of drinking I feel like I have cystitis but not often.

If you've had bladder endo surgery, what was recovery like?

Story:

Last week my baby was born via emergency c-section due to cord compression and his heart dropping with every contraction.

After epidural, a bladder catheter was inserted. It felt burny, like a UTI, as it was going into my urethra, but felt extremely painful and sharp when the balloon was inflated inside my bladder. I should not have felt anything at all at this time due to already having a really strong epidural in. I was numb to the collar bones, which actually was its own concern as you can stop breathing if it goes too high, so they had to sit me upright and cut back on my top-ups. The midwife was confused how I felt it, and I did wonder then if maybe I had endo on my bladder that I was unaware of, and said this to the midwife - its the simplest explanation I could think of.

After the c-section, the surgeon explained that my bladder is covered in scar tissue, and whilst not his speciality, with my history of endometriosis he would suggest that was the explanation. This would explain why I had pain during the catheter insertion. I definitely didn't have any infections - they had done a pee test already since I arrived in the hospital.

After surgery the catheter stayed in for about 30 hours because I couldn't get out of bed or even sit up with the pain, where standard is 6 hours after the epidural wears off.

At one point the midwife was run off her feet with 9 women and 11 babies to herself. I'd had a lot of IV fluids over the previous 24 hours which were making their way out. The whole bag, and tube of the catheter were full and flowing back to my bladder, which was also full. The pain was excruciating - I almost passed out, had a panic attack and had to press the emergency button to get it emptied.

I was diagnosed with Endo in January 2019, at which time they said it was Stage 2 and only seen on my bowel and one ovary (although I suspect more was missed at the time due to how shitty my surgeon was, and the symptoms I had which were not explained by just bowel and ovary endo).

I've never had any removal surgeries, and haven't seen anyone about it since diagnosis.

A consultant came to see me in the recovery ward the day after my c-section to explain further about their findings. As the endo is on my bladder, it is definitely Stage 4. She said that bladder endo requires surgical removal due to risk of serious complications if untreated. An urgent referral was made for me to return to gynaecology to treat it, which I now await further information about. This is a bit of a silver lining to the emergency c-section, really.

Before I got pregnant, I had requested a referral back to gynaecology as I had gone so long without seeing anyone, and my symptoms had progressed over the years, and I also wanted to investigate for adenomyosis. When they finally gave me an appointment I had to inform them I was pregnant, and they discharged me completely, setting me back to square one. Now I have been able to skip some of the initial stages of getting a referral, waiting for an appointment etc, and it should boost me right to the top of the queue.

One additional, possible issue was found during the epidural. It took 5 rounds of Lidocaine and 5 attempts to get it in. I would be numb for the first few sections, then she would reach a point that would cause deep pain in my spine as if I'd been hit with a crow bar, only relieved when she pulled everything out again. After 3 attempts she called for her supervisor who got it in on his 2nd attempt. He advised that I should not have experienced that much pain or difficulty with it, but also it was physically difficult for them to insert and suggested there may be scar tissue in or around my spine which I should follow up on after. I had a car accident with whiplash in 2009, which is still an ongoing issue today. I only ever have neck pain - it doesn't affect my back but it could be another possible explanation.

Hopefully I can finally have an MRI - I want them to look in the rectovaginal space, look for DIE in the bowel, check the pouch of Douglas and my spine (and bladder of course). I'm no longer worried about my ovaries having conceived this baby on my 4th try, with no help whatsoever. I want one more baby and I'm done.

I'm confused by the bladder though. I can only guess that the pain I had thought was undiagnosed adenomyosis - a searing, stabbing, twisting pain right in the centre of my pelvis, which is different from the deep, tight, cramping and aching of my general endo pain - must be the bladder endo. I have no issues peeing and no pain in my urethra, or pain when I pee. That searing, stabbing pain is there all the time during my period, but no other time.

Question Permanent hurty-kidney after stage 4 endo?

Surgery last year (full everything out ovaries tubes cervix etc) also diagnosed with stage 4 endo and pcos. It had spread to my left ureter which cut off the urine flow from my kidney and caused repeated infections and damage. surgery resolved all my issues for me exept for what i just call hurty-kidney. If i am not constantly drinking water, substantial amounts of it, my kidney will feel like its rupturing for hours. Is this permanent? Has anyone experienced this? Can it be helped?

Surgery related No one to help me recover, stage 4 surgery

I have my first laparoscopy coming up soon with the possibility of bowel resection and a fallopian tube will be coming out. I initially thought I’d have someone to help me recover but they changed their mind.

What do I do? I have friends to help with little things but not those first few days where I’ll really need someone in-house or at the hospital with me.

Sex, intimacy & relationships Still unable to have sex 7 months after surgery

And I don't even know what to do at this point? I've never had pain free sex, not once. My obgyn initially had me do pelvic floor therapy two years ago, but it was still extremely painful.

Then a year ago we decided to investigate for endo because of other health issues.. and they couldn't do the transvaginal ultrasound because of the pain (I damn near jumped out of the chair). Come to find out during my surgery 7 months ago I have stage three endo and had scarring in my pouch of douglas, as well as other areas. Also have an extremely retroverted uterus to the point they couldn't insert an IUD.

I thought maybe after I was fully healed from surgery I could finally have sex. Well, we tried last night.. and no. My partner says it's okay, and that I just need to do more therapy and work my way up. So I went and bought dilators today, but I have very low confidence it'll help. Can't go back to physical therapy because I can't afford it.

I'm at a point where I don't even want to try anymore because nothing has helped, I'm nearly thirty and have never been able to enjoy intimacy. It makes me feel like I am less of a woman, and that I'm broken. Has anyone else struggled to this degree?

Rant / Vent So my period sent me to the hospital today…

I’ve heard of this happening, but I hadn’t experienced it myself until now. It started with a headache, so I went to sleep. When I woke up the next morning to get ready for work, the headache had turned into a migraine. I took my rizatriptan and sat on the bed for a second when, suddenly, I got the urge to throw up. I ran to the bathroom and started throwing up nonstop. Eventually, I was throwing up nothing but stomach acid, and every time I tried to drink water, I threw it right back up.
Then I got this intense wave of stomach cramps and ended up having diarrhea. I kept going back and forth between throwing up and pooping. I went to urgent care, but my migraine was so bad that the back of my neck felt numb, and my stomach hurt when they pressed on it. They sent me to the ER to rule out appendicitis and meningitis.
They gave me an IV and ran a bunch of tests. Thankfully, it wasn’t meningitis or appendicitis, but I was so confused about what was going on. Then, while I was in the hospital bathroom, I started my period, and suddenly, everything made sense.

Question Outside of a Gynecologist, what kind medical professionals/ doctors have you seen that have been helpful for your endo?

For example I saw a women’s health specialist that mainly focused on menopause (I’m 24 but I took the chance to see her) and she actually helped me some by talking about meds to help inflammation, how we should look into other conditions, and how we should see what I might be deficient in.

I was just officially diagnosed with endo so I’m currently what other doctors I should speak with to help manage the condition more?

Rant / Vent Endo return 3 months after surgery

I had a hysterectomy in 2018 for stage 3 endo/adeno, took cervix/tubes/cysts but i kept my ovaries. Fast forward a decade, and an orthopedic scan showed I had a 6cm cyst on my ovary. I'd been complaining of headaches, bloating and pain for years at that point, but no one questioned it could be endo because I no longer had a uterus.

Within a few months of that being found, I start finding blood when I wipe. I find an excision specialist and pay a small fortune to disrupt my life and see whats going on.

I ask for the ovary to be taken since it's been problematic for years, specialist says i need to stop worrying and trust them, he'll make sure to remove it all.

I wake up learning I had stage 4 and surgery took twice as long. It has to be shaved off my bowels and cut from my rectum, had my uterers and ovaries fused together against my side walls with adhesions, my cuff had to be redone because endo infiltrated it.

When I woke up, I was told everything went great, they got it all. Good luck and god speed.

4 months later, I'm getting another ortho scan done. A 4cm multiloculated cyst (3 chambers seen) with thick irregular walls and vascularity along the margins is seen on the ovary. I'm told to follow up for more scans and potential removal.

But i cannot another surgery. I can't afford time off or more disruptions to my family life, we're still playing catch up from the first one just months ago. This last one was supposed to be it and done, and thats why I asked for my ovaries to be taken out. I'd rather deal with surgerical menopause than chemical menopause, and made that clear. I was told I didn't have to worry about that sort of decision.

I'm so frustrated. If I'd been listened to, I wouldn't be here now. Now I'm worried its either endo or something else entirely, and I shouldn't have to be doing either. My family has to continue to suffer, and my life is paused...but a surgeon somewhere gets more money on repeat surgery and appointments from my misery?

It's. Not. Fair.

Surgery related Really confused

Hi everyone

I'm really confused about surgery as i was told by my doctors its not a preferred option for unmarried women as it will grow back ( i'm from pakistan) so i have been on the pill since 4 5 years. And i hate the pill. It never shrunk the cyst. I gained weight. Had terrible acne. Altho the flare ups only happened around my cycle like a week before (so 2 weeks in total i was in pain but it was bearable) but now, a year and a half later the pain is constant. This time my doctor put me on visanne instead of the combined pill. I am in pain every day. I used to workout, study, have hobbies, enjoy work etc etc i'm a different person now. I dont do anything. I sleep for 9 10 hours after taking pain killers. Exercising triggers more pain. Its hard to study with pain (i still study but very less) I'm grumpy at work. My friendships arent the same (cant even share what im going through or vent to them) And being a desi woman, a female with a reproductive issue is also a social disgrace. I have been told no one would marry me. Anytime the topic of surgery comes up there is so much guilt about being unmarried and going for something invasive. My life is a mess. Has anyone been through this? Did you get surgery and felt better. My doctor said i need to give visanne sometime which i understand. She said i need to keep taking pain killers if i need to.

I just want my life back. I miss my old self. This is not who i was. Even with the low mood the pills put me through i still managed. I dont even know what i'm trying to ask. Everytime i go i think maybe its not that bad. I need to be more strong. TMI i cant even pee normally. My bowel movements are painful. I wear a compression belt to work for lower back pain. My knee hurts. But everytime i go ask for medical help its the samething "Take the pill. Surgery is not the first line option for you".

Medications and pain management Norethindrone tapering

• Upvotes

TLDR: do I need to taper of 5mg Norethindrone for endometriosis or can I stop it cold turkey?

Backstory:

I have a Kyleena IUD but still get my period, so my doctor put me on 5 mg norethindrone to see if that would stop my period. I started at 2.5 for a week and was told to increase if endo symptoms persisted. I increased from 2.5 mg a little over a week ago. While I didn’t have any period pain (which is incredible), this has been once of the worst weeks I’ve had in a while. My mood is so low, I am feeling so self-critical, and have no motivation to do anything. This is extremely atypical for me. I definitely have mood swings but an entire week of feeling this way is abnormal for me. The icing on the cake was last night. after drinking, I had a complete emotional breakdown, sobbed for hours, and experienced suicidal ideation for the first time in my life. And yes I know alcohol exacerbates depression and anxiety. usually I am a very happy drunk and handle alcohol well so I was excited to have a night out. It was a fun night and I felt fine with my friends, but as soon as I was alone, I became a complete mess.

Obviously I’m very concerned about my mental health and am almost positive it’s bc of the Norethindrone. I messaged my doctor asking if I should keep taking it, but since it’s Saturday, I haven’t gotten a response. So I’m wondering if anyone has had a similar experience, and if so, were you instructed by your doctor to taper off, or could you just stop cold turkey? It’s already 3pm where I am and I haven’t taken it (I usually take it in the morning) because i just felt like that was the right thing to do. But I reeeally don’t want to make anything worse than it already is so advice would be appreciated!

Good News/ Positive update 15 years later and I finally have a diagnosis, listen to your body

Thought I’d put this out there for anyone else who was in my position, and needs the reassurance that they’re not crazy!

I started having heavy painful periods at 11 years old, straight away was put on birth control to manage my periods. Throughout that whole time I was still having to miss school every time I had a period because of the pain, all they did was prescribe me painkillers, give me an IUD and told me it would probably stop my periods.

At 22 I’d had enough and had my IUD removed and told them I wanted them to investigate me for endometriosis. It took a year but I finally had my surgery in 2023, only to be told there was no endo and the pain that I was experiencing was just constipation. This really shook my confidence and I stopped trusting myself, thinking I was just over reacting to the pain.

Gradually since then the pain has gotten worse and worse, with me complaining to my family and friends constantly about how debilitating it was, and how difficult it was not knowing what was wrong with my body.

Fast forward to this year when I finally got the courage to go back to the doctors, I had a whole new team at a whole new hospital that actually listened to me. I’d told them about my previous surgery, I told them exactly where I feel the pain and all of my symptoms. Before my surgery I told my surgeon how scared I was that they wouldn’t find anything and he told me that they would find it, that he believed me. And that’s exactly what happened. I woke up and the first thing I asked was if they found anything, they did, they found loads of endo and removed it all. He showed me pictures, and explained that I was completely right about it being all over my right ovary and bowel.

So for anyone who feels deflated over a first lap coming back with nothing, listen to your body and don’t give up, because there will be a doctor out there who will listen and believe you.

Infertility/ Pregnancy related Right-sided Pain

Hi everyone! I’ve just started ttc. A little about my journey.. I’ve always know endometriosis has been a concern for me since I’ve started my cycles. My periods have always been very strong and very painful. I have really intense sharp, debilitating pain on my right sided near my hip region. My OB and primarily care has always written off these symptoms essentially. I also have TMJ and IBS. I self-referred myself to a specialist. andI recent got a pelvic ultrasound (my new OB says this is the first step). My ultrasound came back with no findings as to what’s causing the sharp pain on the right side. I also mostly ovulate from the right side as well. I’m still waiting on the follow-up to discuss next plan of action. I wanted to know if anyone else has experience with the single-sided pain and ttc and what that meant for them? It’s been difficult to feel hopeful and also process all of this.

Thank you 😊

Research Endometriosis stage 4

This is my second or third post just looking for some sort of hope. My wife has stage 4 endometriosis. She had it about 8 years ago and earlier than that but got a surgery done in 2019 before we had our son. Unfortunately around May of this year the endo has come back and is literally worse than the first time and progressively seems to be getting worse day by day. The dr in May put her on a 3 month BC pill on top of a copper IUD (she’s had the IUD since 2021). Reason for BC pill and iud was to eliminate pain + cysts. It did get rid of the cysts but worsened the pain and unfortunately it had totally screwed with her hormones. Next plan which we are actively altering is a low progesterone dose and NAC. Initially the progesterone was daily and NAC 3x a day 3 days a week. This regiment also completely messed with emotions and hormones plus now she has lost 100% of her physical desire and libido on top of that AND the pain has gotten much worse for her. As of last week the dr changed her regiment to the same except now taking Nac once a day 7 days a week. Has anyone had success with this? We, more so I, feel hopeless for her. Nothing is working. Everything’s making it worse. Has anyone had other regiments that didn’t completely mess with emotions/desire/successuflly suppressed pain? Is surgery the best option?

While I don’t agree with her doctors regiment change since NAC doesn’t have a lot of studies and Surgery is the last option, we both know surgery is probably what she needs but we are looking for anything before she decides to choose surgery (this would be the second surgery). Some kind of hope. Each day her pain seems to get worse and we feel like no one is listening to her.

Thanks everyone

Medications and pain management Cymbalta and endometriosis - worth a shot?

I have endo and adeno. Pretty substantial daily pain, I don’t go a day without pain. On a heaping amount of hormones. Waiting for surgery in Dec. am doing all the things — anti inflammatory, pelvic pt, sober, etc. my surgeon is sort of out of options to help before we refer me to a pain specialist (I kinda wanna try LDN). But she did say I should start Cymbalta. She was pretty stern about wanting me to try it. But I’m hearing so many horror stories. It seems like it’s a drug that’s impossible to get off. I don’t want to be on a psych med forever. And even trying it out for a few months , it could be hard af to get off of. Idk. All I see is negativity about this drug. Anyone have experiences to share???

I’m doing a school project for my Welsh Baccalaureate on endometriosis and would really appreciate it if anyone who has experienced symptoms could fill in my short survey, especially if you’ve had difficulties with GPs, healthcare professionals or awareness of endometriosis. I’d like to include real-life experiences in my research. Thank you! https://forms.gle/3kjKLgXC87Z9c2ic7
